PAKISTAN ISLAMABAD POST PARTNER VISA APPLICATION CHECKLIST (SUBCLASS 309/100) This checklist helps to ensure that you have included all of the necessary documentation that is needed by the Australian Mission to process your visa application. How to use this checklist 1. Before you sign your visa application form, use this checklist to ensure that you have included all necessary documentation to support your claims. 2. When you are satisfied that you have correctly completed your visa application form and all of the necessary documentation is included, you should sign and date your visa application form. 3. Include this completed checklist, when you lodge your visa application form and all necessary documentation. Other important information Please be aware that the decision on your visa application may be decided on the information and documentation that you include at the time of lodgement. Delivery and courier charges If you are requested to provide additional information, you will be responsible for the costs of delivering this information to the Australian High Commission Islamabad or Australian Consulate Dubai by mail or courier, including any additional information that may be requested by the relevant Australian Mission. If you give this requested additional information to the Australian Visa Application Centre, you may be charged a courier fee for delivery to the Australian Mission. If you submit additional documents without a request from the relevant Australian Mission at the Australian Visa Application Centre, you are responsible for all delivery and courier charges. About the documentation that you include: Documentation provided will depend on your personal circumstances. Visa Application Charge Note: All fees and charges must be paid by bank draft in Pakistani Rupees, made payable to the 'Australian High Commission, Islamabad'. Payment cannot be made by cash, personal cheque or credit cards. The current application fees and charges are published on the DIBP website http://www.border.gov.au/trav/visa/fees. The DIBP website also contains a currency converter to assist you to convert the current charges and fees in Australian Dollars to Pakistani Rupees. If the Visa Application Charge has been paid at a DIBP office in Australia, a copy of the Page 1 of 5
receipt must be submitted with your application. Copies of the biodata page of passport (s) of all people included in the application Four recent passport-size photo for applicant and two recent passport-size photo for sponsor Certified copy of applicant(s) national identity card Urdu and English (translated and attested) Certified copy of applicant(s) computerised birth certificate issued by Union Council/Cantonment Board/Capital Development Authority or Municipal Corporation, in Urdu and English or the National Database and Registration Authority (NADRA). Penal certificate from countries where the applicant has resided for 12 months or more during the last 10 years (applicable to applicants 17 years old or over). Note: You must provide the original of any police/penal clearance certificates. Certified copies are not acceptable. In Pakistan a Character Certificate should be obtained from the District Police Officer (The Head of a Police District who is not below the rank of Senior Superintendent of Police) in your local Police Station. If you and your partner or any other migrating dependents included in your application are married, the official marriage certificate(s) i.e. Nikkah Nama Urdu, Nikkah Nama English (translation) and marriage registration certificate issued by the National Database and Registration Authority (NADRA) is required. Evidence that you and your partner genuinely intend to remain in a genuine and ongoing relationship. This may include but is not limited to: Evidence that you and your partner share financial commitments and responsibilities (e.g. joint ownership of assets, sharing of finances/bills/expenses, joint loans, bank accounts, legal commitments that you and your partner have undertaken as a couple etc). Evidence that you and your partner share responsibilities within your household (e.g. Joint utilities accounts, joint responsibilities for children, correspondence addressed to you both at the same address, evidence attesting to your living arrangements etc). Evidence demonstrating how your relationship is seen by your friends and family (e.g. Evidence that you and your partner are generally accepted as a couple socially, joint invitations, outings/ travel together, photos, statutory declarations from friends and relatives who know you as a couple, evidence of Joint participation in cultural/social/other activities etc). Evidence demonstrating the mutual commitment between you and your partner (e.g. correspondence/email communication and itemised phone accounts to show that contact was maintained during periods of separation etc). Form 888 - Statutory declaration by a supporting witness relating to a partner visa application. At least two statutory declarations, preferably from individuals who are Australian citizens or permanent residents and who have personal knowledge of your partner relationship (such as relatives or friends) and support your claim that the relationship is genuine and continuing. Signed, written statements from you and from your partner detailing the history of your