Approval Of Agenda (Additions/Corrections/Deletions) On a motion by Commissioner Mike Slavik, seconded by Commissioner Kathleen A. Gaylord, the agenda was unanimously approved. Consent Agenda On a motion by Commissioner Thomas A. Egan, seconded by Commissioner Nancy Schouweiler, the consent agenda was unanimously approved as follows: 6.1 Approval Of Minutes 6.2 Authorization To Execute Contracts And Joint Powers Agreements For Community Corrections Services WHEREAS, Community Corrections supervises adults and juveniles placed on probation by the courts; and WHEREAS, probation staff focuses on community safety and reducing recidivism in their supervision of these
2 November 29, 2016 Page 2 individuals; and WHEREAS, supervision, services, and court ordered programs are provided through direct care delivery and purchased services, and a variety of services are in place to meet the varied needs of these individuals; and WHEREAS, staff recommends authorization of contracts with Alternatives in Healing, LLC for sex offender treatment services; with River Ridge, LLC, for the delivery of the One-Day Driving While Intoxicated Program; and with General Security Services Corporation for supervision of the work crews as part of the Sentence to Service Program; and WHEREAS, Sentence to Service provides supervised work crews for non-violent adults receiving probation services as an alternative to being ordered to jail; and WHEREAS, staff recommends joint powers agreements with the Cities of Apple Valley, Burnsville, Eagan, Farmington, Inver Grove Heights, and Lakeville for the purchase of Sentence to Service; and WHEREAS, staff recommends a joint powers agreement with Independent School District #917 to provide education services to clients at the Juvenile Services Center; and WHEREAS, funding for all these contracts and revenue from the Sentence to Service related joint powers agreements is included in the 2017 County Manager s Recommended Budget. Community Services Director to execute contracts for Community Corrections services for the period of January 1, 2017 through December 31, 2018, subject to approval by the County Attorney s Office as to form as follows: Alternatives in Healing, LLC provides sex offender treatment services for juveniles at the Juvenile Services Center and in the community. Group Therapy: $84.69 per individual for two hour sessions Individual/family therapy sessions: $ per one hour session Case Consultation: $ per hour for up to one hour per week Quarterly Report: $ per hour for report on each youth Discharge Report: $ per discharge report Education and Assessment: $ per hour with individuals and family for the community based treatment program River Ridge LLC was selected through a Request for Proposal process for the delivery of the One-Day Driving While Intoxicated Program. The 10-hour program serves first time Driving While Intoxicated clients and includes a chemical health dependency assessment, lectures, small group discussions, and a victim impact panel. The goals of the program include: reducing recidivism through early education on alcohol use and its effect on drivers, reducing county staff time required to monitor these individuals on probation, and increasing recipient compliance with court ordered conditions. One Day Driving While Intoxicated Program: $ per participant General Security Services Corporation (GSSC) supervises the work crews as part of the Sentence to Service program. 3 November 29, 2016 Page 3 ; and BE IT FURTHER RESOLVED, That the Dakota County Board of Commissioners hereby authorizes the Community Services Director to execute joint powers agreements with the Cities of Apple Valley, Burnsville, Eagan, Farmington, Inver Grove Heights, and Lakeville, and with Independent School District 197 for the purchase of Sentence to Service days from Dakota County at a rate of $ per day, for the period of January 1, 2017 through December 31, 2017, subject to approval by the County Attorney s Office as to form; and BE IT FURTHER RESOLVED, That the Dakota County Board of Commissioners hereby authorizes the Community Services Director to execute joint powers agreement with Independent School District 917 for education services at the Juvenile Services Center, for the period of January 1, 2017 through December 31, 2019, subject to approval by the County Attorney s Office as to form; and BE IT FURTHER RESOLVED, That the service rates authorized for the period of January 1, 2017 through December 31, 2018, shall be increased by the inflation rate used in the Dakota County budget process as authorized by the Dakota County Board of Commissioners effective January 1, 2018, as applicable; and contracts, consistent with the amount