(14.00 Hrs.) in the Tender box as per instructions given below :- Original copy of Technical Bid (Annexure-I) & Terms and Conditions with other document along with DD of EMD must be kept in the Envelope Marked as Envelope No. 1 (Original Technical Bid) Duplicate copy of the above duly filled up in all respects should be kept in envelope Marked No. 2 as (Duplicate Technical Bid). Sealed offers submitted by Hand Delivery should be delivered to the Tender Box available at Head Office of the U.P. Beej Vikas Nigam on the above address. Cabled or Faxed offers will not be accepted. However, U.P. Beej Vikas Nigam will not be responsible for any postal delay. Conditional Tender: Any condition such as Subject to immediate acceptance shorter validity period or condition supers cribbed on envelope or in the Tender Documents or separately shall not be considered and all such tenders may be rejected. In other words the conditional tender will not be considered. Validity: The Tenderer shall keep the offer open for acceptance for a period of 60 days (Sixty Days) from the Date of Opening/Finalization of Tender. The successful tendered shall have to execute an agreement, if required, within 07 (Seven) days after having been called to do so on their own cost. Earnest Money Deposit : Every Tender must be accompanied with a Bank Draft drawn on any Nationalized Bank in favour of U.P. Beej Vikas Nigam for Rs. 10,000/- (Rupees Ten Thousand only) payable at Lucknow No. Erasing No Erasing or alteration in the text of tender document is permitted and such erasing or alteration will be disregarded or render the whole tender void at the option of the committee until unless the same has been signed in ink by the person signing the tender alongwith the stamp of the firm/company.

- 3 - Clarification ; FOR FURTHER CLARIFICATION, IF REQUIRED, MAY BE HAD FROM THE INCHARGE OF COMPUTER CELL, AT OUR HEAD OFFICE, IN OFFICE TIME IN ANY WORKING DAY Important Dates to be remembered: - 1- Sale of Tender Documents from HQ, Lko : 13-07-2012 up to 12.00 Noon 2- Receiving of Tender Documents at HQ, Lko : 26-07-2012 up to 2.00 P.M. 3- Opening of Technical Bid & Financial Bid at HQ, Lko : 26-07-2012 at 3.00 P.M. TERMS AND CONDITIONS : (a) The firm shall provide comprehensive the Maintenance Services, which includes replacement of DMP Printer Head & UPS Batteries under the Agreement to keep this system in good working condition for a period of One (01) Year from the date of issue of work order for this work. (b) firm. Unscheduled (on call) corrective and remedial maintenance service will be provided by the (c) Replacement of defective part (s) except consumable items viz. Ink Cartridges/Toners & Paper will be covered in the Comprehensive Annual Maintenance Contract. Any defective part removed from the system will have to be replaced with new one. (d) Provision of stand by equivalent equipment/system is to be made by the firm in case of any malfunction in the equipment/system if that cannot be set right within 24 hours of making complaint. (e) Schedule preventive maintenance once in every month (in first week of each month) is to be done by the firm. (f) The firm shall provide on call service from Monday to Saturday excluding public holidays between 10.00 A.M. to 6.00 P.M. (g) The response time on a request for the services shall not be more than 6 hours from making complaint. However, if the complaint is reported before 14.00 hours, it shall be attended on same day itself. (h) The firm shall supply all spare parts required for repair or replacement, free of cost. (i) The firm shall provide all normal tools and test equipments needed for maintenance of the system. Nothing extra will be paid. (j) Annual Maintenance Charges will be paid quarterly i.e. after 3 (Three) months, ¼ of the Total One Year Charge less all the penalties/recovered etc. (if any)

