Gurdarshan Singh's taxicab driver's licence will be renewed, effective immediately, subject to the following conditions: (1) Mr. Singh must provide to Municipal Licensing and Standards proof of payment of all or any outstanding fines; period of three (3) years to commence on April 25, 2013; (3) prior to each of the next three renewals of the licence, Mr. Singh must provide to Municipal Licensing and Standards, at his own expense, an updated abstract of his driving record; (4) during the probationary period, if Mr. Singh incurs any new charges or convictions under the Toronto Municipal Code or the Highway Traffic Act, he must notify Municipal Licensing and Standards, in writing, within three (3) business days; and (5) during the probationary period, if Municipal Licensing and Standards has 5935, and any updating material, shall be brought back before the Tribunal for a With the consent of all parties, the Tribunal ordered an amendment to the wording in Item No. 1 of the Pre-hearing Settlement: Mr. Singh must provide to Municipal Licensing and

April 25, 2013 Page 5 Malcolm Hassan Zoraik's taxicab driver's licence will be issued, effective immediately, subject to the following conditions: (1) Mr. Zoraik must provide to Municipal Licensing and Standards proof of payment of all or any outstanding fines; (2) immediately upon being issued, the licence will be placed on probation for a period of three (3) years to commence on April 25, 2013; (3) prior to each of the next three renewals of the licence, Mr. Zoraik must provide to Municipal Licensing and Standards, at his own expense, an updated abstract of his criminal record; (4) during the probationary period, if Mr. Zoraik incurs any new charges or convictions under the Criminal Code, he must notify Municipal Licensing and Standards, in writing, within three (3) business days; and (5) during the probationary period, if Municipal Licensing and Standards has 6041, and any updating material, shall be brought back before the Tribunal for a Settlement: Mr. Zoraik must provide to Municipal Licensing and 7. The Tribunal adjourned this matter to July 18, 2013, on consent of both parties, without prejudice to the City's right to seek a suspension of the licence on the adjournment date until any outstanding fines are paid. 8. (Report No. 5912) o/a Studio 9 Mr. Duy H. Vu, President Holder of Body Rub Parlour Owner Licence No. B38-3721115. The Tribunal approved a joint settlement in this matter and the following conditions will be placed on 1724640 Ontario Ltd.'s licence No. B38-3721115: (1) 1724640 Ontario Ltd., operating as Studio 9, admits to the facts contained in Report No. 5912 dated February 14, 2013; (2) 1724640 Ontario Ltd. must provide to Municipal Licensing and Standards proof of payment of all outstanding fines prior to May 15, 2013; (3) the licence will be suspended for a period of five (5) days commencing May 15, 2013; (4) the licence will be placed on probation for a period of two (2) years to commence on April 25, 2013; (5) during the probationary period, if 1724640 Ontario Ltd. incurs any new charges or convictions under the Toronto Municipal Code or the Criminal Code, he must notify Municipal Licensing and Standards, in writing, within three (3) business days; and (6) during the probationary period, if Municipal Licensing and Standards has 5912, and any updating material, shall be brought back before the Tribunal for a 10. 2124058 Ontario Inc.
