1 CERTIFICATION BY ELECTION SERVICES SOLUTIONS Pursuant to a request of SEIU Local 1021, Election Services Solutions designated Maralin Falik, President as the Local 1021 Election Administrator and Saundra Tovin, Vice President as the Election Supervisor for the 2013 Election of SEIU Local ELECTION SERVICES SOLUTIONS FURTHER CERTIFIES that the candidates running for office who provided personal addresses received an electronic sample ballot prior to printing. ELECTION SERVICES SOLUTIONS FURTHER CERTIFIES THAT 47,000 ballots were printed and 42,932 ballots were mailed to members as provided in a database from SEIU Local 1021 to Election Services Solutions on February 8, 2013 from Handled with Care, Wicks Boulevard, San Leandro, Ca Although invited to be present at the printing and mailing of the ballots, there were no observers or candidates in attendance. Attached hereto and considered part of said certification are the United States Postal Services receipts for the mailing and the certifications of printing by Lithograph Reproductions, Inc., and mailing by Handled with Care. ELECTION SERVICES SOLUTIONS FURTHER CERTIFIES that in addition to the 42,932 ballots mailed, 59 ballots were distributed as duplicate ballots and 12 names were added to the mailing list after February 8, In addition, 73 ballots were r ed to better addresses provided by the United States Postal System or by the SEIU Local 1021 Membership Department after being returned as undelivered. ELECTION SERVICES SOLUTIONS FURTHER CERTIFIES that the candidates running for office who provided personal addresses received a weekly activity report and election updates, including the names of the members who requested duplicate ballots (59); whose envelopes were returned undelivered and better addresses were obtained (73); whose envelopes were returned and better addresses were not available (690); and the names of individuals whose names were not on the original mailing list of February 8, 2013 and were added to the list (12). 2 ELECTION SERVICES SOLUTIONS FURTHER CERTIFIES that all returned envelopes were secured at the designated Post Office Box in Oakland, CA until 9:00 A.M. on March 8, Observers were present at the collection of mail and witnessed the transporting of the ballots, sealed in mailbags and signed by observers, to the tabulation site, the SEIU Local 1021 Myrtle Street office located at 155 Myrtle Street, Oakland, CA Observers, candidates, Local 1021 members and staff present at the tabulation were provided printed final interim results upon completion of the tabulation results. ELECTION SERVICES SOLUTIONS FURTHER CERTIFIES that Election Services Solutions representatives reviewed all procedures that would be followed at the tabulation and showed all present the remaining ballots (3224). The unused ballots were sealed, with the signatures of observers present on the packages and secured prior to the start of the tabulation. ELECTION SERVICES SOLUTIONS FURTHER CERTIFIES that 3792 envelopes were returned to P.O. Box by 9:00 A.M. on March 8, 3013 of which 48 envelopes were not included as follows: 1-envelope was empty and not sealed 2-envelopes were not returned in the Business Reply Envelope 1-envelope contained a change of address card only 4-envelopes had removed identification on the reverse side 31-envelopes were substitute ballots which did not affect the outcome of the election (7 were required for the position of Special Districts Industry Chair and only 2 were found to be eligible) 7-envelopes were names added to the list after the initial mailing which did not affect the outcome of the election 2-envelopes were original and substitute ballots from the same member and were challenged. 3 ELECTION SERVICES SOLUTIONS FURTHER CERTIFIES that two (2) envelopes were hand delivered to the Local 1021 Office at 100 Oak Street, Oakland, CA which were not included as all ballots were required to be sent through the post office. ELECTION SERVICES SOLUTIONS FURTHER CERTIFIES that printed listings of the 3744 eligible envelopes returned representing the members who were eligible to vote were produced in alphabetical order for review by observers and candidates present. ELECTION SERVICES SOLUTIONS FURTHER CERTIFIES that of the 3744 envelopes to be opened, the following 25 envelopes/ballots were not included as follows: 19-ballots were blank 1-ballot was blank (member claimed not being eligible to vote) 2-ballots were over-voted 3- secret ballot envelopes were empty ELECTION SERVICES SOLUTIONS FURTHER CERTIFIES that a total of 3719 valid ballots were cast as to the true and accurate results of the election.
