Help Referral Form 1 Receive the Help Referral Form from the voter
Acceptable Photo ID 2 1. Ask the voter for acceptable photo ID. If the voter does not have acceptable photo ID, explain the options for voting (No Acceptable ID tab explains options in the NC Voting Site Station Guide)
**The deadline for requesting absentee ballots is May 31, 2016**
3 Provisional Poll Book In the provisional poll book, you will need to complete the following: Derek Bowens/322 Hibiscus Way ROO2 1. Enter the voter s name and address as seen on the Help Referral Form 2. Enter the ballot style of the voter
4 Provisional Envelope Provisional envelopes can be found in the red suitcase. The following will be completed on the provisional envelope. 1. Complete the Poll Worker Complete section for the BOE
Reasonable Impediment Form 5 1. If applicable, complete the administrative section of the reasonable impediment form. If a reasonable impediment form is required, it SHOULD be indicated on the help referral form. Even if it is not written on the help referral form, be sure that the voter is not at the help station due to a reasonable impediment (See Reasonable Impediment tab in the NC Voting Station Guide).
Pin Labels 6 Affix the pin labels to the poll book log, provisional application, and provisional instructions. Pin labels may be found in the provisional poll book Derek Bowens/322 Hibiscus Way ROO2
7 Provisional Voter Application 1. The voter will need to complete the voter registration/update section of the form and sign. The voter will also need to choose a reason for voting provisionally. If the voter does not provide acceptable ID, the voter must also notate that he or she received their alternative voting options (First check box under the list of provisional voting reasons).
8 Reasonable Impediment 1. If the voter indicated a reasonable impediment during step 5 of the provisional process, the voter must complete and sign the reasonable impediment declaration. If the voter does not provide alternative identification, the precinct official must check the box indicating that the voter did not provide alternative identification. The precinct official must also initial this form confirming the information provided by the voter.
Accept and Review 9 Once all of the preceding steps have been completed, accept all forms from the voter and review. Be sure to sign or initial forms where required.
Acknowledgement Forms In the provisional book provided to each help station, there will also be voter acknowledgement forms. These will be used for voters that do not have acceptable ID and do not desire to vote provisionally. This form will ensure that voters who present without acceptable ID have been informed of all options available (More details on voting options can be found in the Help Station section of the NC Voting Site Stations Guide).
10 Evaluate Photo ID If applicable, review the acceptable photo identification provided and determine whether the photo identification bears reasonable resemblance to the voter. 1. Ensure that the photo on the identification provided depicts the voter 2. Determine that the name on the identification is substantially equivalent or the same to the name on the provisional poll book. **If you are unable to determine reasonable resemblance, contact the Chief Judge and Judges**
11 Notate Provisional Application Notate at the bottom of the provisional application the type of identification provided if applicable. This section will have already been signed by the precinct official under step 9. It is imperative that you note whether the provisional voter provided acceptable ID.
Obtain the Ballot 12 PROVISIONAL FP02 It is critical that this process is completed! Once all steps have been completed by the voter, complete the following: 1. Obtain the appropriate ballot from the distribution 2. Notate at the top of the ballot the word Provisional and write the precinct code beside it (ex. FP02)
13 Voting the Ballot PROVISIONAL CF02 1. Provide the voter with the notated ballot and provide the voter with verbal instructions on voting. Voter should sit behind yellow privacy screen. 2. Provide the voter with the provisional envelope. Instruct the voter to place the voted ballot into the provisional envelope and seal when completed (Sealer will be provided).
14 Voter Instructions 1. Provide the voter with the instructions that you previously labeled and provide the voter verbal instructions on providing acceptable ID, if applicable, to the county Board of Elections by 12:00 PM on the day prior to the county canvass. Just read this statement if a voter needs to show ID at the Board of Elections!
Used Forms 15 1. All forms used during the provisional process should be placed in the plastic sleeve on the exterior of the provisional envelope. At a minimum, forms should include: Completed Help Referral Form Completed Provisional Application Completed Reasonable Impediment Form (If Applicable) **Completed Acknowledgement of Voting Options forms should be placed in the accordion folder**.
16 Place Ballot Away 1. Place the sealed ballot in the red polybag designated for provisional ballots. Do not seal the red polybag until voting has ended.
What do I do? If you are ever unsure of what to do at the Help Station, reference your NC Voting Site Station Guide. In the alternative, you may notify your Chief Judge who may have to obtain further guidance from the Board of Elections.