budgeted, to alter the number and types of clients served, types of services provided, contract amount and the contract term, consistent with County contracting policies, subject to approval by the County Attorney s Office as to form; and BE IT FURTHER RESOLVED, That the contracts shall contain a provision that allows the County to immediately terminate the contracts in the event sufficient funds from county, state, or federal sources are not appropriated at a sufficient level to allow payment of the amounts due. 6.3 Authorization To Execute Contract For Office Support Services Utilizing Supported Employment WHEREAS, the Dakota County Community Services Division issued a Request for Proposal (RFP) on September 19, 2016, for the purchase of office support services to complement its workforce; and WHEREAS, the goal was to contract with a vendor who provided cost-effective office support services for each of the Community Services departments by providing employment opportunities for individuals with disabilities; and WHEREAS, three proposals were received in response to the RFP; and WHEREAS, after a thorough review, staff recommended the contract be awarded to Ally People Solutions (APS); and WHEREAS, By Resolution No (November 17, 2015), the County Board recommended the most recent contract be executed with APS for office support services at Northern Service Center and Western Service Center for the period of January 1, 2016 through December 31, Community Services Director to execute a contract with Ally People Solutions for office support services in an amount not to exceed $400,000 for the period of January 1, 2017 through December 31, 2018, subject to approval by the County Attorney s Office as to form; and contract, within the amount budgeted, to alter the types of services provided, and contract amount, consistent with County contracting policies, subject to approval by the County Attorney s Office as to form; and BE IT FURTHER RESOLVED, That the contract shall contain a provision that allows the County to immediately terminate the contract in the event sufficient funds from county state, or federal sources are not appropriated at a level sufficient to allow payment of the amounts due.
4 November 29, 2016 Page Authorization To Execute Contracts For Interpretation And Translation Services WHEREAS, Dakota County requires interpretation and translation services to meet the needs of residents who have Limited English Proficiency, or who are deaf or hard of hearing; and WHEREAS, a Request for Proposal (RFP) was issued on September 19, 2016, to solicit proposals for the purchase of interpretation and translation services; and WHEREAS, twenty-five proposals were received, and the selection team thoroughly reviewed all proposals to determine which proposals best met the selection criteria; and WHEREAS, criteria included capacity and proven experience providing general, corrections, probation, and medical interpretation; and WHEREAS, vendors are required to hire qualified interpreters who have been screened for cultural competency and tested for language proficiency, enforce a Code of Ethics and Competency Standards, conduct criminal background checks for all interpreters prior to their receiving an assignment in Dakota County, and be Health Insurance Portability and Accountability (HIPAA) compliant; and WHEREAS, staff recommends execution of contracts with Ad Astra Inc., All Hands Interpreting Services Inc., ARCH Language Network, ASL Interpreting Services, Betmar Languages Inc., Fox Translation Services, Itasca Interpretation Services, Keystone Interpreting Solutions, Kim Tong Translation Services Inc., Language Line Solutions, Middle English Interpreting, Minnesota Language Connection Inc., and University Language Center for the period of January 1, 2017 through December 31, Community Services Director to execute contracts for interpretation and translation services to meet the needs of residents who have Limited English Proficiency, or who are deaf or hard of hearing with Ad Astra Inc., All Hands Interpreting Services Inc., ARCH Language Network, ASL Interpreting Services, Betmar Languages Inc., Fox Translation Services, Itasca Interpretation Services, Keystone Interpreting Solutions, Kim Tong Translation Services Inc., Language Line Solutions, Middle English Interpreting, Minnesota Language Connection Inc., and University Language Center for the period of January 1, 2017 through December 31, 2020, subject to approval by the County Attorney s Office as to form; and BE IT FURTHER RESOLVED, That these contracts shall contain a provision that allows the County to immediately terminate the contracts in the event sufficient funds from county, state, or