{$FIRSTNAME} {$LASTNAME} NUROFEN SPECIFIC PAIN RANGE CLASS ACTION - GUIDE FOR COMPLETING STATUTORY DECLARATION This guide contains important information to assist you to correctly complete and return the statutory declaration. Please read it carefully. 1. What is a statutory declaration? A statutory declaration is a written statement which you sign and declare to be true before an authorised witness. A person who intentionally makes a false statement in a statutory declaration is guilty of a criminal offence under section 11 of the Statutory Declarations Act 1959 which carries the possibility of up to four years imprisonment. 2. Completing the statutory declaration When completing the statutory declaration please use a black or blue pen and ensure that your handwriting is clear and legible. The information you are required to include in the statutory declaration is set out below. First section: set out in the space provided your name, address and occupation. Paragraph 1: set out in the space provided how many packets of each Nurofen Specific Pain Range product you purchased between 1 January 2011 and 31 December 2015. If you didn t purchase a particular Nurofen Specific Pain Range product, leave that particular line blank. Paragraph 2: set out in the space provided how much you estimate you spent on Nurofen Specific Pain Range products between 1 January 2011 and 31 December 2015. Please include a dollar amount in the space provided. Paragraph 3: set out in the space provided the dates during which you purchased the packets of Nurofen Specific Pain Range products. For example between February 2012 and March 2013 and between December 2014 and June 2015. Paragraph 4: set out in the space provided how often you purchased packets of Nurofen Specific Pain Range products. For example one packet every two weeks OR 4 packets every two months. Paragraph 5: set out in the space provided the stores and locations at which you purchased the packets of Nurofen Specific Pain Range products. Paragraph 6: set out in the space provided what payment method or methods you used to purchase the packets of Nurofen Specific Pain Range products. For example cash, credit/debit card and EFTPOS. Paragraph 7: set out in the space provided your postal address, email address and phone number. Paragraph 8: if you estimate in paragraph 2 that you spent $200 or more, and you have the documents available, you must attach to your statutory declaration copies of receipts, bank and credit card statements and/or photos of packets that show you purchased Nurofen Specific Pain Range products between 1 January 2011 and 31 December 2015. If you estimate in paragraph 2 that you spent less than $200, and you have these documents available, you can also attach the documents to your statutory declaration. The first page of any documents you attach to your statutory declaration should include the following wording: This is annexure A to the statutory declaration made by [insert your name] on [insert date declaration made]. 3. Signing the statutory declaration Once completed, you must sign the statutory declaration in the presence of an authorised witness such as a lawyer or Justice of the Peace. A full list of authorised witnesses is at the back of the statutory declaration. Each page of your statutory declaration should also be initialled by you and the authorised witness. The witness does not need to sign any documents you attach to your statutory declaration. 4. Submitting the documents to the Administrator Once you have signed the statutory declaration, you should make a photocopy or take a photo for your records.
