Please state previous matters/convictions (case/year of conviction) 1. Hereby agree to abide by the Blanco Neighbourhood Watch Constitution, Western Cape Provincial Constitution and Code of Conduct for Neighbourhood Watch Structures. 2. I hereby grant the BNW permission to advise SAPS and the CPF of my membership status, details will be limited to call sign, name and address, no e-mail address or cell phone will be forwarded. 3. This Code of Conduct shall be binding on the Blanco Neighbourhood Watch structures and all the members. 4. Blanco Neighbourhood Watch and all its members must at all-times act in a non-violent manner. No member may take the law into his/her hands irrespective of the circumstances. 5. No Member of the Blanco Neighbourhood Watch shall display any racism, sexism or any other form of discrimination towards any member and/or person. 6. The Blanco Neighbourhood Watch shall not allow any member to carry any weapon/s perceived to be dangerous and that could inflict serious body harm and/or damage to property, this includes Dangerous Weapons as defined by law, when patrolling. Only weapons for the sole purpose of Self Defence, as approved by the Provincial Commissioner, South African Police Services, maybe carried. These weapons have to be concealed at all times. (Tonfa/pepper spray/baton/handcuffs/restraining aids) 7. Should a Member be found guilty of a criminal offence, the Blanco Neighbourhood Watch must respect the decision of the court/s and immediately institute disciplinary measures. The Blanco Neighbourhood Watch shall issue a public statement to it members, in regards to its action. 8. I acknowledge that at any stage during my valid membership term the Blanco Neighbourhood Watch may conduct an additional police clearance search, at the Watches own cost and without my prior knowledge, to ensure the compliance of their members. 9. No member is permitted to have dual membership of another Neighbourhood Watch Structure or watch related organisation within the precinct of the Blanco and George SAPS. Unless otherwise approved by the Sector Management Committee and the EXCO of the Blanco Neighbourhood Watch. 10. SAPS, SAPS Reservists, Law enforcement, Law enforcement Auxiliaries, Metro and Traffic may become members of the BNW but may not serve on any management committee, Sub-Committee, and/or Executive committee. 11. As a member of the organisation I acknowledge that any confidential information that is shared amongst members via our Radio Network, e-mail, instant messaging, telephonically etc. is not to be disseminated to non-members and Media. Should it be determined that I have transgressed this confidentiality, it will be deemed a negligent act and can result in my immediate suspension and possible dismissal and expulsion from the Blanco Neighbourhood Watch. 12. As a member of the Watch I may not communicate with the media and/or claim to represent the Watch on any channel. All press and media enquiries must be redirected to our Media liaison at pr@blancowatch.co.za or our Chairperson at chairman@blancowatch.co.za 13. As a Blanco Neighbourhood Watch member you hereby agree to abide by the principals, policies and Procedures of the Neighbourhood Watch regarding social media platforms. 14. The radio frequency utilised by the Blanco Neighbourhood Watch is the express property of the Watch and on cessation of the membership I acknowledge that I have 3 days to return my radio or to have the radio frequency decommissioned. I note that failure to comply with this can and will result in a costly legal remedy. Any cost incurred will be for my account. 15. Under no circumstance may I lend or permit a non-member to utilise my radio except immediate family members that reside at my premises and then only in an emergency. 16. I will in the execution of my voluntary service to the residents and businesses in the area act within the framework of the law, the Blanco Neighbourhood Watch and the Western Cape Provincial Constitution and Code of Conduct for Neighbourhood Watches structures. 17. I am aware that I am personally liable for my actions during such voluntary service and indemnify the Blanco Neighbourhood Watch, its Management or Agents against any consequences arising out of my actions. 18. No Member may accept any payment, commission or gratuity in connection with his her membership of the Blanco Neighbourhood Watch, except after being duly authorised to do so by the Executive of the Blanco Neighbourhood Watch. 19. Notwithstanding the above, should any payment, commission, gratuity or donation be offered to the to the Blanco Neighbourhood Watch, the Treasurer of the Executive, after accepting this and acting in the best interests of the Blanco Neighbourhood Watch should notify the Executive of this and include this in the monthly financial reports. 2 P a g e I n i t i a l : A p p l i c a n t : _

