HEMPFIELD TOWNSHIP VOLUNTEER CLEARANCE PACKET Complete Guide & Forms for all Act 153 Clearances
INTRODUCTION Thank you for your interest in volunteering with Hempfield Township. We strive to create a safe environment for our staff, volunteers, and participants. All adult volunteers, no matter what capacity they will serve, are required to follow and submit all clearances in accordance with the new Pennsylvania Act 153. Those volunteers under the age of 18 are not required to obtain their clearances. It is the highest priority of Hempfield Township to ensure the safety and well-being of all citizens. In order to provide a safe and secure atmosphere for our community, all employees, contractors, and volunteers will be screened. All information received from the background check applications and reports will be used for the purpose of determining applicant s eligibility as a volunteer. The following information is only needed if we do not have up-to-date clearances or waiver on file dated within the past 60 months. Clearances will need to be renewed every 60 months from the date of your oldest clearance. Which certifications are needed? All adult volunteers must obtain the following certifications: CLEARANCE 1 Pennsylvania State Police Background Check directions found on Page 3 CLEARANCE 2 Pennsylvania Child Abuse History Clearance directions found on Page 3 CLEARANCE 3 FBI Federal Criminal History Clearance select one of the following: CLEARANCE 3A FBI Clearance Waiver Form to be completed if the volunteer has been a resident of the Commonwealth of Pennsylvania for the entirety of the previous 10 years waiver form found on page 8 CLEARANCE 3B FBI Fingerprinting to be completed if the volunteer has NOT been a resident of the Commonwealth of Pennsylvania for the entirety of the previous 10 years directions found on Page 5 Please follow the directions on the indicated pages above to complete the needed clearances. The last page of this packet is the only page needed to be turned back in to the appropriate Hempfield Township office. NO VOLUNTEER WILL ACT IN ANY CAPACITY UNTIL ALL CLEARANCE CERTIFICATES/WAIVERS ARE ON FILE WITH THE APPROPRIATE HEMPFIELD TOWNSHIP DEPARTMENT. 2
STEPS TO COMPLETE YOUR CLEARANCES CLEARANCE 1 - Pennsylvania State Police Background Check This clearance can be obtained by completing the last page of this packet (page 8) and return to Hempfield Township. The Township will be responsible for running this clearance. CLEARANCE 2 - Pennsylvania Child Abuse History Clearance This clearance can be obtained by completing the following steps. It must be completed by the volunteer. Hempfield Township will provide you with a payment code, so they can obtain a copy of your clearance upon completion. Please use the payment code provided or contact Hempfield Township to receive a payment code. You DO NOT have to submit your paper certificate to the Township if you use the given payment code. We will receive a copy. 1. Navigate to the CWIS Child Welfare portal (also referred to as the self-service portal) by entering the following URL in your browser: https://www.compass.state.pa.us/cwis. 2. Click CREATE INDIVIDUAL ACCOUNT 3. On the General Information screen, scroll down and click NEXT 4. On the Profile Information screen, enter the requested information: a. Pick a unique ID for your Keystone ID (Note: if you pick a generic user ID, i.e., JohnSmith, it will likely already be in use) b. Be sure to enter a valid email address; this is where the temporary password and Keystone ID confirmation emails will be sent c. Click FINISH. 5. On the next screen, click CLOSE WINDOW 6. You will receive an email with your userid. You will also receive an email with a temporary password. 7. Navigate back to the CWIS Child Welfare portal (also referred to as the self-service portal) by entering the following URL in your browser: https://www.compass.state.pa.us/cwis. 3
8. Click INDIVIDUAL LOGIN 9. Click ACCESS MY CLEARANCES 10. On the Learn More screen, scroll to the bottom and click CONTINUE. 11. On the Keystone Key screen, enter the user ID and temporary password you received in the emails in the Username and Password fields. Remember to copy and paste the temporary password from the email to avoid typographical errors that would cause your login to fail. 12. Click LOGIN. 13. On the Set Permanent Password screen, enter a new password (note: be sure to follow the password requirements listed below the fields on the screen). 14. Click SUBMIT. 15. On the Keystone Key screen, enter your Username and (permanent) Password. 16. Click LOGIN. a. Note: Be sure to use the permanent password you created in previous steps. If you login five times incorrectly, your account will be locked. If locked, click the FORGOT PASSWORD button and follow the reset/unlock password procedures contained later in this document; alternatively, you may wait 30 minutes and your account will unlock. 17. On the My Child Welfare Terms and Conditions screen, read the terms and conditions and click the I have read, fully understand and. radio button to confirm acceptance. 18. Click NEXT 19. On the Learn More screen, scroll to the bottom and click CONTINUE. You will now be able to login to the Child Welfare Portal. 20. Click CREATE NEW APPLICATION. 21. Click BEGIN. 22. Select Volunteer Having Contact with Children 23. Select OTHER from dropdown of Volunteer Category 24. Type Hempfield Township in box for Agency Name 4
25. Click Next 26. Follow through the multiple pages of the application. 27. Click NEXT at the end of each section. 28. Check box next to I hereby certify 29. Type in your name as in the Signature box as it appears at the beginning of your application 30. Select Yes for payment code 31. Enter Payment Code: a. Click Submit Application CLEARANCE 3 FBI Federal Criminal History Clearance HAVE YOU BEEN A RESIDENT OF THE COMMONWEALTH OF PENNSYLVANIA FOR THE ENTIRETY OF THE PREVIOUS 10 YEARS? IF YES TO THE ABOVE QUESTION: This clearance is not necessary, but you must complete CLEARANCE 3A FBI Clearance Waiver Form. This waiver can be found on the last page of this packet (page 8) and must be submitted prior to serving as a volunteer. IF NO TO THE ABOVE QUESTION: This clearance is CLEARANCE 3B FBI Fingerprinting. It can be obtained by completing the following steps. It must be completed by the volunteer. Contact Hempfield Township Parks & Recreation to obtain billing password. 6. Select EMPLOYMENT for reason fingerprinted 7. Complete Form 8. Click NEXT 9. Verify information. 10. Click NEXT 11. Print Completed Applicant Registration. 12. Take Registration ID to a finger printing location. The closest location is the: UPS Store 645 E Pittsburgh Street, Greensburg, PA 15601 NOTE: Make sure you provide us with a copy of your clearance when you receive it in the mail.