federal sources are not appropriated at a level sufficient to allow payment of the amounts due; and contracts, within the amount budgeted, to alter the types and amount of services provided, and contract term, consistent with County contracting policies, subject to approval by the County Attorney s Office as to form. 6.5 Authorization To Execute Contract With Legal Assistance Of Dakota County, Ltd. WHEREAS, the provision of legal assistance services are for the benefit of Dakota County residents; and WHEREAS, the County is not mandated to provide these services; however, funding of these services helps prevent the need for more costly services and/or improves access to existing services; and WHEREAS, by Resolution No (November 17, 2015), the County Board authorized the execution of a contract with Legal Assistance of Dakota County, Ltd. (LADC), for the period of January 1, 2016 through December 31, 2016, for the provision of legal assistance services; and WHEREAS, staff recommends executing a contract with LADC in the amount of $65,273 for an additional year for
5 November 29, 2016 Page 5 the period of January 1, 2017 through December 31, 2017; and WHEREAS, the outcomes are included in the terms of the contract and will be tracked throughout the 2017 contract period. Community Services Director to execute a contract with Legal Assistance of Dakota County, Ltd., in an amount not to exceed $65,273 for the period of January 1, 2017 through December 31, 2017, to be reimbursed at a rate of 12 equal payments of $5, per month, totaling the respective contract amount, subject to approval by the County Attorney s Office as to form; and contract, within the amount budgeted, to alter the number and types of clients served, type of services provided, and the contract term, consistent with County contracting policies, subject to approval by the County Attorney's Office as to form; and BE IT FURTHER RESOLVED, That the contract shall contain a provision that allows the County to immediately terminate it in the event sufficient funds from county, state, or federal sources are not appropriated at a level sufficient to allow payment of the amounts due. 6.6 Authorization To Accept Grant Funds From Minnesota Department Of Employment And Economic Development For Supplemental Nutritional Assistance Program Employment And Training Program, Execute Grant Agreement, And Amend 2016 Budget WHEREAS, on August 4, 2016, Minnesota Department of Employment and Economic Development (DEED) issued a Federal Fiscal Year 2017 (October 1, 2016 to September 30, 2017) Minnesota Supplemental Nutrition Assistance Plan Employment and Training Local Plan Application; and WHEREAS, the local plan was developed and requested $80,000 in program funds and $8,400 in support service funds to serve 84 participants; and WHEREAS, the Evaluation Committee and the Executive Committee of the Dakota-Scott Workforce Investment Board (WDB) reviewed and recommended the plan s submission to DEED; and WHEREAS, Dakota County submitted a SNAP Employment and Training Plan to DEED on August 12, 2016, and the plan and allocation was approved as modified by WDB on October 21, 2016; and WHEREAS, DEED has awarded Dakota County $51,590 to provide SNAP employment and training activities for participants for the period of October 1, 2016 through September 30, Community Services Director to accept grant funds from the Minnesota Department of Employment and Economic Development for Supplemental Nutritional Assistance Program Employment and Training Program, and execute a grant agreement in the amount of $51,590 for the period of October 1, 2016 through September 30, 2017, subject to approval by the County Attorney s Office as to form; and BE IT FURTHER RESOLVED, That the 2016 Employment and Economic Assistance budget is hereby amended as follows: Revenue DEED $51,590 Use of FB (Admin) (7,410) Total Revenue $44,180
6 November 29, 2016 Page 6 Expense DWP Program Exp $41,988 DWP Staff Exp (41,988) SNAP Program Exp 2,192 SNAP Staff Exp 41,988 Total Expense $44, Authorization To Execute Amendment To Cooperative Agreement With Anoka, Chisago, Dakota, Hennepin, Isanti, Ramsey, Sherburne, Stearns, Washington, And Wright Counties For Nonemergency Medical Access Transportation Services WHEREAS, in July 2004, the Department of Human Services (DHS) implemented the Minnesota Nonemergency Transportation (MNET) program to improve services and control costs; and WHEREAS, under MNET, DHS contracted with Medical Transportation Management, Inc. (MTM), a medical transportation management company (broker), to determine the appropriate level of transportation service for eligible publicly funded health care program enrollees statewide