If it doesn t work, check that the cords between the units are securely attached and that the cord from the end unit is plugged into the electrical outlet. Make sure the machines are daisy-chained together, then plug the last unit into the electrical outlet in the wall. The Poll Manager must call the Elections Office to get the pass word. Follow the instructions included in the supplies A zero tape will print automatically when the machine is powered on. Question 45 If a voter leaves the area before casting the ballot, poll officials must before another voter can use the unit. Voters will check it as they enter the polling place to make sure they are in the right precinct to vote. Make sure it can be seen if a line forms to vote. Question 70 To prevent a vote from being counted twice. Mark the voters who were sent absentee ballots in the Poll Book and on the Official Precinct Voter Registration List. Ask voters if they received an Absentee Ballot. Have voters witnesses vouch for them. Write the number of provisional ballots on the accounting chart. This information will allow poll workers to determine which ballot to issue to the voter on Election Day. 19

"I voted" stickers, name tags, locks and seals, oath and payroll sheets, precinct road map Un-voted Provisional Ballots, Provisional Ballot Envelope, Provisional Voter Notice, Provisional Ballot Tally Sheet, Affirmation Statements Change of Name/Address From, Voter Registration Application, Challenge Form (Form 10-U), Party Affiliation Change Form (Form 10-W or 10-X), Disabled Voter Aided by Election Officials Form ( 12 C), Declaration of an Elector Unable to Mark the Ballot (Form 228) Signature Poll Book, Voter List, Precinct List, Precinct Street Directory Arrange these materials in an organized manner on the table so you can quickly and easily get to them if a voter needs them. 20

21 Question 74 On Election Day the polls must open. As soon as the poll workers have the polling place set up but not after 7:00 AM. At 6:30 AM; it is the law. At the time designated by your County Board of Elections. After 6:00 AM but before 7:00 AM. All polling places in Ohio open at 6:30 AM and allow voters to cast their ballots. Poll worker teams have a lot to do before the doors open. It is important to arrive on time and work efficiently as possible under the direction of the Presiding Judge. Question 75 can use a "Regular" Ballot. A voter marked Absentee in the Poll Book. A voter who refused to provide ID. A voter whose name is not in the Poll Book. A voter who provides valid ID and whose name and address are in the Poll Book. Voters who do not qualify for a regular ballot must cast a provisional ballot if they choose to vote. Question 76 If a voter does not provide a valid ID, the voter must cast a Ballot. Regular Provisional Absentee Curbside Provisional Voters who do not present a valid ID, must cast a Provisional Ballot. Question 77 If, the voter must use a Provisional Ballot. The voter provides a valid ID. The voter s name and address are listed correctly in the poll book. The voter is registered to vote in Ohio. A notation in the poll book that indicates the voter was sent an absentee ballot. Such a notation means the voter must vote a Provisional Ballot. The other answers are the criteria for issuing a Regular Ballot. In addition, a "Regular" Ballot voter may also have moved within the precinct and complete a "Change of Address Form" to cast a regular ballot. 21

22 Question 78 If you cannot find the voter s name in the Signature Poll Book or the Supplemental Voter List (if applicable), you should first. Check the precinct street list to make sure the voter is in the correct precinct polling location. Call the County Board of Elections to double check. Check the voter s ID. Have the voter complete an Affirmation Statement. If the voter is in the wrong polling location, or precinct, provide directions to the correct polling location. Question 79 must fill out an Affirmation Statement before voting. A voter who cannot provide any valid form of ID. A voter who provides an Ohio Driver s License as ID. A poll worker who arrives after the polls open on Election Day. Any Certified Poll Observer who comes to the polling location. Having the voter sign the Affirmation Statement (also called Form 10-T) is part of the process to issue a Provisional Ballot. Question 80 A voter must meet all of these requirements except to vote a regular ballot. Present a valid ID. Bring an Attorney-in-Fact to the polling location. Be listed with the correct name and address in the Signature Poll Book. Have no special notations next to their name in the Signature Poll Book. The other choices are the requirements to qualify to vote a regular ballot. If the voter cannot meet these requirements he or she is not qualified to vote a regular ballot and must vote a provisional ballot. 22

23 Question 81 is NOT required for curbside voting. An Attorney in Fact The voter is driven to the polling location and is physically unable to get out of the car There must be parking at the curb close to the entrance to the polling location The polling location has a waiver from ADA requirements. This is not a criterion for curbside voting. The criteria for a voter to vote curbside are: The polling location has received a waiver from ADA compliance, The voter is driven to the polling location and is physically unable to get out of the car, and The driver of the vehicle stands in line and informs the poll workers that assistance is needed. Curbside voting is conducted by 2 poll workers, one from each political party. The voter uses an optical scan, or paper, ballot. Question 82 If of the statements below are true, give the voter a provisional ballot. The voter's name is not found in Poll Book; The voter is marked absentee or provisional in the Poll Book or Signature Book (this indicates the voter have been sent an absentee ballot); The voter does not provide proper or valid ID; The voter refuses to provide ID; The voter has changed his or her name. all none any at least two If ANY of these criteria are true, the voter must cast a provisional ballot. Once this determination is made, use the 9-step process to issue a provisional ballot. Question 83 is an INVALID form of I.D. and cannot be used at the polling location. State of Ohio Identification Card College photo ID A work ID Other State s Drivers License It contains the voter s photo, name and current address that matches the Board of Elections records. Question 86 is an invalid form of ID at the polls in OH. A government check Insurance card A government document Bank statement It does not have the voter s current address that matches the Board of Elections records. Other common forms of ID that are not accepted at the polling location for the same reason are: Another state s driver s license and photo identification cards. Social Security Card Birth Certificate College ID Passport Any notice from the County Board of Elections Question 87 is an invalid form of ID at the polls in OH. State of Ohio Driver s License Birth Certificate Pay check or check stub Utility bill This is an INVALID form of ID because it does not show the voter s current address that matches Board of Elections records. 24

25 Question 88 The steps to fill out the Provisional Ballot Envelope does not include. The Certified Poll Observer challenges the voter. The voter fills out the envelope. The poll worker checks it, completes the back and signs it. A second poll worker double-checks the completed envelope. The correct first step in filling out the Provisional Ballot Envelope is for the poll worker to indicate why the voter is casting a Provisional Ballot. And, the observer must submit the paperwork for approval in advance to the County Board of Elections. Credentialed observers may be in the polling place before, during, and after polling hours.
