1 2018 Nomination Form for the office of CSU Council of Representatives Thank you for showing interest in the CSU's 2018 General Elections. The following form must be completed and submitted before March 9, 2018 at 5:00pm, if you hope to be a candidate for CSU Council. Please read all directives, by-laws and standing regulations carefully, and make sure to fill in all information fields as requested. COMPLETED NOMINATION FORMS MUST BE SUBMITTED IN PERSON, BY THE PROSPECTIVE NOMINEE, TO BE VALID. There are no exceptions to this, forms that are placed under the door of the CSU or CEO office will not be accepted. Forms must be submitted to a CSU Elections Officer at the CEO office (H-462-3). Upon submission, please be sure to sign the nomination acceptance on page 2 of this form in the presence of a CSU Elections Officer. Further inquiries should be directed to the Chief Electoral Officer, at their coordinates below, or check the CSU Please read the CEO's election directives package and the CSU's electoral regulations carefully. 2 2018 Nomination Form for Council of Representatives Candidate Biography: Before Candidates nomination forms are to be accepted candidates must submit a brief biography via to ceo@csu.qc.ca. Candidate bios will be posted on the CSU's election website. The biography must be under 500 words and can include your platform, experience, etc. A second biography of 75 words or less is required to be placed at polling stations. Nomination forms will not be accepted if bios have not been submitted at the time of submission. I, (full name) do attest that the above information is true and accurate, to the best of my knowledge. I have completed all information in the fields above, including my name, Concordia ID #, registered faculty and submitted my candidate biography to the CEO. I am eligible to run as a candidate for the CSU Council of Representatives, and at least 45 eligible signatories have signed pages 3-6 of this form. I have read the CEO Directives package and understand the, electoral rules and policies and understand that I must abide by them to remain a candidate. 3 2018 Nomination Form for the office of CSU Council of Representatives To become a candidate for CSU Executive, a nominee must gather the signatories of at least 45 eligible voters for the position in which they are running. All signatories must be registered undergraduate students at Concordia University as of January 22, Signatories must fill in all fields to be counted. It is recommended that you collect extra signatures just in case some of the signatures you collect are from students that are in the wrong faculty or are grad students.