and make access transportation services (ATS) arrangements for fee-for-service enrollees in the eleven-county metropolitan area; and WHEREAS, in 2009, there was a cost shift from the state to the counties for nonemergency medical ATS, and 2009 Legislative action transferred the administration and coordination of ATS from the state to counties in the eleven county metropolitan area effective July 1, 2009; and WHEREAS, Anoka County executed a master contract with MTM that allowed Anoka and the other metropolitan area counties continued use of MTM for ATS transportation services for the period of July 1, 2009 through June 30, 2010; and WHEREAS, by Resolution Nos (June 22, 2010), (October 15, 2013), and (October 20, 2015), the County Board authorized a Cooperative Agreement between Hennepin County and the other metropolitan counties choosing to participate in this arrangement that allowed Hennepin County to be reimbursed for the administrative services the broker provides for a period covering July 1, 2010 through December 31, 2016; and WHEREAS, the selected medical transportation management company billed Hennepin County for the administrative costs and then Hennepin billed each participating county for that county s administrative costs based on the number of rides for recipients from the respective county; and WHEREAS, a Cooperative Agreement between Anoka, Chisago, Dakota, Hennepin, Isanti, Ramsey, Sherburne, Stearns, Washington and Wright Counties is required in order for Dakota County to reimburse Hennepin County for Dakota County s share of the administrative costs. Community Services Director to execute the Amendment to the Cooperative Agreement with Anoka, Chisago, Dakota, Hennepin, Isanti, Ramsey, Sherburne, Stearns, Washington, and Wright Counties for the provision of nonemergency medical access transportation services for eligible Medical Assistance and MinnesotaCare recipients effective upon approval by all counties on the date of approval by the last county, but no later than January 1, 2017, and shall continue through December 31, 2020, unless extended by some or all of the Metropolitan Counties, subject to approval by the County Attorney's Office as to form; and agreement within the amount budgeted, to alter the number and types of clients served, types of services provided, service expectations and rates, and the contract term, consistent with County contracting policies, subject to approval by the County Attorney's Office as to form. 6.8 Authorization To Execute Contract For Transportation Services With Medical Transportation Management, Inc. For Minnesota Family Investment Program Participants
7 November 29, 2016 Page 7 WHEREAS, Dakota County has provided the Minnesota Family Investment Program (MFIP) Transportation services since 2005; and WHEREAS, the program allows MFIP participants with significant barriers to transportation to access rides to preemployment services, such as meetings with employment counselors, English language training, and job search activities; and WHEREAS, Medical Transportation Management, Inc. (MTM) continues to be the provider of MFIP transportation services as a result of a 2016 Request for Proposal (RFP) process; and WHEREAS, on November 18, 2016, the Dakota-Scott Workforce Development Board recommended executing a contract with MTM, Inc., in an amount not to exceed $60,000 for the period of January 1, 2017 through December 31, Community Services Director to execute a contract with Medical Transportation Management, Inc., for transportation for Minnesota Family Investment Program participants, at a cost per trip of no more than $21.58, in an amount not to exceed $60,000 for the period of January 1, 2017 through December 31, 2017, subject to approval by the County Attorney s Office as to form; and contract, within the amount budgeted, to alter the number and types of clients served, type of services provided, contract amount, and the contract term, consistent with County contracting policies, subject to approval by the County Attorney s Office as to form; and BE IT FURTHER RESOLVED, That the contract shall contain a provision that allows the County to immediately terminate the contract in the event sufficient funds from county, state, or federal sources are not appropriated at a level sufficient to allow payment of the amounts due. 6.9 Authorization To Execute Contract With Resource, Inc., For Minnesota Family Investment Program Employment Services WHEREAS, Dakota County implements Minnesota Family Investment Programs (MFIP) to eligible Dakota County residents; and WHEREAS, a competitive Request for Proposals (RFP) process was established and followed using Dakota County guidelines to seek providers of