1 Important changes to NHS Jobs application forms Introduction On the 1 August 2017 NHS Jobs will be introducing some changes to all NHS Jobs application forms: 1. Changes to the disability questions in the Monitoring Information section 2. Changes to the Safeguarding section guidance and questions 3. Changes to the additional application form questions relating to Fitness to Practise These changes are being implemented following work undertaken by the team with the Equality Human Rights Commission and NHS Employers to ensure that job seekers clearly understand what information is required and if it is applicable to them. Monitoring Information The disability section of the application form will display the revised wording and the free text box where an applicant was asked to provide details of their disability has been removed. 2 Safeguarding The guidance and questions around the rehabilitation of offenders act has been changed as follows: Current question / guidance You are required to declare all current 'unspent' criminal convictions or cautions (including reprimands and final warnings). You are not required to disclose convictions or cautions which have become 'spent'. During the rehabilitation period you are required to declare current 'unspent' criminal convictions or cautions (including reprimands and final warnings). Please note you are not legally obliged to disclose convictions or cautions which are protected or have become 'spent' under the Exceptions Order. Are you currently bound over or do you have any current UNSPENT convictions, cautions, reprimands or final warnings that have been issued by a Court or Court-Martial in the United Kingdom or in any other country? * Are you currently bound over or do you have any current UNSPENT convictions that have been issued by a Court or Court- Martial in the United Kingdom or in any other country? You should tick NO if any convictions are protected (or filtered out); and/or have become SPENT as defined by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Please refer to further information about protected and spent convictions and cautions in the Application form Help - criminal background section. 27 July NHS Jobs 2017

[It is important to note that we will only consider current fitness to practise investigations and/or proceedings that are relevant to the position you have applied for].
