HRM Department H. O. Pune Guidelines for New Employees for submitting various documents (For CLERKs) At the time of joining the Bank s service at Allotted Zone, you will have to submit the following documents. Without submission of proper documents you may not be allowed to join. Important Guidelines: Login to the link provided (www.bankofmaharashtra.in) with your credentials i..e Roll Number & Password allotted to you by IBPS for Online Test, Date of Birth registered with IBPS etc. for downloading documents. Take a printout thereof for submitting it at the Zonal Office of allotted zone. For Documents to be submitted please refer the Check-List & Guidelines below. All photocopies to be submitted must be on A-4 size paper ONLY & must be readable and should be self-attested / signed by you. For verification of photocopies; you should carry with you ORIGINAL Certificates / Documents. All documents should be arranged properly and in order as mentioned in the Check-List. The candidate shall fill in all required information in the documents and sign. Affix Photographs of proper size as mentioned in the documents. Scan each document separately and prepare scan files of minimum possible size. However there should be ONE scan file for all pages in the document e.g. scan all pages of Offer for Employment continuously and prepare only ONE file for the same. Similarly in case of NPS Form, scan all required pages alongwith photocopies of Proof of Identity and Address and prepare ONLY ONE File. Login to the link provided and upload scan files one by one. Important Guidelines regarding Documents to be submitted / Check List Out of documents mentioned below; you should submit ALL documents applicable to you; to the Bank s officials at Zonal Office of Allotted Zone before / while joining the Bank s service. In case of non-submission of any document you will not be allowed to join the Bank s service. You will have to join and report the Zonal Manager of Allotted Zone on 18 th May 2015 for further instructions & posting. Page 1 of 5

For downloading these documents please LOGIN to the link provided in the Bank s Website (www.bankofmaharashtra.in) with your credentials. 1. Proof of Identity: Self-attested photocopy of Proof of Identity like Pan Card / Valid Passport / Valid Driving License / Voter s ID Card etc. 2. Proof of Date of Birth: If your Date of Birth is not mentioned in the Proof of Identity you will have to submit proof therefore e.g. self-attested photocopy of Birth Certificate, School Leaving Certificate, SSC / HSC / Graduation Mark-sheet / Certificate wherein Date of Birth is clearly mentioned. 3. Proof of Address: Self-attested photocopy of Proof of Address like Adhar Card / Valid Passport / Valid Driving License / Voter s Id Card / Latest Telephone (Landline) / Mobile bill in your name (not more than 3 months old) / Latest Electricity Bill (not more than 3 months old) / Latest Property Tax Receipt (not more than one year old) etc. 4. Educational & Professional Qualification Certificates & Mark sheets: Self-attested photocopies of your educational certificates & mark sheets like SSC, HSC, Graduation & Post-Graduation Certificates and self-attested photocopies of Professional Qualification Certificates & mark sheets like MBA, C.A., C. S. etc. As per IBPS Advertisement for CWE IV for Pos / MTs All the educational qualifications should be from a University / Board recognized by Govt. of India / approved by Govt. Regulatory Bodies and the result should have been declared on or before 11.08.2014 (for P. O.s) / 01.09.2014 (for Clerks) / 09.12.2014 (for Specialist Officers). The Date of passing eligibility examination will be the date appearing on the mark-sheet / provisional certificate issued by university / institute. 5. Self-attested photocopy of SC / ST Caste Certificate: Certificate shall be in the format prescribed by the Central Government and format given in the IBPS Advertisement. 6. Self-attested photocopy of OBC Non-Creamy Layer Certificate: Certificate should have been issued on or after 02 nd Sept. 2013 mentioned below and should be in the proper format. 7. Affidavit on Stamp Paper of Rs.100/- for OBC (Original Form with Stamp): This shall be in prescribed format only. 8. Acknowledgment from Tahsildar / Block Revenue Officer regarding Caste Certificate (SC / ST / OBC): Take two printouts of the letter and fill in details of Tahsildar / Block Revenue Authority who has issued you SC / ST / Non-Creamy Layer certificate. Visit any nearest branch / office of the Bank and request to issue the letter with signature of Authorized Officer of the Bank with Bank s stamp. Please submit both the copies to the concerned Tahsildar / Block Revenue Authority and obtain his / her signature and stamp on one copy for having received a copy of the letter and submit the same to the Bank s officer at the allotted zone. 9. Ex-serviceman Discharge Letter: If you are ex-serviceman, self-attested photocopy of Discharge Letter is essential. Page 2 of 5

