It passed The committee made a motion to approve four applications with conditions: Wellcare Compounding LLC, approval pending submission and approval of completed compounding worksheets and an operational inspection report; Talon Compounding Pharmacy, approval pending submission and approval of an updated inspection report from the Texas Board of Pharmacy; SZY Holdings, LLC, approval pending payment of $250 fine for shipping into South Carolina without a permit; Aspirar Pharmacy, approval pending submission and approval of an operational inspection report from the North Carolina Board of Pharmacy. It passed The committee made a motion to approve seven applications with conditions: PIMD International, approval upon submission and approval of their May 2015 Florida Board of Pharmacy inspection; OHM Pharmacy Services, approval of computerized compounding worksheets from May 1 to May 10, 2015; Skip's Pharmacy, approval with submission of an amended application to indicate sterile compounding; Y Medical Associates, approval with submission and approval of recent Texas Board of Pharmacy inspection; Applegee BioPharm, approval with submission and approval of recent New Jersey Board of Pharmacy inspection; Physicians RX Pharmacy of Clinton, approval with submission and approval of a recent Maryland Board of Pharmacy Inspection; Cystic Fibrosis Services, approval with submission of a statement indicating how CFS will remain compliant with South Carolina law regarding pharmacist - technician ratio. It passed The committee made a motion to deny American Custom Compounding, due to lack of evidence of sufficient compounding training and experience of pharmacists and technicians.
