WA LIONS HALL OF FAME NOMINATION FORM The WA Lions Hall of Fame is an initiative to celebrate the outstanding contribution made by individual Lions and to recognize an elite group of people who have given exceptional service to the advancement of Lions in Western Australia during the past 52 years. Who can be nominated? Any current member of a Lions Club or a deceased Lion who has made a significant contribution to Lions in Western Australia. Lions members currently resident in another state or who have been members interstate are eligible to be nominated, however it is only their contribution to Lions in Western Australia that will be taken into account. Who can nominate? Nominations must be on the official Nomination Form and must be signed by a Lions Club President. The Nominee does not have to be, or have been, a member of that particular Club. How are nominations assessed? Nominations will be assessed by a panel of judges consisting of WA Lions Hall of Fame Committee members (W1 & W2) and eminent non Lion Western Australians. In considering nominations, the judging panel will take into account the degree and value of their contribution to their community as well as the District and or State. Length of Service whilst considered will not be the primary focus. Up to 3 Lions will be inducted this year. Does it cost anything to nominate someone? Yes. We are asking Clubs to pay a fee of $50.00 per nomination. The fee will be used to offset costs involved including the cost of the plaques and certificates. What if the person we nominate misses out? No one will miss out. Firstly being nominated is a huge reward in itself. Every person nominated will also receive a framed certificate acknowledging their nomination. Secondly, all nominations will be automatically considered for the following two years, so there will be no need to nominate them again for that period of time. How will the Awards be presented? A special WA Lions Hall of Fame Dinner & Presentation Night will be held on Friday 7 th November in Perth to honour and recognize the contribution that the nominated Lions have made in Western Australia. The general public will be invited to the Awards Night. Further details of the Awards night will be made available to Clubs during September. Will there be equal representation between W1 Lions and W2 Lions? No, not necessarily. We are acknowledging the outstanding Lions from throughout Western Australia and recipients will be those deemed the most worthy at the time by the judging committee. (Don t forget that for many years WA was just one District) What will happen in future years? In future years a select number of Lions will be inducted each year into the WA Lions Hall of Fame. The number of Lions inducted will vary from year to year depending on nominations received, however will be capped at 3 in line with making this a very special award. PLEASE COMPLETE THE NOMINATION FORM AND RETURN IT TO: WA Lions Hall of Fame Committee PO Box 455 FORRESTFIELD WA 6058 Nominations close 31 st August 2014
