Tracking Your WellPoints: You track your participation via our online WELL 4 U Blackboard environment. Tracking your points is easy! Participants must self-enroll in the WELL 4 U wellness program. How to enroll in Well 4 U @ EKU! Instructions Sign into Blackboard Click on the Community tab along the top next to My EKU Type Well in the Search box Click on the arrow to the right of 0_WELLNESS_BAUER Click on Enroll Click Submit You will receive this message if you have completed the enrollment successfully!
Click OK to continue. You are now enrolled & can begin logging your Wellpoints! 1. First, you will log into Blackboard. You may access Blackboard at http://learn.eku.edu/ o o Your Blackboard username and password match your EKU email account. If you lose your username or password, call 859-622-3000(Option 2) for help. 2. Second, you will find WELL 4 U under My Organizations 3. Click the organization name: WELL 4 U 4. Once you click the organization name, you arrive at the Announcements Page. a. Any new announcements made by the administrators will be here when you log in. It is a good idea to read over this area first! 5. On the left is a menu. In the next few items, we will take you through the menu options. 6. First, you see Announcements. This is the page you are already on. If you go to another area and would like to return to the Announcements area, click this link. 7. Next, you see Add Wellpoints. This is the page where you will be able to track your participation for this semester.
a. You can save your progress and return as often as you want! b. Each semester, you will have the opportunity to start and submit a new Wellpoints tracking sheet. c. Make sure to SAVE your progress and do not submit until you are done. Once you submit your Wellpoints tracking sheet, you cannot get it back. 8. Click BEGIN to start tracking your Wellpoints. The questions will appear one by one on the screen. You can move back and forth between questions by using the arrows in the lower right corner. The Wellpoints tracking sheet is structured as follows: Question 1: Check "Yes" or "No" if you've completed your Health Risk Assessment--Real Age. Questions 2-4: Check 1 box for every 30 minutes you workout. If you fill up the boxes in Question 2, move to Question 3 and so on. There are 60 check boxes total (30 hours of activity). Exercise a minimum of 56 times during the spring semester and receive 15 WellPoints. Question 5: Biometric Screening/Preventive Health Exam/Fitness Tests: Check a box for each Biometric Screening/Preventive Health Exam/or Fitness Test that you had this semester. Question 6: Check "Yes" or "No" if you completed the online E-chug assessment.
Question 7: Check "Yes" or "No" if you completed the online E-Toke assessment. Questions 8-11: On-Campus Wellness Seminars: Please fill in the name of the wellness seminar that you attended. Questions 12-13: Wellness Courses: Please fill in the name of the wellness class that you are taking this semester. Question 14: Check "Yes" or "No" if you completed the Cooper Clayton Smoking Cessation Series, through Health Education, this semester. Or Check "Yes" if you have been Tobacco Free for the past 6 months. Question 15: Wellness Coaching: Check Yes or No if you utilized the wellness coaching services this semester. Question 16: Health Challenges: Check "Yes" or "No" if you completed the Group Fitness Challenge between January 10 th February 25 th 2011. Question 17: Heath Challenges: Check "Yes" or "No" if you completed the Big Green Climb Challenge between March 14 th April 29 th 2011. Question 18: Heath Challenges: Check "Yes" or "No" if you completed the Road Rulz Challenge between March 14 th April 22 nd 2011. Questions 19-23: Health Awareness Event: Please fill in the name of the health awareness event that you attended. Question 24: Check Yes or No is you attended the EKU Health Fair on March 30, 2011. NOTE: the point values assigned to each question represent the maximum point value for the program. After your online Wellpoints tracking sheet is submitted and reviewed, you may view your semester Wellpoints in the Blackboard Grade Center, available by clicking Wellpoints on the side menu.
9. Next on the navigation menu is Take Your Health Risk Assessment. This is where you go to complete your Real Age test. 10. Next on the navigation menu is Spring 2011 WELL 4 U Program. This page provides you with program details and descriptions, including how to participate in WELL 4 U; how to earn rewards; how many Wellpoints are needed to earn prizes; what activities count towards Wellpoints; how many Wellpoints are various activities worth; what is a local wellness event. 11. Click on this link when you are ready to meet with a Wellness Coach. Send us your request by completing the form and we will respond to you within 48 hours to schedule an appointment. 16. Forms. This area will provide access to all WELL 4 U forms. Examples include: SWRC Consultation Worksheet; Exercise Tracking Sheet; Wellness Challenge tracking forms and flyers; etc. 17.
