THE BIG USELESS DEBATE! Is the TB infection rate in RSA mines: 3,000 7,000 per 100,000 population? 2,984 per 100,000 population? 5%? Does it matter which is correct? Whichever is correct: Its higher than the RSA population rate of 1% estimated by WHO & that of Swaziland (1,200 per 100,000 population) Fangs of the snake are in RSA mines!

GLOBAL & REGIONAL RESPONSE October 2010 STOP TB Partnership Board meeting in South Africa: Concern raised about worsening TB situation in mines by: Minister of Health Lesotho Minister of Health South Africa Minister of Health Swaziland Global partnership comprising Stop TB Partnership, World Bank, IOM Issues taken up by SADC Ministers

SADC RESPONSE RSA will prioritize cutting of the head of the snake But Fully committed to participating in the SADC region-wide process to coordinate cross-border management of TB & implement the Declaration with focus on: 9 Harmonization of policies & guidelines Referral systems between countries

SADC DECLARATION ON TB IN MINING INDUSTRY Establish Ministerial Commission with tripartite representation Classification of TB as an occupational disease to be compensated throughout SADC Harmonization of treatment policies & guidelines Enforce limits to silica dust exposure Mines to be responsible for treatment including post-employment 10

PROGRESS TO DATE - RSA Government political will: Deputy President Responsible for TB & HIV programmes in South African government on World TB Day 2012, issued guidance to mines: 11 Screen & test miners of TB at least twice a year Expand access to peri-mining communities to mines health services Procure Genexpert (RSA will reach 100% coverage by end of 2013) Provide MDR-TB management & care

PROGRESS TO DATE - RSA Potential funding from Global Fund, World Bank and DFID to: Development of a system to trace all contacts of miners who have TB Development of a service delivery model to expand services in peri-mining communities Establishment of an electronic monitoring system to trace all miners with TB Restructuring of compensation system for 12 ex-miners to be more responsive & efficient

PROGRESS TO DATE - SADC Signing of Declaration by Heads of government Economic analysis of the cost of TB in the mining sector Draft Harmonization document produced and will be discussed with technical teams in South Africa, Swaziland, Lesotho and Mozambique Draft action plan to pilot service delivery models in one mine, a labor sending area in SA and one labor sending country 13