MFIP Employment Services; and WHEREAS, Dakota County s work participation rate was 50.6 percent, annualized from the period of July 1, 2015 through June 30, 2016, exceeding the 50 percent threshold, and WHEREAS, the Dakota-Scott Workforce Development Board (WDB) approved the Executive Committee s recommendation of RESOURCE, Inc., to provide MFIP Employment Services in 2017 at its November 18, 2016, meeting; and WHEREAS, staff recommends executing a contract with RESOURCE, Inc., doing business as Employment Action Center, in an amount not to exceed $908,244 for the period of January 1, 2017 through December 31, Community Services Director to execute a contract with RESOURCE, Inc., doing business as Employment Action Center, to provide employment and training case management services and job club services for Minnesota Family Investment Programs participants, in an amount not to exceed $908,244 for the period of January 1, 2017
8 November 29, 2016 Page 8 through December 31, 2017, subject to approval by the County Attorney s Office as to form; and BE IT FURTHER RESOLVED, That the Community Services Director, is hereby authorized to amend said contract, consistent with the Minnesota Family Investment Programs Employment Services grants awarded by the Minnesota Department of Human Services and the Minnesota Department of Employment and Economic Development, to alter the number and types of clients served, types of services provided, contract amount, reporting requirements, and contract term, consistent with County contracting policies, subject to approval by the County Attorney s Office as to form; and BE IT FURTHER RESOLVED, That this contract shall contain a provision that allows the County to immediately terminate the contract in the event sufficient funds from county, state, or federal sources are not appropriated at a level sufficient to allow payment of the amounts due Authorization To Execute Contract With YMCA Of The Greater Twin Cities For Drop-In Child Care Services WHEREAS, the Community Services Division has provided drop-in child care services at the Northern Service Center for many years to assist Dakota County clients and citizens accessing services; and WHEREAS, by Resolution No (November 17, 2015), the County Board authorized execution of a contract with YMCA of Greater St. Paul for the period of January 1, 2016 through December 31, 2016, for the provision of drop-in child care services; and WHEREAS, YMCA staff must observe approved policies and procedures in accepting and releasing children from care, explaining policies and procedures to parents or guardians regarding use of the services, and caring for children in a safe, developmentally appropriate manner that provides a positive experience for children while at the Center. Community Services Director to execute a contract with the YMCA of Greater St. Paul in an amount not to exceed $72,893 for the period of January 1, 2017 through December 31, 2017, to be reimbursed at a rate of $6,074 per month, totaling the respective contract amount, subject to approval by the County Attorney s Office as to form; and contract, within the amount budgeted, to alter the number and types of clients served, type of services provided, and the contract term, consistent with County contracting policies, subject to approval by the County Attorney s Office as to form; and BE IT FURTHER RESOLVED, That the contract shall contain a provision that allows the County to immediately terminate the contract in the event sufficient funds from county, state, or federal sources are not appropriated at a level sufficient to allow payment of the amount due Authorization To Execute License Agreement With Independent School District 197 For Space For Emergency Public Health Activities WHEREAS, the Dakota County Board of Commissioners, acting as the Dakota County Community Health Board, has responsibility to support efforts to prevent disease and disability, and to promote and protect the health and safety of County residents; and WHEREAS, in the event of a public health emergency, the public health department is responsible for establishing Points of Dispensing (PODs) where county residents pick up medications; and WHEREAS, Independent School Districts (ISD) 191, 192, 194, and 197 have space that could be utilized by
