SUBJECT: FACILITY LEASE AGREEMENT EXTENSION YUCAIPA SENIOR NUTRITION SITE RECOMMENDATION: That City Council approve and authorize a one-year extension of the Facilty Lease Agreement Yucaipa Senior Nutrition Site pursuant to Section 3(B) of the Agreement with Family Service Association. P. 45 3. SUBJECT: SPECIAL EVENT PERMIT APPLICATION BICYCLE SAFETY RODEO RECOMMENDATION: That the City Council: 1. Approve the Special Event Permit Application, submitted by the Yucaipa Police Department, to conduct a Bicycle Safety Rodeo on Saturday, June 2, 2018, at the Yucaipa Community Park; and 2. Agree to co-sponsor the event, waive the $16.50 Special Event Permit Application fee, authorize the use of the City of Yucaipa s logo on marketing material, and assist with the marketing and promotion of the event. P. 53 4. SUBJECT: AUDITING SERVICES AGREEMENT 1. Approve and authorize the Mayor to execute a Professional Services Agreement with Van Lant & Fankhanel, LLP, for auditing services for the fiscal years ending June 30, 2018, 2019 and 2020 in the amounts of $29,900, $29,900 and $30,850, respectively, for the General City and Single Audit reports; and 2. Direct staff to include sufficient appropriations in the Proposed Budgets for fiscal years ending June 30, 2019, 2020, and 2021 to ensure the total costs of the agreement are allocated. P. 72 5. SUBJECT: SEAMLESS SUMMER FEEDING PROGRAM AGREEMENT BETWEEN THE CITY OF YUCAIPA AND THE YUCAIPA-CALIMESA JOINT UNIFIED SCHOOL DISTRICT 1. Approve the Seamless Summer Feeding Program Agreement between the City of Yucaipa and the Yucaipa-Calimesa Joint Unified School District, to be held Monday through Friday, from June 11, 2018 through August 3, 2018, at Yucaipa Community Park and 7th Street Park; and 2. Authorize the City Manager to execute the terms of the Agreement. P. 83 6. SUBJECT: CANCELLATION OF MAY 30, 2018 COUNCIL MEETING Page 2

RECOMMENDATION: That City Council cancel its regularly scheduled meeting of May 30, 2018 and hold a Special City Council meeting on May 29, 2018. Receive and file the 2018 Raise the Roof Rodeo Event Wrap-Up; and 2. Accept the $30,000 contribution and allocate the funds toward the Dottie Potter Arena Cover. P. 88 8. SUBJECT: AWARD OF CONTRACT UPTOWN PARK GRADING PROJECT, PROJECT NO. 11001 1. Award and authorize the Mayor to execute a contract in the amount of $147,498, with Three Peaks Corporation of Calimesa, CA for the Uptown Park Grading Project, Project No. 11001; and 2. Approve the amount of $15,000 for construction contract contingency and the amount of $20,000 for construction contract administration, survey, testing and inspection, for a total construction budget in the amount of $182,498; and 3. Approve the Plans and Specifications prepared for this project; and 4. Accept the letter of Bid Withdrawal from James McMinn, Inc. P. 94 9. SUBJECT: ENGINEERING SERVICES WILDWOOD CANYON ROAD INTERCHANGE PROJECT STUDY REPORT AND NEW CONNECTION REPORT 1. Award and authorize the Mayor to execute a contract with HDR Engineering, Inc., in the not to exceed amount of $596,330 for professional engineering services for the preparation of the Wildwood Canyon Road Interchange Project Study Report and New Connection Report; and 2. Approve the amount of $120,000 for contract contingency, the amount of $300,000 for staff time, contract administration and project coordination, and the amount of $300,000 for Page 3

Caltrans oversight and quality assurance for a total not to exceed project cost in the amount of $1,316,330; and 3. Authorize the City Manager or his designee to administer the terms of the contract; and 4. Amend the FY2017/2018 Capital Improvement Program to identify the amount of $250,000 in SBCTA funds for the project P. 112 10. SUBJECT: AWARD OF CONTRACT UNDER DRAIN REPAIR- CASA VISTA STREET AND ESTACIA LANE- PROJECT NO. 11072 1. Award and authorize the Mayor to execute a contract, and authorize the City Manager or his designee to administer the terms of the contract to the lowest responsible bidder, GLC Landscape and Engineering of Yucaipa, CA. in the amount of $16,500 for the Under Drain Repair- Casa Vista Street and Estacia Lane- Project No. 11072; and 2. Authorize the amounts of $3,000 for construction contract contingency and $3,000 for construction contract administration, staff time and inspection, for a total construction budget in the amount of $22,500; and 3.
