HAVLICEK SCHOOL COMMONS ROOM 6401 W. 15 th STREET, BERWYN, ILLINOIS The Regular Meeting of the Board of Education, School District 98, Berwyn, Illinois, was called to order at 6:41 p.m., November 20, 2014 by President Lancsak. Present - Members Mounce, Swade, Arenella, Paul-Emile, Cuevas, Valeriano and Lancsak Superintendent Dr. Ayala Assistant Superintendent Dr. Zaher Business Manager Regina Johnson Julissa Avila, Katherine Echeverria, Julian Flores, Julian Garcia, Stephen Mendoza, Naomi Nava led the assembled group in the Pledge of Allegiance. COMMUNICATIONS A. Student Achievement Recognition: The Board of Education, Dr. Ayala and Dr. Zaher recognized two of our top performing students scoring the highest in ISAT in grades 3-8. The students and their families attended the Board meeting and the students were presented with a certificate and medal. B. Tax Levy Presentation Regina Johnson, Business Manager presented information about our Tax levy and how it generates local funds. BOARD COMMENT A. Stephen Lancsak, Board President thanked the Berwyn Fire Department for the coats that had been presented to the 1 st grade students at Prairie Oak Elementary School. Board member, Brian Swade and Dr. Ayala, Superintendent personally reached out and thanked the Berwyn Fire Department for their contributions. B. November 15 th was Board Member Appreciation Day. All Board members were appreciative and thankful for the job well done on all of the thank you cards they received.

Personnel Report A. New Hires: Substitute: 1. Daniel Anwar- Substitute Teacher 2. Stevana Tito Substitute Teacher 3. Debra Williams Substitute Teacher B. Leave of Absence 1. Nancy Cioe, a first grade teacher at Havlicek Elementary School currently on FMLA requests an extended leave of absence from December 17, 2014 through the end of the school year. 2. Angela Kuczak, first grade teacher at Prairie Oak Elementary School requests an FMLA Leave to begin January 18, 2015 through April 19, Leigh Miles, an eighth grade math teacher at Lincoln Middle School requests a Leave of Absence from December 19 through the end of the school year. C.Resignation: 1. Denise Ramos a second grade teacher at Havlicek Elementary School submits a letter of resignation effective November 11, 2014 for the school year. 2. Rosalina Resendez an EL Kindergarten Teacher at Prairie Oak Elementary School submits a letter of resignation effective November 24, 2014 for the school year. E. Building Use Applications 1. 5 APPROVAL OF ITEMS ON THE CONSENT AGENDA Member Arenella moved and was seconded by Member Valeriano that the Board approve the following items listed on the Consent Agenda A. Approval of Minute items 1-4, B. Personnel Report A. New Hires items 1-3, B. Leave of Absences items 1 and 3, C. Resignations items 1-2, C. Building Use Application items 1 and 2 and D. Finance items 1-3 as submitted. 6 Tax Levy Resolution APPROVAL OF RESOLUTION FOR TAX LEVY Member Arenella moved and was seconded by Member Cuevas that the Board approve to adopt the Tax Levy Resolution Ayes - Mounce, Arenella, Paul-Emile, Cuevas, Valeriano Swade, and Lancsak INFORMATION AND REPORTS 1. Monthly Fund Report Regina Johnson, Business Manager, provided the monthly status report as of October 31, 2014 the District had balances of $35,406,691 in the Education Fund, ($39,458) in the Tort Fund, $2,920,889 in the O&M Fund and $166,369 in the Life Safety Fund. The District s combined operating fund balance (Education/Tort/O&M/Life Safety Funds) as of October 31, 2014, was $38,454,491 which represents a decrease of $504,564 from the previous month s balance.

7 Upcoming Events November 20, 2014 Community Conversations starting at 4:00 in Spanish and 5:00 in English at Havlicek School Board of Education meeting starting at 6:30 at Havlicek School November 21, 2014 Triple I Board member Conference in Chicago (Dinner together that evening) Erie Café, 536 West Erie Street, Chicago Our dinner will begin at 6:30. November 26, 2014 District will be closed November 27-28, 2014 Thanksgiving Holiday December 4, 2014 Curriculum coordinating Council at the PTLC starting at 3:30 Parent/Teacher Advisory Committee at the district office starting at 5:00 December 5, 2014 District Holiday party at the VFW starting at 4:00 December 18, 2014 Community Conversations at Havlicek starting in Spanish at 4:00 and English at 5:00 Board of Education meeting at Havlicek starting at 6:30 December 22, January 2, 2015 Winter break