- 4 - (k) First Instalment of payment will be made after 03 (Three) Months from signing of the contract provided system is being maintained at the satisfaction of the undersigned. (l) At the time of making the final payment after completion of one year, the services provided by the firm in the year will be reviewed and if found necessary, the penalty or recovery may be levied and recovered thereof. (m) The firm shall provide at least one engineer everyday for full time visit between 10.00 A.M. to 06.00 P.M. to our Head Office Lucknow. OTHER TERMS AND CONDITIONS FOR ILIGIBILITY FOR THE TENDERER 1. EMD Draft for Rs. 10,000.00 (Rupees Ten Thousand only) made from any Nationalized Bank in favour of U.P. BEEJ VIKAS NIGAM, payable at LUCKNOW. 2. The Tendered should submit Complete Profile along with Turn-over Per Year. 3. The firm should have minimum 30 Lacs Turn-over Per Year through CAMC authentic document must be enclosed. 4. The firm should have at least 05 (Five) years experience in the field of CAMC of Computers and should submit the proof of maintaining more than 100 (One Hundred) Computers in at least in 2 (Two) Government Organizations. The firm should also submit at least 3 (Three) Satisfactory Performance Certificates issued by Government Organizations. 5. The firm should have at least Five (5) Branches at Different Commissionery in U.P. other than Lucknow to handle Multi Location Support. 6. Documentary Proof of Registration in Income Tax / Sale Tax / Service Tax Department alongwith No./ Tin No. 7. PAN No. 8. Details of Department/Organization with complete addresses and Contact Nos. where you have provided services of CAMC of Computers (more than 50 Computers) in Last Three (3) Years. 9. Copies of work orders. 10. No. of full time Engineers available with your firm. 11. Affidavit from Notary / Self Declaration about Non-blacklisting of your firm. Managing Director, U.P. - 7 - AGREEMENT OF CAMC OF COMPUTERS This agreement made this day of (Two Thousand Ten) between U.P. BEEJ VIKAS NIGAM, a Company having its Registered Office at C-973/74-B, Faizabad Road, Mahanagar, Lucknow hereinafter called the CORPORATION which expression shall be deemed to include its successors and assigns on the one part and M/s a Registered Firm having its Registered Office at (hereinafter called the Contracted firm) which expression shall be deemed to include its successors and assigns. WHEREAS the Corporation (U.P. Beej Vikas Nigam) has agreed to award an CAMC (Comprehensive Annual Maintaintence Contract) of Computers and its peripherals, details of which have been furnished hereunder and the Contracted Firm has agreed to undertake the same CAMC of Computers and its peripherals as per terms, conditions & rates given below:- Details of Computer & Peripheral for CAMC S.N. HP Office jet 1319 Printer Copier Scanner & Fax Head Office 01 13 FULL NETWORK MANAGMENT Head Office TERMS AND CONDITIONS : (a) The firm shall provide the Comprehensive Maintenance Services, which includes replacement of DMP Printers Head & UPS Batteries under the Agreement to keep this system in good working condition for a period of one year from to.. from the date of issue of work order for this work. (b) Unscheduled (on call) corrective and remedial maintenance service will be provided by the firm. (c) Replacement of defective parts except consumable items viz. Ink Cartridges/Toners & Papers will be covered in the Comprehensive Annual Maintenance Contract. Any defective part removed from the system will have to be replaced with new one. (d) Provision of stand by equivalent equipment/system is to be made by the firm in case of any malfunction in the equipment/system if that cannot be set right within 24 hours of making complaint. - 8 -

(e) Schedule preventive maintenance once in every month (in first week of each month) is to be done by the firm. (f) The firm shall provide on call service from Monday to Saturday excluding public holidays between 10.00 A.M. to 6.00 P.M. (g) The response time on a request for the services shall not be more than 6 (Six) hours from making complaint. However, if the complaint is reported before 14.00 hours, it shall be attended on same day itself. (h) The firm shall supply all spare parts required for repair, free of cost. (i) The firm shall provide all normal tools and test equipments needed for maintenance of the system. Nothing extra will be paid. (j) Annual Maintenance Charges will be paid quarterly i.e. after 3 (Three) months, ¼ of the Total One Year Charge less all the penalties/recovered etc. (if any) (k) First Instalment of payment will be made after 3 (Three) months from signing of the contract provided system is being maintained at the satisfaction of the undersigned. Bill will be raised at the end of the quarterly. (l) At the time of making the final payment after completion of one year, the services provided by the firm in the year will be reviewed and if found necessary, the penalty or recovery may be levied and recovered thereof. (m) The firm shall provide at least one engineer everyday for full time visit between 10.00 A.M. to 06.00 P.M. to our Head Office Lucknow. (n) All disputes are subject to Lucknow jurisdiction. 2. The Cancellation of the Contract may be either for whole or part of the Contract at Corporation s (U.P. Beej Vikas Nigam) option. In the event of the Corporation terminating this contract partly or in full due to partial or total failure of the Contracted firm to execute Comprehensive Annual Maintenance Contract in whole or in part, they may award CAMC upon such terms and in such manner at deems appropriate similar to those so terminated and the Contracted firm shall be liable to pay to the Corporation (U.P. Beej Vikas Nigam) for any excess charges for such similar contract towards risk contract that the Corporation (U.P. Beej Vikas Nigam) shall continue with the performance of this contract to the extent, not terminated under the provision of this clause. 3. All disputes or difference in relation to this Contract or the interpretation of any of its terms or implementation thereof or arising out of, or concerned directly or indirectly with the Contract will be referred to the Arbitration of the Chairman, U.P. Beej Vikas Nigam. 4. The venue of Arbitration shall be U.P. Beej Vikas Nigam s Head Office at Lucknow or any other suitable place decided by the Corporation. The Arbitration Act, 1956 of India shall apply to the Arbitration proceeding. The Arbitrator shall have power with the consent of the parties to extend the time for making and publishing the award. If for any reason, the matters have to be referred to a Court of Law, the Court of Law, having jurisdiction at Lucknow (U.P.) only shall have jurisdiction in this matter.
