Mini Contracts Consent Agenda Upon the recommendation of the Superintendent of Schools, a Motion was made by, seconded by, and carried unanimously (8-0) to approve a consent agenda for mini contracts a-p. a. Approval of Agreement between the Sachem Central School District and Reach for the Stars Tutoring, Inc. seconded by Mr. Scavo, and carried unanimously (8-0) to approve agreement between Sachem Central School District and Reach for the Stars Tutoring, Inc. to provide academic tutoring services. The fee for this service is $37.00 per hour to be paid by the Office of Federal Funds. The term of this agreement is September 1, 2010 to June 30, 2011. This contract has been reviewed and approved by the school district s attorney. b. Approval of Agreement between the Sachem Central School District and South Huntington Union Free School District Central School District and South Huntington Union Free School District to provide health and welfare services for children residing in Sachem Central School District and attending non-public schools in the South Huntington Union Free School District. South Huntington UFSD shall be entitled to bill Sachem CSD $770.85 per student. The term of this agreement shall be from July 1, 2009 through June 30, 2010. This agreement has been reviewed and approved by the school district s attorney. c. Approval of Agreement between the Sachem Central School District and Middle Country Central School District Central School District and Middle Country Central School District to provide health and welfare services to students who reside in the Sachem Central School District. The rate for this service is $549.05 per student. The term of this agreement shall be from July 1, 2009 through June 30, 2010. This agreement has been reviewed and approved by the school district s attorney. d. Approval of Agreement between the Sachem Central School District and Bay Shore Union Free School District Central School District and Bay Shore Union Free School District to provide health and welfare services to students who reside in the Sachem Central School District. The rate for this service is $702.66 per student. The term of this agreement is from September 8, 2009 through June 25, 2010. This agreement has been reviewed and approved by the school district s attorney. e. Approval of Agreement between the Sachem Central School District and Smithtown Central School District Central School District and Smithtown Central School District to provide health and welfare services to students who reside in the Sachem Central School District. The rate for this service is

$759.53 per student. The term of this agreement is from September 1, 2009 through June 30, 2010. This agreement has been reviewed and approved by the school district s attorney. f. Approval of Agreement between the Sachem Central School District and Hauppauge Union Free School District Central School District and Hauppauge Union Free School District to provide health and welfare services to students who reside in the Sachem Central School District. The rate for this service is $694.81 per student. The term of this agreement shall be from September 1, 2009 through June 30, 2010. This agreement has been reviewed and approved by the school district s attorney. g. Approval of Agreement between the Sachem Central School District and Hicksville School District seconded by Mr. Scavo, and carried unanimously (8-0) to approve agreement between Sachem Central School District and Hicksville School District to provide health and welfare services to students who reside in the Sachem Central School District. Approval of Agreement between the Sachem Central School District and Little Angels Center for Speech and Language Central School District and Little Angels Center for Speech and Language to provide academic tutoring and speech therapy services. Academic tutoring rates are as follows: Individual Tutoring Session 30 minute session $ 40.00 (cap of $60.00 per 30 minute session for a group of two (2) to five (5) students) This shall include all services in and out of the district including home, private and parochial schools. Annual review testing and report preparation is included in the therapeutic process and no additional charge will be claimed. If more than 3 professionals are located in a facility, an on-site administrator will be provided as needed at no additional charge. Some of the missing devices were concealed in existing ductwork. Provisions made for some duct detectors in the base bid, just not enough. 3. Some devices existing in the old fire alarm system are not required by code so they were not included in the base bid. The District requested that all existing devices be replaced or restored. 4. New pre action panels were left out of the construction project from several years age. It was not known that this was never done. 5. Some new devices were required due to the size and location of some existing structural members. 6. There are concealed relays for fan shut down. Provisions were made for them in the base bid. There were more of them than was anticipated. 7. Other miscellaneous devices were either concealed or overlooked during the walk through.
