Govt.Memo.No.649/Elections.E/ , dated: addressed to all District Election Officers and Collectors in the State (except Hyderabad). *** Attention of all the District Election Officers in the State except Hyderabad and Commissioner, Municipal Corporation of Hyderabad & District Election Officer, Hyderabad is invited to the references cited. 2. They are informed that in the Memo 2 nd and 3rd cited while communicating the programme approved by the Election Commission of India for Preparation of photo electoral rolls for the remaining 281 Assembly Constituencies in Andhra Pradesh in the 22 districts except Hyderabad in conjunction with the issue of Electors Photo Identity Cards, it was requested to take preliminary action for appointment of BLO's/Supervisors and issue of identity cards to the BLO's and Supervisors. It was also requested that as far as possible the BLO be appointed for 2 polling stations to keep the number of BLO s to a minimum as each BLO will be accompanied by a photographer. 3. The following forms Manual are required to be used at house to house enumeration stage between October, 2007 and March, Manual for BLO s/supervisors/electoral Registration Officer. 2. 2 :: 2 :: 5. Record of Enumeration (This is a booklet with 100/200 sheets. Each sheet will be in duplicate in different colours and serially numbered. The Enumerator is required to fill these forms in duplicate, legibly and obtain the signature of the head of the family or the family member who verified the Division-B and gave information in Division-C. The enumerators will handover carbon copy to the Head of the family with the instruction that the receipt shall be preserved for all subsequent queries as well as for getting EPIC.) 6. Form 4 (Appended to the Registration of Electors Rules For Supervisors to be used in door locked cases ) 7. Form ID ECI- IR to only those enumerators who are allocated polling stations which are identified with substantial presence of foreign nationals. 8. Declaration by service personnel Form ID; ECI IR : (To only those enumerators who are allocated polling stations where service personnel reside) copies of the BLO s Manual (500 English and 1500 Telugu) will be supplied to each district by this office. Additional copies if required can be downloaded from the website ceoandhra.nic.in copies of the Software Manual (250 English and 250 Telugu) will also be supplied to each district by this office. The Preliminary list Division A & B has to be generated at District Level from the Software after integrating the rolls. This software will be supplied shortly. In the meanwhile advance action may be taken by the Districts to print copies of the Following forms. 1. Identity Card 2. Division C 3. Record of Enumeration 4. Form-4 (Sl.No. 2 to 6 to print in A4 size only) 5. Form ECI-IR Form ECI-IR Copies of these forms are enclosed. They are also available in the Annexures to the BLO Manual. Adequate quantities of these forms have to be printed and distributed to the enumerators before start of the house to house enumeration on 15 th October, Simultaneously Division A & B will have to be generated from the computer and supplied to BLOs. 3 :: 3 :: 8. The following forms will be required at the Claims and Objections Stage between March, 2008 and April, Form 6- Application for inclusion 2. Form 7 Objection for inclusion 3. Form 8 Application for correction 4. Form 8 A- Application for transposition 5. Form 001A- Application for providing passport size photographs 6. Letter (ECI proforma) for producing photographs for residual voters at the time of draft publication 7. ECI Form 001B to be signed by elector along with two passport size photographs 9. As per the past practice the forms mentioned in Para 8 above will be printed centrally and distributed to the districts in March, All the District Election Officers in the State except Hyderabad are requested to confirm the action taken relating to paras 2 and 7 above. 11.
