1 Call Tracing Call Tracing allows you to initiate an automatic trace of the last call you received. Your telephone company will automatically receive a message containing the phone number where the obscene or abusive call originated if it is from within the Custom Calling II area, plus the time and date of when the abusive call was placed. It is necessary, however, for you to call your telephone company if you wish to investigate further. Please call before the end of the next business day. Dial (225) between 7:30 A.M. to 6:30 P.M. Central Time. To Use: 1. Hang up after receiving the abusive call. 2. Listen for dial tone. 3. Press *57 (1157 for rotary or dial pulse telephones), then listen for the announcement. Notes: It is essential that the Call Tracing action, pressing *57 (or 1157) be done immediately after you hang up the phone following the abusive call. If you delay taking action and receive a subsequent incoming call, Call Tracing will not trace the correct number. In the event that Call Waiting tone is received during a call that is to be traced, tracing will occur on the Call Waiting number and not the current calling number. The telephone company must have two confirmed traces of the same number in order to take further action. Therefore, it is important to activate Call Tracing each time an abusive call is received. When calling EATEL's Business Office, identify yourself as a Customer Calling II Call Tracing customer. You must give EATEL's Business Office the time and date of the abusive call. In the case of emergency, contact your local law enforcement agency. When you use Call Tracing, you will not be given the phone number you are tracing or the person's name. Should you desire to seek legal action, the person's number and name will be provided to law enforcement authorities.

I hereby release East Ascension Telephone Company, Inc. and its employees from any actions or damages, which may occur in connection with their use of the Trap and Trace Service. I further agree to hold East Ascension Telephone Company, Inc. harmless and agree to indemnify East Ascension Telephone Company, Inc. against any expense, loss, cost, damage, claim, action or liability it may incur as a result of the use of its Trap and Trace Service. I fully understand that I may be expected to prosecute the caller upon completion of this investigation. Failure to do so may result in my being responsible for all or part of the Telephone Company s expense incurred for this service. 5 ANNOYANCE CALL LOG If you keep getting annoying calls, keep a record of each call you receive. The information you record might give you valuable clues to the identity of the caller, and could help both the company and the police (if necessary) establish a pattern that could help identify the origin of the calls. If you continue to receive calls and wish to investigate further, this form must be presented to the appropriate Law Enforcement Agency. No person shall: (1) Engage in or institute a telephone call, telephone conversation, or telephone conference, with another person, anonymously or otherwise, and therein use obscene, profane, vulgar, loud, lascivious, or indecent language, or make any suggestion or proposal of an obscene nature or threaten any illegal or immoral act with the intent to coerce, intimidate, or harass another person: (2) Make repeated telephone communications anonymous or otherwise in a manner reasonably expected to annoy, abuse torment, and harass, embarrass or offend another, whether or not conversation ensues; or (3) Make a telephone call and intentionally fail to hang up or disengage the connection; or (4) Knowingly permit any telephone under his control to be used for any purpose prohibited by this section. B. Any offense committed by use of a telephone as set forth in this Section shall be deemed to have been committed as either the place where the telephone call or calls originated or at the place where the telephone call or calls were received. C. Whoever violated the provisions of this Section shall be fined not more than five thousand dollars, or be imprisoned for not more than two years, or both.