10. Physically Challenged Certificate (Orthopedically Challenged OC / Hearing Impaired HI / Visually Impaired VI): The certificate should have been issued by District Medical Board. Self-attested photocopy thereof should be submitted. 11. Medical Certificate / Report (Original copy): Approach any branch / office of the Bank and request for issuing covering letter addressed to Medical Officer of the Bank / District Civil Surgeon Paste your photo on the Medical Report. Approach Bank s Medical Officer / District Civil Surgeon for Medical Check-up and issuing of Medical Certificate. Obtain his signature with stamp and other details. Bank has appointed Medical Officer at select places. For getting the information you may contact the Zonal Office / Staff Officer of the zone. Wherever Bank s Medical Officer is not available, certificate issued only by District Civil Surgeon should be submitted. 12. Self-Declaration Form (Original duly filled & signed copy): Candidate has to affix his/her latest photograph on the first page & fill in the complete form and submit the same to the Bank officials at allotted zone. Candidate may also submit one copy of the form to Police Authorities along with police verification report to enable the Police Authorities to send the certificate to the Bank. 13. Acknowledgment of Letter to Police Authorities regarding Police Verification Report : Approach any branch / office of the Bank and request for issuing covering letter addressed to Police Authorities. First page of the police verification letter is addressed to the Police Authorities. The candidate has to write name of the nearest Police Station / Commissioner s Office on the said letter. On the second page of Police verification report candidate has to affix his/her recent photograph duly attested by him/her. Candidate has to submit both the pages alongwith a copy of self declaration form to his /her nearest police station /Police Commissioner s office and obtain their acknowledgement of having received the documents on photocopy of the same. The acknowledged copy by the Police Authorities be submitted to Bank Officials for Bank s record at allotted zone. (Letter and reply formats are available on web site) 14. TWO Character Certificates (Original Copies ONLY): Certificates should be issued by TWO reputed individuals of the society who are not related to you. Issuing authority shall mention his / her Full Name, Address and contact details thereon. 15. Security / Service Bond for Rs.50,000/- for ONE Year (should be NOTERIZED) (Original Form): The candidate should open a Fixed Deposit Account at any branch of the Bank of Maharashtra for Rs.50,000/- for ONE year in his / her name only. A valid Fixed Deposit Receipt thereof; obtained from the issuing branch; should be duly signed & endorsed by the candidate and verified & attested by the Authorized Official of the issuing. The candidate should submit the said receipt to the Bank s Official at the allotted zone. The said receipt will be released by HRM H. O. only after expiry of TWO years from your joining the Bank s service. However instead of submitting Security Deposit as above, the candidate can submit Security / Service Bond in the prescribed format only which is available in the website link provided. The bond should be executed on Non-judicial Stamp Paper of Rs.200/-. There shall be TWO Sureties for the same. The Bond should be signed on all pages by the candidate and both the sureties mentioning address and contact details. The bond must be NOTORIZED i.e. should be signed by a Notary. Notary should affix his / her stamp on the bond Page 3 of 5

papers. Original Bond Paper should be submitted to the Bank s Officials at the Allotted Zone. 16. Cash Security Bond: Candidate has to submit cash security of Rs.10000/- ( Rs. Ten thousand only) in the form of deposit in Bank of Maharashtra at the time of verification /within 3 months from the date of appointment on probation. 17. Staff / Personnel Inventory Form (Original Form): The candidate shall fill in ALL necessary details and sign the form. All information filled in shall be supported by self-attested documents like educational & professional qualification certificates & mark-sheets etc. (Please refer above points). This is essential for capturing your data in the Bank s Personnel Inventory System (PINS) maintained at HRM H. O. 18. Gratuity Nomination Form: If you nominate more than one nominee, Total of Percentage Shares of ALL nominees must be 100 else the form may be rejected. New Pension Scheme (NPS) Form (Original Form):- Any New Employee joining the Bank s service on or after 01 st April 2010 is to be covered under New Pension Scheme / National Pension System (NPS). a) For registration under New Pension Scheme and allotment of a Permanent Retirement Account Number (PRAN), candidate should submit CS-S1 form i.e. Subscriber Registration Form alongwith self attested photocopies of proof of identity, Address and Date of Birth. (b) However, if the candidate already holds a PRAN under NPS, he/she should submit ISS-1 form; alongwith self attested photocopies of Proof of Identity, Address, Date of Birth and PRAN Card for shifting his/her PRAN to Corporate Sector Model of NPS implemented by the Bank for eligible employees. For verification of documents originals are required to be given to bank officials at allotted zone. Before filling in the form please read all instructions contained in the form itself. 20. Printout of Online Application submitted to IBPS & Score Card /marks allotted by IBPS submit self-attested copies of the same. 21. Offer for Employment: Please generate online Offer of Employment. Take two print-outs of the same. Duly accepted & signed copy should be submitted to the Bank s Officer at the Allotted Zone. The candidate can retain another copy for his record. The candidate should upload duly accepted and duly signed copy in the Bank s website before joining the training programme. (This will be available in a couple of days). 22. Any additional document / self-attested photocopies of any additional documents desired to be submitted. Originals shall be produced for verification. All above documents shall be filed properly and serially in above order and shall be submitted to the Bank s officials. All documents shall be signed by you. Except specifically mentioned above; Original copies of these documents are to be submitted. Page 4 of 5

Uploading of Scan Documents 1) Scan all the pages of a document in one scan file e.g. all pages of Offer for Employment are to scanned continuously and prepare ONLY One Single scan file for the same. Similarly in case of NPS Form (CS-S1) scan Page no.s 1, 2, 3, 6 and 7 and Proof of Identity and Proof of Address continuously in ONE Single Scan File. 2) Please see that size of each scan file is minimum possible so as to upload the file successfully. 3) LOGIN to the link provided with your credential and upload each file, one by one, in the Bank s website. 4) In case of any technical difficulty, you may contact our IT Department by email to sitalaxmi.chari@mahabank.co.in. 5) In case of any query you may contact HRM H.
