May 2018 Dear 8th Grade Parents: The 8th grade team of teachers is excited about this year s trip to Camp Kulaqua. Our students will be participating in a two-day outdoor education program Thursday, August 23 and Friday, August 24. We will depart from Trinity Prep at 7:30 a.m. on Thursday morning and we will return at approximately 5:00 p.m. on Friday. Please make arrangements to have your child picked up on time. We have chosen curriculum that fosters cooperation and team building skills. Accommodations are in air-conditioned cabins. Excellent meals, including vegetarian choices, are served in a dining room and in picnic areas for lunch. If your child has any special dietary needs, please indicate those on the permission slip. Why miss classes for outdoor education? Our students can learn things away from their familiar environment and close to nature that we simply cannot teach as well in the classroom. This program is designed to help them: Strengthen relationships and develop new ones, Gain a better sense of self-identity and independence, Foster attitudes of inquiry and openness to challenge, Accept responsibility more readily, and Gain a better understanding of the environment and their relationship to it. Participant Agreement RELEASE, EXPRESS ASSUMPTION OF RISK, CONSENT, WAIVER, & INDEMNITY AGREEMENT READ CAREFULLY THESE IMPORTANT CONDITIONS FOR PARTICIPATION THAT AFFECT YOUR RIGHTS AND OUR LIABILITY (Initial and sign below to confirm your agreement) In consideration for the opportunity to participate in the adventure recreational services, consisting of cables, lanyards, harnesses, pulleys, elevated walkways and platforms, powerfan rapid descent machine, climbing walls and zipline, provided by the Florida Conference of Seventh Day Adventists d/b/a Camp Kulaqua, I, the undersigned participant, hereby acknowledge and agree: I understand that, although uncommon, risks of serious injuries, illness, or death always exist, and cannot be eliminated in adventure recreational activities such as these, due to falls, to contact with objects, other persons, or the environment, or to moving and motion from being transported in the activities; and that there can be risks of emotional or psychological injury or distress, resulting from personal touching, whether necessary, unwelcome, or inadvertent, in the preparation for participation, as well as a range of emotions from simple hurt feelings to panic or psychological trauma (such as fear of heights). I expressly hereby assume the risk of such injury, illness or death, that may occur as a result of my participation in the activities, whether resulting from the negligence of any party, including myself, and also even the negligence on the part of Camp Kulaqua, its respective employees, officers, directors, stockholders, agents, successors-in-interest, tower engineers, and contractors, and assigns (Released Parties) themselves, whether passive or active. I represent and agree that I am in reasonably good health and physical condition, weigh not less than 65 lbs nor more than 250 lbs, am appropriately dressed, am not pregnant, have no existing injuries or limitations, have no musculoskeletal disorders, have no heart condition or condition of hemophilia, do not have epilepsy or other seizure disorders, and am not under the influence of alcohol, or any drug, prescription or illegal, or any other substance that would affect or impair my judgment in order to participate in the adventure recreational activities provided by Camp Kulaqua. I hereby waive and release, discharge, and covenant not to sue, forever, Released Parties for any claims of liability against them, whether for any injury, death, loss, or other damages to myself, my family, my heirs, or my assign, and representatives. By signing this Release, Express Assumption of Risks, Consent, Waivers, & Indemnity Agreement, I hereby acknowledge that I understand the conditions stated in it, and that my participation in the recreational activities provided is solely based on those conditions and my agreement to them. I further agree to save, defend, indemnify, and hold harmless (i.e., defend and pay, including costs and attorneys fee, including appellate proceedings) Released Parties from any claim or lawsuit by me, or by anyone purporting to act on my behalf, my family, my estate, my heirs, or my assigns, for damage, injury, or death, arising directly or indirectly out of my choice to participate in the recreational activities provided. I intend that this Agreement be enforceable to the fullest extent provided by law, and in the event that any of the terms set forth in this Agreement or any word, phrase, clause, sentence (including without