DS-11 Form DS 11 Form can be completed online or in hardcopy Hardcopy is 6 pages in total Pages 1 4 are instructions Pages 5 6 are submitted with application packet Must be completed in black ink Applying for a: US Passport Book Standard Size (non-standard is more money and only necessary if you plan to travel extensively) Mailing address should be where you would like your passport sent in 4 6 weeks Sample
DS-11 Form Do NOT attach photo to form (see following Passport Photo information) Sample Parents Information Full Name (including mother s maiden name) Date of birth Place of birth Sex US Citizen Statue Do NOT sign application!!! You will need to sign in presence of Passport Agent

Certified Birth Certificate To be valid must have: Registrar s raised, embossed, impressed or multicolored seal Registrar s signature Date the certificate was filed with the registrar's office (which must be within 1 year of birth) Must be original (see following slide about mailing important documents)
Identification Primary (one of the following) Previously issued, undamaged U.S. passport Naturalization Certificate Valid Driver's License Current Government ID (city, state or federal) Current Military ID (military and dependents) Secondary (you must submit as much secondary identification as possible) Present a combination of documents that can be used to verify your personal identity. These documents are not acceptable as secondary identification when presented alone. Example: Social Security Card + Credit Card + NYU Student ID + Library Card
Identification If you apply at NYU Passport Day 2012 and submit out-of-state primary identification, you must present an additional ID document. For example, if you apply in Maryland with a Virginia s Driver s License, you must present a second ID containing as much of the following information as possible: Photo Full name Date of birth Date Issued
Photocopies A copy machine will be available on site at Passport Day for students use Front and back of each ID document Plain white, 8 ½ x 11 standard paper stock (don t cut, fold, etc. to size) Images on only one side of each page submitted (can t be double sided copy) No other images and/or markings (no recycled paper, hand prints in background, etc.) Image must be same size or bigger (don t shrink)
Fee Two separate payments required $110 for the passport book (Payable to U.S. Department of State) $25 Processing fee (Payable to U.S. Post Office) Cash or Check only (no credit cards) * Fees for First Time applicants only
Passport Photo Official Passport Style Photos: In Color Printed on photo quality paper 2 x 2 inches (51 x 51 mm) in size Sized such that the head is between 1 inch and 1 3/8 inches (between 25 and 35 mm) from the bottom of the chin to the top of the head Taken within the last 6 months to reflect your current appearance Taken in front of a plain white or off-white background Taken in full-face view directly facing the camera With a neutral facial expression and both eyes open Taken in clothing that you normally wear on a daily basis NYU Card Center Photos are available and eligible
Appointment First time applicants must have an appointment to apply in person Set up an appointment during NYU Passport Drive Complete NYU Passport Drive Day Registration Form OR Set up an appointment with passport agency or acceptance station (authorized post office) Full list of locations available on http://travel.state.gov/passport/
Mailing Materials (birth certificate, social security card, passport application, etc.) Traceable Delivery Method Mailing service that allows you, the sender, to confirm the receipt of your package at the intended destination. Traceable delivery methods include: USPS Priority Mail with Delivery Confirmation USPS Priority Mail with Signature Confirmation USPS Express Mail (for overnight delivery requests) traceable delivery method USPS Certified Mail and USPS Registered Mail (can take longer) The traceable delivery method used must be able to deliver to a PO Box address. Secure Packaging Mail your passport application and any personal documents using a secure means of packaging, such as a Tyvek envelope Keep Copies / Records Make sure to keep copies of the tracking numbers and complete photocopies of all materials mailed
