Kirby Shakespeare Theater Group: 8 th Grade January 8, 2019 High School Orientation 2 C Motion to affirm that procedures have been followed and the Lebanon Township Board of Education approves of the actions taken in response to incidents that were classified as Harassment Intimidation and Bullying (HIB REPORT). 3 C Motion to approve the 2019/20 Special Education Extended School Year Program: July 5 th 2019 through August 8 th 2019. 1 F Motion to approve the transfer of funds for the month of September 2018 2 F As required by N.J.A.C. 6A:23A-16.10 (c)3, I, Abigail J. Postma, Board Secretary, certifies that no line account has been over expended as of September 30, 2018; Motion to approve, on the recommendation of the Superintendent, the acceptance of the monthly financial reports of the Board Secretary and the Treasurer for the month of September 2018 and further that, in compliance with N.J.A.C. 6A:23A-16.10 (c)4, the Board of Education certifies that as of September 30, 2018, after review of the secretary s monthly financial report and upon consultation with the appropriate district officials, to the best of our knowledge no major account or fund has been over expended in violation of N.J.A.C. 6A:23A-16.10 (b) and that sufficient funds are available to meet the district s financial obligations for the remainder of the fiscal year. 3 F Motion to approve the State of Emergency for the Woodglen School Building due to the public health and safety of the staff/students due to a lightning strike on August, 3, 2018;
3 Resolved that the following repairs were completed as necessary due to the lightning strike: Building Wi-Fi Access Point replacements with eplus Contract for the Wireless Access Points in the amount of $3,720.00; and the fire alarm system repairs by FAST Security & Fire ESCNJ 17/18-33 Contract in the amount of $2,582.00; Whereas the District is in the approval process with the District Insurance Company, New Jersey Schools Insurance Group, for the approval of the complete fire alarm replacement contract for the building in the amount of $38,650.00 with FAST Security & Fire ESCNJ 17/18-33 Contract; Whereas, the emergency has been filed as active insurance claim with the District Property Insurance Company, New Jersey Schools Insurance Group. 4 F Motion to approve the State of Emergency for the Woodglen School Building due to the public health and safety of the staff/students due to a roof leak in September 2018 which resulted in the damage to the interior of the building in the Girls Locker Room; Resolved that the District contracted environmental services, PARS Environmental Inc., is conducting the on-going air testing/sampling and management of the remediation project, including but not limited to the management, planning, testing and reporting of the cleaning/remediation project; Whereas the following cleaning/remediation are in process, and/or were completed as necessary due to the roof leak: Cleaning and Remediation with PuroClean/Mast Distasters Inc. contract in the amount of $11,052.37; Further Resolved that the District continues the cleaning protocols under the direction of PARS Environmental and will seek quotations for ceiling repairs; and Whereas, the emergency has been filed as active insurance claim with the District Property Insurance Company, New Jersey Schools Insurance Group. 1 Policy Motion to approve the first reading of Policy 0155, Board Committees as presented. 2 Policy Motion to approve the second reading/adoption and abolishment, as noted in listing, of the following policies and regulations as presented: P and R 1613 M NEW Disclosure and Review of Applicant's Employment History Policy 5512 M Revised Harassment, Intimidation, and Bullying HIB Policy 5512 M ABOLISH Harassment, Intimidation, and Bullying HIB P and R 5561 M Revised Use of Physical Restraint /Seclusion Tech for Students with Disabilities Policy 8561 M Revised Procurement Procedures for School Nutrition Programs