9 November 29, 2016 Page 9 public health to set up a POD in the event of a public health emergency and training events; and WHEREAS, by Resolution No (January 22, 2013), the County Board authorized the Community Services Director to execute a license agreement with ISD 197 for use of space for public health emergency activities. Community Services Director to execute a renewal of the license agreement with Independent School District 197 for use of space for public health emergency activities for a term from the date of execution through December 31, 2019, substantially as presented to the Community Services Committee of the Whole on November 29, 2016, subject to approval by the County Attorney s Office as to form Authorization To Execute Contracts For Volunteer Driver Transportation Services WHEREAS, Dakota County community agencies fill a transportation gap by providing rides through the use of volunteer drivers; and WHEREAS, Dakota County has limited transportation options for County clients and the general public who do not have a car or other appropriate and available transportation options; and WHEREAS, the majority of volunteer driver rides are for medical/dental appointments for clients with the remaining rides taking clients to mandated or court-ordered activities, such as child support hearings or visitation appointments; and WHEREAS, active engagement in 2016 has occurred through the Dakota County Transportation Coordinating Collaborative (DCTCC) to identify opportunities and efficiencies in the delivery of volunteer driver transportation services; and WHEREAS, County staff will continue to work with the vendors, the Office of Planning and Analysis, and other county and community entities on County-wide outcomes; and WHEREAS, funding for the volunteer driver contracts is included in the 2017 County Manager s Recommended Budget. Community Services Director to execute contracts with the following vendors at the indicated per year not to exceed amounts: GAPP Services, Inc. $173,512, Hastings Family Services, Inc. $23,255, and Neighbors, Inc. $23,255, for the period of January 1, 2017 through December 31, 2018, subject to approval by the County Attorney s Office as to form: The rate paid for administrative/operating costs will be $.44/mile plus the rate paid for mileage at the prevailing Internal Revenue Service (IRS) mileage rate for rides provided during normal business hours. The rate paid for administrative/operating costs will be $.54/mile plus the rate paid for mileage at the prevailing IRS mileage rate for rides provided after normal business hours and on weekends. The wait time for drivers for appointments longer than one hour in length will be paid at $14.00/hour, and one cancellation per client with less than 24 hours notice will be paid at $2.00 per round trip for Social Services rides. One no-show per client with less than 5 hours notice will be paid at $.44/mile plus the rate paid for mileage at the prevailing IRS mileage rate for Community Corrections rides. ; and BE IT FURTHER RESOLVED, That the service rates authorized for the period of January 1, 2017 through December 31, 2018, shall be increased by the inflation rate used in the Dakota County budget process as authorized by the Dakota County Board of Commissioners effective January 1, 2018, as applicable; and BE IT FURTHER RESOLVED, That these contracts shall contain a provision that allows the County to immediately terminate the contracts in the event sufficient funds from county, state, or federal sources are not
10 November 29, 2016 Page 10 appropriated at a level sufficient to allow payment of the amounts due; and BE IT FURTHER RESOLVED, That the Community Services Director is hereby authorized to amend the contracts, within the amount budgeted in the Social Services and Community Corrections budgets, to alter the types and amount of services provided, and contract term, consistent with County contracting policies, subject to approval by the County Attorney s Office as to form Authorization To Execute Joint Powers Agreement With Ramsey County For Overnight Telephone Crisis Services WHEREAS, Dakota County and Ramsey County have collaborated for many years to create an integrated mental health crisis service delivery system in the East Metro, resulting in many system improvements and measurable results; and WHEREAS, Ramsey County needs mental health crisis calls answered from its residents during the hours of 12:00 a.m. to 8:00 a.m. 365 days per year; and WHEREAS, the Dakota County Crisis Response Unit (CRU) currently provides comprehensive response to the many needs of clients, families, and community members; and WHEREAS, Dakota County CRU would not have responsibility to provide an on-site crisis assessment service as part of this Agreement; and WHEREAS, revenue from Ramsey County for overnight phone coverage of mental health crisis calls is included in the 2017 County Manager s Recommended Budget; and WHEREAS, there is no anticipated net County cost for the purchase of the liability insurance premium estimated at $16,000 as it has been included in the indirect cost estimates that will be reimbursed by Ramsey County under terms of the Agreement. 