relationship (for example when and how you met, when you became engaged, joint activities, significant events in the relationship) and your future plans together. If you are applying on de facto grounds (not legally married): Evidence that your de facto relationship has existed for the entire 12 months before lodging this application, that you are both aged 18 years or over at the time you lodge this application and evidence that your relationship is registered under a prescribed law of an Australian state or territory. years or over Form 956 Advice by a migration agent/exempt person of providing immigration assistance Please fill in this form if you want a migration agent or other authorised person to receive communications about your application with the department. If you have served in the armed forces of any country, certified copies of military service records or discharge papers. If there are migrating dependents 18 years of age or over, evidence that they are financially dependent on the main applicant for the basic needs of food, shelter and clothing, and how long this support has been provided. Evidence may include bank statements, money transfers, rent receipts etc. If you, your partner or any other migrating dependents included in your application has been, divorced, widowed or previously married, a statutory declaration/separation certificate, divorce decree absolute, death certificate of the deceased partner or marriage certificate (as appropriate) are required. If you, your partner or any other migrating dependents included in your application has changed his or her name (for example by marriage or deed poll), evidence of the name change is required through official NADRA documentation. Documents to verify custody and access arrangements, for children under 18 years of age unless it is the child of you and your partner. Note: this can be Divorce memorandum, Divorce deed or death certificate if one of the child s parent(s) is deceased. If any child included in your application is adopted, a certified copy of the adoption papers is required. A sworn statement before a Notary public from the non-migrating custodial parent or legal guardian of each child consenting to that child migrating permanently to Australia with you. A certified copy of this parent s passport or identification card must be provided. If you want to bring a child under 18 years of age to Australia, and that child s other parent is not included in the application, documentary evidence that you have the legal right to include that child in your application must be included. Evidence may include, but is not limited to: A completed 1229 form consent form to grant an Australian visa to a child under the age of 18 years. Certified copies of official legal documents such as a court issued custody, access or guardianship order A statutory declaration from each person with a legal responsibility for the child, stating that they have no objection to the child migrating to Australia. Supporting documents specific to the Sponsor Two recent passport-size photographs of the sponsor. (Taken in the last six months). Evidence that your sponsor is an Australian citizen, Australian permanent resident or eligible New Zealand citizen, such as: Australian passport or foreign passport containing evidence of permanent residence Evidence that your sponsor usually resides in Australia For New Zealand citizens; evidence of length of residence in Australia and of continuing links with Australia. A statement from your sponsor regarding any other person they have previously sponsored or nominated for a Spouse, Partner, Prospective marriage or Interdependency visa or any other person for whom your sponsor has signed maintenance order and/or Assurance of Support. Note: The statement must indicate the nature of the sponsor s relationship with this other person or persons, the date the sponsorship was lodged and the date the relationship ceased If there is a child under 18 years of age included in the application, the sponsor must provide the following: An AFP (Australian Federal Police) National Police Check, if the sponsor has lived in for more than 12 months or more (cumulatively and continuously) in Australia since turning 16 years of age. Police certificates from each country in which the sponsor has lived in for more than 12 months or more (cumulatively and continuously) in the last 10 years since turning 16 years of age. It is recommended that the sponsor provide the following documents, as they may be requested by the department at a later time: An Australian Federal Police National Police Check, if the sponsor has ever travelled to Australia (this is irrespective of the amount of time spent in Australia). Police certificates from each country in which the sponsor has lived in for more than 12 months or more (cumulatively and continuously) in the last 10 years since turning 16 years of age. Note: The Form 40SP will request the sponsor s consent to disclose details of certain criminal convictions to the visa applicant(s). A failure to consent may result in a visa application being refused. Evidence of polio vaccination If you have spent 28 days or longer since 5 May 2014, in any of the countries listed below, or in any combination of those countries, you should provide your current certificate of vaccination against Polio.