limitation any geographic, temporal, or participatory restrictions), part, or provision should be found to be illegal, void, or unenforceable for any reason, such word, phrase, clause, sentence, part, or provision shall be modified or deleted in such manner as otherwise required, in to extend the fullest effects, rights, duties, and protections under this Agreement, as so modified, so that the validity of the remaining words, phrases, clauses, sentences, parts, and provisions shall not be affected thereby and shall be deemed, interpreted, and enforced, as being severable and independent from such illegal, void, or unenforceable provisions, and said illegal or invalid part, term or provision shall be deemed not to be a part of this Agreement and all other valid provisions shall survive and continue to bind the parties, and be interpreted and enforced, as if such illegal, void, or unenforceable provisions were never a part of this Agreement. This Agreement shall be interpreted and enforced according to the laws of the State of Florida, notwithstanding the choice-of-law rules or conflicts of laws principles of this State, or of any other state, territory, province, or nation; and any claim or action relating to, or arising out o, this Agreement may be brought only in a court located in, or comprising, Alachua County, Florida; and only after participation in a presuit mediation conference, which is a prerequisite condition to bringing suit. I agree to follow and comply with all conditions, rules, and directions by Released Parties, and that failure to do so is reasonable basis to be excluded from participation in the recreational activities provided.

Horseback Riding Permission Slip Under Florida law, an equine activity sponsor or equine professional is not liable for any injury to, or death of, a participant in equine activities result from the inherent risk of equine activities. I have read the above statement and by signing below, I realize that I am riding at my own risk. Maximum rider weight 250 lbs No physical or mental limitations that would affect the rider s ability to control their horse. Pregnant guests are not permitted to ride. Jeans are suggested for comfort/closed shoes with hard soles that cover the entire foot are required. Minimum charge of $30 if no-show. Camp Kulaqua Details On Thursday morning dress in comfortable clothes you can wear for outdoor activities there will be no place to change before we begin our canoe trip. Boys may wear board shorts and T-shirts, and girls may wear bathing suits with shorts and shirts over them. (If girls plan on taking off their shirts to swim, the bathing suit must be a one piece or a tankini, where the top and bottom meet.)

Be sure to bring insect repellent and sunscreen. CLOSED-TOED SHOES MUST BE WORN AT ALL TIMES INCLUDING on the bus and in the lodges. It is suggested that you bring an old pair of tennis shoes for some activities. Watershoes are fine as long as they are closed in all the way around. You will need a pair of dry shoes when we are at camp. You may bring movies for the bus G or PG only. We would prefer that you do not bring electronic devices with you. You will not be able to use them in the cabins. The general rule is: don t bring anything that you would be upset losing. Meet the buses at 7:30 a.m. by the DAC. Your bags and bedding will be stored under the bus. Anything that you want to have before we get to the campsite must be with you ON THE BUS (towels, money for the rest stop, etc.). We will travel to High Springs (northwest of Gainesville), making one stop at the rest stop near Gainesville. You may purchase a snack then, so you should bring coins (sometimes the change machines don t work). After we arrive at camp we will break up into smaller groups for activities. Later, there will be evening activities as well dinner, hayride, bonfire, and s mores are just a few. Lights-out is at 11:00 p.m. All cabins have inside bathrooms and air conditioning, so no one will be outside his or her assigned cabin after lights-out. ABSOLUTELY no girls allowed in boys cabins or boys allowed in girls cabins. Friday morning we will eat breakfast, clean up our lodges, and load the buses that take us to the tubing site. Bring the water shoes to the State Park. You may put them on right before we get off the bus. We will tube down the Ichetucknee River, have a cook-out for lunch, change into our dry clothes, and head for home. Make sure you have a towel, dry clothes, and dry shoes to change into with you on the bus. Parents must be at school by 5:00 p.m. to meet the bus as we arrive. It is not OK to call your parents when we get back to the school they need to be there waiting for you.