11 November 29, 2016 Page 11 On a motion by Commissioner Kathleen Gaylord, seconded by Commissioner Nancy Schouweiler, the following resolution was unanimously recommended to the County Board: WHEREAS, the Community Services Division presented to the Board the Pathways Initiative on December 17, 2015, focusing on the growing poverty rate in Dakota County and potential next steps to create more systematic linkages among Dakota County, schools, city officials, and residents to identify needs and develop strategies towards improved self-sufficiency and an overall reduction in poverty; and WHEREAS, the Community Services Division devoted significant time in 2016 creating community awareness and support for this work, developing an initial measurement framework, and developing policy proposals that support self-sufficiency; and WHEREAS, the Community Services Director proposes to contract with the Humphrey School of Public Affairs, Future Services Institute to assist with Pathways strategy development, capacity building for community engagement, operational execution and evaluation in NOW, THEREFORE BE IT RESOLVED, That the Dakota County Board of Commissioners hereby authorizes the Community Services Director to execute a contract with the Humphrey School of Public Affairs, Future Services Institute to assist with strategy development, capacity building for community engagement, operational execution and evaluation in an amount not to exceed $129,300 for the period of January 1, 2017 through December 31, 2017, subject to approval by the County Attorney s Office as to form; and contract, within the amount budgeted, to alter the types of services provided, and contract amount, consistent with County contracting policies, subject to approval by the County Attorney s Office as to form; and BE IT FURTHER RESOLVED, That the contract shall contain a provision that allows the County to immediately terminate the contract in the event sufficient funds from county, state, or federal sources are not appropriated at a level sufficient to allow payment of the amounts due. Community Services Directors Report Kelly Harder, Division Director, began his report by letting the Board know that some grant opportunities became available since the last Community Services Committee of Whole meeting, which have been submitted and we will come back before the Board to accept grant funds, if awarded. One was for respite for families with children with mental health issues and the other, which aligns with the Adult Detention Alternatives Initiative work plan, would aim to bring Trauma Informed Care practices to our criminal justice system. Next, Kelly informed the Board that discussion has begun to consider adding access to the Administration Center for the WIC clinics as well as the Criminal Justice Center through the Hastings loop. Then, Kelly explained that through the Statewide Health Improvement Program work, we are assisting city planners on their healthy comprehensive plan by providing updated healthy communities reference documents that now reflect the social determinants of health. These materials have been shared with the city planners in Dakota County who have been appreciative of the data and information. The materials are available on the Dakota County website and are for planning purposes as they include examples of model language and best practices. Last, Kelly previewed a presentation developed by the American Public Human Services Association (APHSA) Locals (comprised of Kelly and his peers from about a dozen state supervised, county run governments across the U.S.) - "A New Pathway Forward: Pathways to Prosperity and Well-Being". Thanks to Commissioners Holberg and Schouweiler, there was a ride-along last week with Representatives Matt Dean and Roz Peterson in which he was able to preview this presentation with them. As an aside, for the ride-along, they sat with our Financial Workers as they worked on MNsure/METS. This was an opportunity for them to see what's working and what the challenges are on an experiential basis. Kelly will also be sharing the presentation with Senators Klobuchar and Franken while in Washington, D. C., later this week for a Board meeting with APHSA. The APHSA Locals under
12 November 29, 2016 Page 12 the model of the social determinants of health came up with a plan to collectively stop generational poverty within the current resources and systems. Kelly accepted feedback of the presentation by Commissioner Slavik and echoed by Commissioner Schouweiler that the last slides might fit better at the front so as to add clarity in setting up the conversation.
