F. ACTION ITEMS (Pages 5-7 ~ MINI CONTRACTS) CONSENT AGENDA FOR MINI CONTRACTS F.1.a. THROUGH F.1.j. 1. Mini Contracts 1.a. Approval of Agreement Between Sachem Central School District and MSG Varsity Network, LLC. Superintendent of Schools, the Board of Education approves the License agreement between Sachem Central School District and MSG Varsity Network LLC for the purpose of exhibiting the football game on October 15, 2010 between Sachem High School North and William Floyd High School on MSG Varsity, MSG Varsity Network, MSG Varsity Network HD, MSG Varsity Interactive channel, MSG Varsity Video on Demand, and the MSG Varsity Network website. The agreement was not received in time to be approved prior to the date of the game. This agreement has been reviewed and approved by the school district s attorney. 1.b. Approval of Agreement Between Sachem Central School District and National Archery between Sachem Central School District and National Archery in the Schools Program of New York for the loan of certain archery equipment to be used for archery in the schools conducted in cooperation with the New York State Department of Environmental Conservation (NYSDEC). If the equipment is lost, stolen, or damaged, except for normal wear and tear, the district shall reimburse NYSDEC for the cost of the equipment or replace the equipment inkind. This agreement has been reviewed and approved by the school district s attorney. 1.c. Approval of Agreement Between Sachem Central School District and Harlem Wizards Entertainment Basketball, Inc. between Sachem Central School District and Harlem Wizards Entertainment Basketball Inc. to play an entertainment basketball exhibition game at Sachem High School East for a fundraiser. The Wizards will receive a minimum guaranteed fee or 50% of ticket sales, whichever is greater. This contract has been reviewed and approved by the school district s attorney. 5

Approval of Agreement Between Sachem Central School District and Music Theatre International (MTI) - Sequoya between the Sachem Central School District and Music Theatre International (MTI) to provide show kits, pay a royalty fee and non-refundable materials fee for the production of Disney s High School Musical Junior at Sequoya Middle School. The cost is approximately $979.00 to be paid for by the Music and Art Department. 1.e. Approval of Agreement Between Sachem Central School District and Music Theatre International (MTI) -Cayuga between the Sachem Central School District and Music Theatre International (MTI) to provide show kits, pay a royalty fee and non-refundable materials fee for the production of Disney s Aladdin Junior at Cayuga Elementary School. The cost is approximately $590.00 to be paid from the Cayuga Extra Classroom Activity Fund. 1.f. Approval of Agreement Between Sachem Central School District and Mark Stolzenberg between Sachem Central School District and Mark Stolzenberg to provide two (2) artistic presentation assembly programs on November 4, 2010 for ESL Community Night. The assembly programs are titled Circus Follies and Basketball Poetry in Motion. The cost of the Presentation will be $1,600 and will be paid for through Title III ESL grant funds. The term of this agreement shall be for the period from September 1, 2010 and shall continue through and including November 30, 2010. This contract has been reviewed and approved by the school district s attorney. 6

(Pages 5-7 ~ MINI CONTRACTS) 1.g Approval of Agreement Between Sachem Central School District and Ride the Ducks between Sachem Central School District and Ride the Ducks, a historical tour through Philadelphia for the Sachem East String Ensemble on May 15, 2011. The approximate cost of $355.50 will be paid for by the students. This contract has been reviewed and approved by the school district s attorney. 1.h. Approval of Agreement Between Sachem Central School District and Grosh Scenic Rentals Sachem North Superintendent of Schools, the Board of Education approves the Lease/Rental agreement between Sachem Central School District and Grosh Scenic Rentals, Inc. for scenery rental for Sachem High School North s drama production. The cost of $685.50 will be paid by the Sachem High School North Drama Club. This agreement has been reviewed and approved by the school district s attorney. 1.i. Approval of Agreement Between Sachem Central School District and Wireless Sounds LTD Sachem North between Sachem Central School District and Wireless Sounds Ltd. for sound system rental for Sachem High School North s drama production. The cost of approximately $2,700 will be paid by the Sachem High School North Drama Club. This agreement has been reviewed and approved by the school district s attorney. 1.j. Approval of Agreement Between Sachem Central School District and AFLAC Reimbursement Services between Sachem Central School District and AFLAC Reimbursement Services for the district s flexible spending account (FSA). There is no cost to the district for services and no set-up fee. This agreement will automatically renew for successive periods of twelve (12) months unless the district or AFLAC gives written notice not to renew. 7

2. Approval of Verizon Maintenance Agreement Extension of Schools, the Board of Education approves a 2 month extension of the current telephone maintenance agreement with Verizon for Alcatel Internetworking at Sachem High School East and Sequoya Middle School. The cost for this 2 month extension will be $1,888.12 per month for a total of $3,766.24. The service term will be from November 10, 2010 through January 09, 2011. 8

5. Approval of Budget Transfers $50,000 or Greater of Schools, the Board of Education approves two budget transfers of $50,000 or greater : One transfer for $4,733,698 is to move ARRA Funded Salaries in to the ARRA Budget Codes Location 99. One transfer for $1,039,208 is to move salaries for 10 positions not replaced in the budget to the negative offset code. 6. Additional Appointments to the 2010-11 Committee on Preschool Special Education Superintendent of Schools, the Board of Education approve the following additional appointments to the Committee on Preschool Special Education for the 2010-11 school year : Parent Members: Jill Mislin Natalie Krempa 7. Appointment of Chairperson Committee for Special Education for the 2010-11 School Year of Schools, the Board of Education approve the appointment of Chairpersons for the Committee for Special Education for the 2010-11 school year as follows : Thiele, Angela Secondary 8. Approval of Applied Behavioral Analysis (ABA) Specialists of Schools, the Board of Education approve the following personnel be approved as Specialists for the home ABA Program : Valle, Lauren 9.
