3 INVITATION TO BID FOR Uniform Supply Service Contract Sealed bids will be received at the City of Blue Springs Public Works Building, office of the Operations Manager, 500 SE Sunnyside School Road, Blue Springs, Missouri, until 2:00 p.11:00 a.m. on Wednesday July 10, 2013Monday, July 9Thursday, July 19, 2018, for a Uniform Supply Service Contract. At said time and place, and promptly thereafter, all bids that have been duly received will be publicly opened and read aloud. The Bid Information package will be available on Monday, July 1, 2013Monday, June 25Thursday, June 28, 2018, and may be picked up at the Public Works/Central Garage Operations Facility, 500 SE Sunnyside School Road. Said information is to be considered minimum requirements for the uniforms and associated services that will be used by the Public Works Operations, Central Garage and Parks Fleet Maintenance Divisions of the City. The City of Blue Springs reserves the right to reject any and all bids, and to waive any informalities or irregularities therein. The City of Blue Springs hereby notifies all bidders that it will affirmatively ensure that in any contract entered into pursuant to the advertisement, minority business enterprises will be afforded full opportunity to submit bids without discrimination, regardless of race, color, or national origin in consideration for any award. No bidder may withdraw his bid within 60 days after the actual date of opening thereof. 4 Uniform Supply Service Contract Scope of Services The City of Blue Springs requests bids for services provided by a professional uniform service to exclusively supply, maintain and launder general purpose uniform-style garments and office floor mat runners per a rental service agreement in accordance with the Specifications and Contract Documents. All garments and associated services will be provided to employees of the City and will be delivered to the addresses listed below: 500 SE Sunnyside School Road -- Public Works Operations 2715 Park Drive -- Parks Maintenance Department add nd other locations for mats? Specifications I) Laborer Uniform: Shall consist of any combination of the options below. Theyears The City reserves the right, if mutually agreed by the contractor, to extend this service agreement for up to five (5) additional years at the same pricing quotedthree (3) renewal terms of two (2) years each. A price increase up to 3% will be allowed at the time of each renewal. The City will determine whether to accept the contract extension agreements thirty (30) to ninety (90) days prior to the completion of the previous term s service agreement. b) The uniform supplier shall provide a 100% cotton denim blue jeans pant, and carpenter pant and bib overall. No exceptions will be accepted. c) The uniform supplier at no additional cost shall provide uniform storage lockers and soiled uniform lockers to the City, if requested. The uniform supplier will determine the quantity of soiled uniform lockers required. d) Shirt color(s) and emblem design(s) shall be determined after the uniform supplier has been selected. The uniform supplier agrees that there will be no quantity restrictions on shirt color. The City agrees to select from colors available from the uniform supplierssupplier s catalog. The t-shirts must be Hi-Visibility Class 3. e) All items included in the rental agreement shall be cleaned and maintained by the uniform supplier. f) All soiled items shall be picked up weekly and returned clean the following week. g) The uniform supplier at no additional cost to the City shall replace any items requiring repair or replacement due to wear or failure. All items requiring repair or replacement shall be returned on the next regular delivery.

6 h) All garments shall be returned in a clean and usable condition. All shirts shall be pressed. i) All items included in the rental agreement shall remain the property of the uniform supplier. j) Uniform supplier will provide clothing to fit all sizes male and female with no special size cost. k) Uniform supplier will provide a count per employee of each items picked up/ and delivered each week.

7 VII) Certificate of Insurance General Liability Automobile Liability Workman s Compensation Minimum $2,000,000 combined single limit for bodily injury and property damage per occurrence $2,000,000 combined single limit Statutory VIII) Guarantee a) Failure to meet the requirements of Section VI - (e, f, g, h) shall result in the City receiving a credit equal to the charge of each non-conforming item. IX) Termination The City reserves the right to terminate this agreement without penalty if the uniform supplier consistently fails to provide quality service required by the agreement. The City agrees to make both verbal and written notification to the uniform supplier of the unresolved issue allowing the uniform supplier an adequate opportunity to resolve all issues. The City agrees to provide the uniform supplier with a thirty (30) day written notice of such action. The City shall not be responsible to purchase any inventory item provided by the uniform supplier in the event of contract termination as described above.

Bidder has given the Operations Manager written notice of all conflicts, errors, ambiguities, or discrepancies that Bidder has discovered in the Specifications and the written resolution thereof by the City is acceptable to Bidder, and the Specifications are generally sufficient to indicate and convey understanding of all terms and conditions for performing and furnishing the Work for which this Bid is submitted. 6. This bid is genuine and not made in the interest of or on behalf of any undisclosed person, firm, or corporation and is not submitted in conformity with any agreement or rules of any group, association, organization, or corporation; Bidder has not directly or indirectly induced or solicited any other Bidder to submit a false or sham Bid; Bidder has not solicited or induced any person, firm, or corporation to refrain from bidding; and Bidder has not sought by collusion to obtain for itself any advantage over another Bidder or over the City.
