AB Health Care Card None, however you will be asked for your: Name Mailing address Telephone number Personal health care number, and Date of birth If you do not know your personal health number, you will be asked for proof of identity. In person: Any registry agent in your area To find a local registry agent call: (Edmonton area), or 310-0000 then 780-427-7013 (Outside of Edmonton) Cards applied for by phone or in person are sent out by mail and generally received in 1-2 weeks. Provincial ID Card 1. One original (not a photocopy) government issued ID with your photo, first & last name, and date of birth. Examples: Driver s licence, Provincial ID Card, Passport, Metis Nations of Alberta card, Secure Certificate of Indian Status (SCIS) Card *If you are homeless, and do not have valid government ID, you can satisfy this requirement (for a provincial ID card application only) by completing an Identity Certification Form. This is a form that a staff member working at a shelter or drop-in center you access must complete with you. Drivers Licence 2. One original (not a photocopy) support document that shows 2 things about you (example: name, date of birth, address, signature). Examples: Birth certificate, utility bill, income tax receipt, insurance policy, income support cheque, a different piece of photo ID. 3. A document to prove you live in Alberta (example: a bill mailed to your house, a lease) *If you are homeless, you can satisfy this requirement by completing an Address Authorization Form. Canadian Passport 1. Proof of Canadian citizenship: Birth certificate, certificate of citizenship 2. Government issued identification that includes your signature. Driver s licence, provincial ID card, military identification, Secure Certificate of Indian Status *Your application will need to be signed by a guarantor. A guarantor is someone who: 1. Has known you for 2+ years 2. Is a Canadian citizen over 18 years old 3. Has their own valid passport, or a passport that has not been expired for more than one year 4. This may lead to delays in replacing your passport.
