Application for a study permit Checklist Complete and place this checklist on top of your application. As of September 4, 2013, you may need to give your biometrics (fingerprints and photograph) and pay a biometric fee. You must include a certified translation with any document that is not in English, French or Spanish. Travel plans are done at your own risk until you receive the approval of this application. Note: the submission of an application with the documents listed below does not guarantee the issuance of a visa. IMPORTANT: It is responsibility of the applicant to submit all information necessary to fulfill the requirements for this type of visa. It is a serious offence to give false or misleading information on these forms. The information and documentation you provide on your application is subject to verification. Canadian visas cannot exceed the validity of the passport. Failure to submit all required documentation may result in the refusal of your application or processing delays. In addition to a study permit, are you submitting applications for the following family members? Spouse or common law partner Minor Children Open work permit - complete an application for a work permit. Temporary resident visa (if not applying for an open work permit) - complete an application for a temporary resident visa. Open study permit - for accompanying children aged 6-18 years old - completion of separate application form required. Temporary resident visa (if a study permit is not required) - completion of separate application form required. Number Required 1

Submit the following items: Two photographs for each applicant. Signed, dated and taken in the past six months. Write the name and birth date of the individual on the back. Valid passport from each person requiring a temporary resident visa. Your passport should be valid for at least six months from the date of travel to Canada. TO NOTE: The validity of a study permit is issued based on the validity of the passport regardless of the length of your studies. The validity of a visa cannot go beyond the validity of the passport. Include all cancelled or expired passports, if applicable. If you do not reside in your country of citizenship, please provide your work permit or temporary stay permit. If you have answered yes to any of the background information questions on the application form, provide a completed and signed Schedule 1 form (Application for Temporary Resident Visa IMM 5257B) and include it with your application. Family Information Form IMM 5707, fully completed, dated and signed. Submit a COPY of the following items: Bio-data page of each applicant s passport. Explain the purpose of your studies. Letter of acceptance from your educational institution in Canada, even if you are intending to study for a period less than six months. If your acceptance is conditional, you must show you can meet the conditions. Certificat d Acceptation du Québec (CAQ) if you will be studying in Quebec. If your tuition has already been paid, provide proof of payment. If lodging arrangements have been made, provide proof. Marriage certificate or Statutory Declaration of Common-law Union IMM 5409 for accompanying spouse, as applicable Birth Certificates for accompanying minor children if applicable. If a minor is travelling with only one parent, custody documents and a notarized authorization to travel signed by the other non-accompanying parent is required. Minors (under 18 years of age) travelling alone or with a person other than their parents or legal guardians should have a notarized authorization to travel signed by both parents 2

or legal guardians. It should also include the name of the adult who will be responsible for the children in Canada. If a minor child intends to study in Canada, submit the letter of acceptance from the institution where the individual will study and the custodianship declaration IMM 5646 signed by both parents and by the adult who will be responsible for the child in Canada. The age of majority is 18 in Alberta, Manitoba, Ontario, Prince Edward Island, Quebec and Saskatchewan. The age of majority is 19 in British Columbia, New Brunswick, Newfoundland, Nova Scotia, Northwest Territories, Nunavut and Yukon. Proof of business: if you are self-employed, provide business registration, tax clearance certificates, trade certificates, license and financial documents etc. (business bank account statement for last three months) Proof of employment - Employment letter dated within one month of the application indicating position, salary, years of employment and authorized leave dates (with and without pay). Proof of sufficient funds to cover your tuition and living expenses for at least one year in Canada**. In addition to the cost of tuition, you should have at least CAD $10,000 available to cover your living expenses; an additional minimum requirement of CAD $4,000 must be added if a spouse or partner is accompanying the student, as well as CAD $3,000 per accompanying child or other accompanying dependent. Please submit: Supporting documents demonstrating a reliable source of funds convertible to Canadian dollars (letter of undertaking and financial documents from parent or sponsor, official letter of scholarship or financial award, investments, income from rental properties, bank statements covering several months, etc.); If you are not paying for your own trip, indicate how your trip will be funded. Submit supporting documentation, for example: parents bank statements; letter from employer covering costs; proof of employment and financial documents for host in Canada* (employment letter, pay slips, T4 or Notice of Assessment, bank statements), etc. *Note: Some hosts or sponsors may prefer not to provide their personal information directly to the applicant for submission to the Visa Office. In this case, please enclose a note to this effect. Please be advised that this may lead to slower processing in some cases. ** Please note that even if a third party will cover the cost of your studies, you still must submit proof of the financial situation of the person that you depend on (for example: your parents) Police clearance certificate for the applicant and any accompanying family member above 18 years of age. Certificates are also required for each of the countries in which they stayed more than six months since they turned 18. Completed Military table. If you are or have been part of the military or police forces you will have to fill out the military table which may be requested in advance at lima.immigration@international.gc.ca or at the VAC office. Up-front Medical report you must undergo a medical examination by contacting a panel physician if you will stay in Canada for more than six months, if you have been out of Canada for more than six months or if you will participate in any medical-related activity. 3

If your visa is approved, it is recommended to obtain an International medical Insurance covering the entire period of your intended stay in Canada. 4
