ALMA SUMMARY REPORT: 2 ND QUARTER 205 Introduction The month of July 205 sees Ethiopia and the United Nations Economic Commission for Africa hosting the 3 rd International Financing for Development Conference, less than 7 days before the end of the Millennium Development Goals period. Convened in Ethiopia, the seat of the African Union, the conference theme is a clarion call pronouncing that it is time for global action. This is a fitting recognition of the concerted action required to address the challenge facing the AU and ALMA member states, as they gear up to realize the continent s vision of an Africa free of malaria. By the end of this year, the continent will have seen an over 60% reduction in malaria mortality since 2000. The gains that have been made, with at least nine countries on track to have achieved 75% reduction by the end of 205 and an additional three countries on track for at least 50% reduction, will need to be protected and the progress not only sustained, but accelerated in the next phase of the fight against malaria; the elimination battle. Malaria elimination In January this year, the ALMA heads of state and government and the AUC Chair launched an all Africa Elimination Scorecard. The scorecard launch built on the leadership demonstrated by SADC, who in May 204, launched the SADC Malaria Elimination Eight (E8) Scorecard for the E8 eliminating countries. This approach is in line with the AU s Agenda 2063 and The Africa we want. The SADC Malaria Elimination Eight (E8) Scorecard was updated this quarter, one year after its initial launch. The sub-region has some important lessons to share with the rest of the continent. The most important is that business as usual will not deliver elimination. Many of the E8 countries experienced an increase in malaria cases in 204 as compared to 203. Initial analysis shows that whilst some of these increases were due to better reporting of cases, the rise was primarily due to: increased rainfall within the sub-region leading to higher levels of malaria transmission cross-border movements insufficient indoor residual spraying coverage in some key districts The Ministers of Health from the region have committed to intensifying efforts to ensure coordinated action against malaria across the region. Turning the commitment to action, under the leadership of the ALMA Chair, His Excellency Prime Minister Hailemariam Dessalegn, an Africa roadmap to malaria elimination is under development with the support of WHO AFRO, the AUC and ALMA. This will guide the countries in Africa along the pathway to elimination by 2030. Countries are also planning to develop malaria control and elimination scorecards, with the Chair of the Elimination 8, Swaziland, and the Chair of ALMA, Ethiopia, leading the charge. A critical aspect of this response that will require careful planning and concerted effort by all countries and partners is financing. Financing malaria elimination There is consensus amongst countries and partners that financing of the Sustainable Development Goals will require a transformation in development financing. One of the biggest growth areas for financing the sustainable development agenda is Africa s domestic resource mobilization potential and effective management of public resources. This was a major area of focus during the 3 rd International Financing for Development Conference, seeking to increase Africa s fiscal space. From Billions To Trillions: Transforming Development Finance Post- 205 Financing For Development: Multilateral Development Finance, a paper produced jointly by the seven multilateral development banks, posits that the change in mindsets, approaches and accountabilities is what will drive the change and generate the level of resources required for the Sustainable Development Goals. A priority area identified in the paper is human development; including health and well-being. Investment in malaria falls squarely in this category. To ensure that universal coverage is maintained, and that every household in rural areas and across borders is reached, the Global Fund E8 regional proposal has moved to grant making and will begin implementation. The enhanced cross border collaboration, surveillance and cross border monitoring of malaria will ensure:. Equity in investment and outcomes 2. Sustainability in the gains in the fight against malaria, limiting cross border infections 3. Empowerment of populations so that malaria related absenteeism from work and school is reduced, enhancing the quality of human capital and 4. Securing development opportunities for the poor, by opening up previously malaria endemic areas for income generating investments Many countries will need to ensure the full implementation of community case management to assist in the elimination efforts. Countries such as Rwanda and Ethiopia are leading the way in this endeavor. The SADC Elimination 8 countries aim to achieve the same outcome by investigating every case at facility level. 3

There was recognition that the fight against malaria has been one of the great success stories of the Millennium Development Goals era, with more than 6 million deaths projected to have been averted between 2000 and 205. Building on this success in Africa, 40 countries have submitted their Global Fund New Funding Model malaria concept notes. The three remaining countries eligible for Global Fund resources will be submitting costed extensions. This means all countries in Africa have now submitted their concept notes for malaria. This tremendous effort translates into approximately US$3.5 billion with over US$.8 billion already approved by the Global Fund board; whilst the majority of the remainder are in grant making. Additionally, across the three diseases, there has been a more than 50% increase in domestic resource commitments from endemic countries to HIV/AIDS, TB and malaria. This represents an increase of more than US$ 4.5 billion, increasing to around US$ 3 billion. Her Excellency Nkosazana Dlamini Zuma called on countries to commit to winning the last mile to malaria elimination. The lap has started, with countries throughout Africa maintaining their 4

Global Fund malaria allocation. This means that the resources allocated at the beginning of the post 205 era, are sufficient to sustain the gains made in recent years; reflecting the continued commitment of countries in Africa and the invaluable support of the Global Fund to the fight against malaria. Next steps The UN Secretary-General, Mr. Ban Ki-moon, observed that the global community has the tools required to attain elimination, as outlined in WHO s Global Technical Strategy for Malaria and the Roll Back Malaria Partnership s Action and Investment to Defeat Malaria." The call to commit to financing this agenda was directed by the ALMA Chair; H.E. Prime Minister Hailemariam Dessalegn towards:. Countries to increase budget allocation and overall domestic financing 2. The private sector to recognize the centrality of human wellbeing to their profit margin by committing to financing the last mile 3. The global community to ensure that there is a successful replenishment of the Global Fund to fight HIV/AIDs, TB and Malaria 4. All partners to ensure that the disease split for malaria in the Global Fund is maintained at 32% so that the tremendous gains made in controlling malaria are not lost The ALMA heads of state and government applaud the Global Fund for supporting countries with short duration grants to secure the resources they need for 207. The lessons learned from the MDG period including those introduced by the new funding model, must help inform the post 205 malaria elimination financing. Conclusion pushing malaria off the ledge The fight against malaria has reached a tipping point at a crucial time, for the global community. As we enter the sustainable development period post MDGs; at last we have a real opportunity to eliminate malaria. We know how to do it, and new tools that will accelerate progress are already in development. The world can at last relieve hundreds of millions of the world s poor of a burden that has robbed communities of income, well-being and self-actualization. Africa cannot afford to let this opportunity go and ALMA heads of state and government, their ministers and people guided by the AU vision and roadmap under development; stand ready to partner with both old and new friends to make the Africa that we want; free of malaria, a reality. 5
