(Atlanta, GA) to povide tanspotation sevices fo the Office of Aging and Behavioal Health pogams. Effective dates: Januay 1, 2015 though Decembe 31, Requiement fo Boad Action (Cite specific Boad policy, statute o code equiement) In accodance with Puchasing Code Section , contact modifications within the scope of the contact and necessay fo contact completion of the contact, in the specifications, sevices, time of pefomance o tems and conditions of the contact shall be fowaded to the Boad of Commissiones fo appoval. Develop a netwok of integated and effective health and human sevices that impove health outcomes and pomote health equity. Objective 4 - Suppot aging in place and a high quality of life fo senios. (Fist sentence includes Agency ecommendation. Povide an executive summay of the action that gives an oveview of the elevant details fo the item.) The Aging and Youth Sevices Depatment equests appoval to amend the existing contact in ode to incease spending authoity to povide tanspotation sevices fo the Office of Aging and Behavioal Health pogams. The incease in spending authoity is needed to pay fo sevice costs incued in Cuently, the Depatment is unable to pay fo sevices endeed as the spending authoity fo 2014 has been exhausted due to the below 2013 expenditues and 2013 billing adjustments applied to the 2014 contact. Additionally, histoical pactices of applying pevious yea expenditues to the cuent yea will have to cease as of FY2015 due to this being the last yea fo the cuent tanspotation contact. This sevice will povide access to sevices fo appoximately 2,500 senios age 60 and olde; and appoximately 200 developmentally disabled Fulton County esidents age 18 and olde.
