Commissioners Doc_24 Attendance at the Board s 5% test shall be limited to the following: Board employees and agents Representatives of the State Board of Elections, the U.S. Attorney, the Illinois Attorney General and the Cook County State s Attorney Representatives of county established political parties and qualified civic organizations ( Representatives ) authorized pursuant to Sections 24B-15 and 24C-15 of the Election Code (10 ILCS 5/24B-15; 5/24C-15) with post-election Representative credentials Members of the news media with special media credentials Authorized Representatives under Sections 24B-15 and 24C-15 of the Election Code (10 ILCS 5/24B-15; 5/24C-15) include representatives of (a) established political parties; and (b) qualified civic organizations that registered with the Board at least 40 days before the election pursuant to sections 7-34 or 17-23 of the Election Code (10 ILCS 5/7-34; 5/17-23) (list of qualified civic organizations attached or available upon request). Representatives must present a completed post-election credential issued by the Board of Election Commissioners. Such credential must be authorized by the real or facsimile signature of the chairman of the State or county political party committee or the president of the qualified civic organization, as the case may be. Established political parties shall be entitled to have one (1) Representative per station where ballots are being tested at a primary election, and two (2) Representatives per station at a general election; and qualified civic organizations shall be entitled to have one (1) Representative per station. Persons authorized to attend the 5% test may observe the proceedings, but they may not touch any election materials. Photographs and filming will be permitted; however, no photographs or filming of individual ballots (whether optical scan or touchscreen ballots) shall be permitted. Board employees will conduct their duties without delay; they will not stop or slow down the performance of their duties simply to allow photographs or filming or to answer questions from Representatives or observers. Board employees shall refer questions, problems or requests to a Supervisor. Board employees, supervisors and agents are not giving consent to have their conversations audio recorded. Requests for copies of any materials shall be made in writing to the Board s Freedom of Information Officer (consult the Board s web site at www.chicagoelections.com). Commissioners Doc_25 CERTIFICATE OF RESULTS TEST OF BALLOTS IN 5% OF PRECINCTS FEBRUARY 26, 2019 MUNICIPALGENERAL ELECTION FORM 5%P We, the undersigned employees of the Board of Election Commissioners for the City of Chicago, being designated for the purpose of testing ballots cast in 5% of precincts as randomly selected by the Illinois State Board of Elections, do hereby certify that for Ward Precinct, we followed the procedures given to us and performed the following tasks: 1. We broke seal # on the Transfer Case. 2. We initialized the ballot scanner for this precinct and verified that there were zero (0) votes cast and counted in the memory. A zero tape was printed (do not detach the zero tape). 3. We ran all the ballots counted on Election Day through the ballot scanner, observing that the Public Counter increased with each ballot card. 4. We generated the first totals tape and detached the entire tape containing the totals and zero tape. We initialed the tape, attached the tape to the top copy of this form (white copy). 5. We generated and detached three additional totals tapes, initialed the tapes, and attached two to the bottom copies of this form (yellow and pink copy). 6. We recorded the vote totals from the ballot scanner results tape on the Insight Tally Report. The IT Department staff compared the results with the Election Day totals to verify whether they match. 7. We examined the votes cast for each candidate and referendum (if applicable) on the Voter Verified Paper Audit Trail (VVPAT) and recorded such votes on the E2P Tally Report. We broke seal # on the Transfer Case. 2. We examined the votes cast for each candidate and referendum (if applicable) on the Voter Verified Paper Audit Trail (VVPAT) and recorded such votes on the E2P Tally Report. The IT Department staff compared the results with the Election Day totals to verify whether they match. 3. We signed this form, and returned VVPAT into the transfer case along with the top copy of this form (white copy) and sealed the transfer case with seal #. 4.
