INSTRUCTIONS 1. Read Petition carefully and fill in all blanks where required. 2. Petitioners must sign in their own handwriting, giving their residence and Post Office addresses under the words Signatures of Petitioners, in the presence of some one person. 3. The person in whose presence the Petition was signed must sign the Petition and make the affidavit on page five before some officer authorized by law to take affidavits. 4. The Candidate or Candidates must sign the Certificate of Acceptance on page five, stating that he, she or they is or are qualified for the office and a member of the political party mentioned in the Petition. 5. File Petition with the Municipal Clerk not later than 4:00 p.m. 57 days prior to the date of the Primary Election. NUMBER OF S REQUIRED (See R.S. 19:23-8) Equal to at least 5% of vote cast for your political party at last Primary Election for members of General Assembly for your Municipality and/or ward.
