You are the guardian of a child under 18 years of age who is living in Finland on a residence permit or is a Finnish citizen. You intend to move to Finland and to lead a family life with the child. In addition to this form, the child living in Finland must fill out form PK3_plus. Enclosing the clarification form with the application proper will make processing of the application quicker. Carefully fill in and sign the application. Incomplete information will delay the processing of the application and may lead to its rejection. An application may be submitted to a Finnish diplomatic mission abroad or at a service point of the Finnish Immigration Service in Finland. A processing fee will be collected when the application is submitted, according to the Decree on charges. Read the instructions for filling in the form at www.migri.fi Clarifications that you need to append to your application are listed in Section F of this form. In that case, the personal information in your passport, as well as your address, mother tongue and occupation, will be recorded in the Population Information System, which is Finland s national population register. The 11-character personal identity code is based on your sex and date of birth (e.g. 300171-931H). You will need it when contacting the Finnish authorities.