20. No member may exploit his/her membership of the Blanco Neighbourhood Watch to his/her personal advantage, financial or any other benefit. 21. Members must at all times act in a manner that will uphold and promote the Aims and Objectives of the Blanco Neighbour Hood Watch as highlighted in the Blanco Neighbourhood Watch Constitution and the Western Cape Constitution for Neighbourhood Structures. 22. On patrol and during Blanco Neighbourhood Watch sanctioned security, patrol and community safety initiatives only approved Blanco Neighbourhood Watch clothing /regalia must be warn. 23. On patrol and during Blanco Neighbourhood Watch sanctioned security, patrol and community safety initiatives only approved Blanco Neighbourhood Watch car magnets and lights must be displayed. Patrollers and their vehicles have to be marked at all times. This is a SAPS standing order and SAPS have forbidden the Blanco Neighbourhood Watch from patrolling unmarked. It is only on the explicit instruction of SAPS may we go unmarked. 24. I acknowledge and accept that it is strictly forbidden to take non-members on sanctioned patrols. 25. All of the information contained in this document is correct to the best of my knowledge. 26. I accept that should any adverse information come to my attention that may be relevant to this application, but not declared by me, the Blanco Neighbourhood Watch reserves the right to cancel my membership with immediate effect. 27. Your Sector manager and Management committee will assess whether your skills, effort, diligence, attitude, health and adaptation to our culture, values business methods are appropriate to the BNW requirements. You may patrol under direct supervision of a senior member. You may participate and complete any of the scheduled training programs. 28. I hereby agree to participate in scheduled training programs offered by the Blanco Neighbourhood Watch. 29. The Blanco Neighbourhood Watch hereby declares Right of Membership is Reserved any issue, incident that may bring the Blanco Neighbourhood Watch into disrepute as a result of your actions that can be deemed as unbecoming of a Blanco Watch Member, can and will result in disciplinary action and possible expulsion from the watch. 30. CONSENT FOR THE USE OF PERSONAL INFORMATION I hereby authorize the Organisation's duly authorized verification agent, Blanco Neighbourhood Watch Chairperson: Deon Van Der Rijst, to access my personal information and conduct background screening checks including, but not limited to, credit, qualifications, employment references, criminal record, fraud prevention, ID verification and drivers licence. I consent to requests for consumer credit information to be released for the below prescribed purposes only: For employment in a position of trust and honesty and entails the handling of cash or finances; Fraud prevention or detection. I understand that verification requests form part of the background screening process and: That requests for credit information from Credit Bureaus will only be conducted under the regulations defined as per the NCA; Data obtained from the FSB serve only for the purpose to determine the fitness and propriety as envisaged in the FAIS Act. I acknowledge that any Personal Information supplied to the Organisation is provided voluntarily and that the Organisation may not be able to comply with its obligations if the correct Personal Information is not supplied to the Organisation. I understand that privacy is important to the Responsible Parties and the Responsible Parties will use reasonable efforts in order to ensure that any Personal Information in their possession or processed on their behalf is kept confidential, stored in a secure manner and processed in terms of South African law and for the purposes I have authorised. I warrant that all information, including Personal Information, supplied to the Organisation is accurate and current and agree to correct and update such information when necessary. By submitting any Personal Information to the Organisation in any form I acknowledge that such conduct constitutes a reasonable unconditional, specific and voluntary consent to the processing of such Personal Information in the following manner by the Organisation and/or verification information suppliers: Personal Information may be stored for a reasonable period by the Organisation, MIE and/or the Verification Information Suppliers, and Personal Information may be transferred cross-border to countries, which do not necessarily have data-protection laws similar to South Africa, for verification or storage purposes. In any cross-border transfer of personal information the recipient will be notified of the need to protect the confidentiality of the personal information. I take note that if the Responsible Party has utilised the Personal Information contrary to the Privacy and Data Protection Conditions, I may first resolve any concerns with that Responsible Party. If I am not satisfied with such process, I have the right to lodge a complaint with the Information Regulator. A copy of Personal Information kept by the Responsible Parties will be furnished to me upon request in terms of the provisions of POPI or the NCA and I understand that I may dispute any information in the record provided. I unconditionally agree to indemnify the Responsible Parties, and Verification Information Suppliers, acting in good faith in taking reasonable steps to process my personal information lawfully, against any liability that may result from the processing of my personal information.
