1 APPLICATION FOR LICENSE FOR RETAIL SALE OF LIQUOR UNDER THE VILLAGE OF RIVERSIDE ALCOHOLIC LIQUOR CONTROL ORDINANCE NEW RENEWAL The undersigned hereby makes application for the issuance of a license to engage in the business of selling alcoholic liquors, as indicated hereafter, for the period commencing, (date) and ending April 30, 20. (Please check the Class for which application is made. Class License Fees payable in advance on or before May 1 of each year.) A non-refundable application fee of $300 is due with this application. CLASS A $1, License to sell and offer for sale at retail in the premises specified, beer and wine in the original container only, and not for consumption on the premises where sold. (Between the hours of 9:00 a.m. to 11:00 p.m. on weekdays and Saturday and 9:00 a.m. to 11:00 p.m. on Sunday). CLASS A-1 $3, License to sell alcoholic liquor in sealed packages but not for consumption on the premises where sold. Holders of the Class A-1 licenses shall be permitted to conduct alcoholic liquor sampling or tasting promotions upon the licensed premises; provided, that no charge shall be made. (Between the hours of 9:00 a.m. to 11:00 p.m. on weekdays and Saturday and 10:00 a.m. to 11:00 p.m. on Sunday). CLASS B Special $10, License to sell and offer for sale at retail in the premises specified, beer and wine for consumption on the premises at the facilities of the Chicago Zoological Society. (Between the hours of 11:00 a.m. to 6:00 p.m. on weekdays and Saturday and 12:00 noon to 6:00 p.m. on Sunday). CLASS BYOB - Bring-your-own-bottle ( BYOB ) license, which shall authorize businesses to allow their patrons to bring beer and wine in closed containers into the premises for their personal consumption while food is being served in said establishment. Beer and wine consumption shall be limited to those hours when food service is available. With seating capacity of 50 or less ($165.00), with seating capacity of 51 to 100 ($330.00), with seating capacity of 101 or more ($475.00). Between the hours of 11:00 a.m. and 11:00 p.m. on weekdays and 12:00 midnight on Saturday and 12:00 noon to 11:00 p.m. on Sunday). CLASS C (Restaurants) License to sell and serve beer and wine for consumption on the premises where sold when provided as part of food service. With seating capacity of 50 or less ($770.00), with seating capacity of 51 to 100 ($1,320.00), with seating capacity of 101 to 150 ($1,900.00). (Between the hours of 11:00 a.m. to 11:00 p.m. on weekdays and 12:00 midnight on Saturday and 12:00 noon to 11:00 p.m. on Sunday). CLASS D (Bowling Alleys) $1, to sell alcoholic liquor. (Between the hours of 11:00 a.m. to 11:00 p.m. on Monday through Thursday, and 11:00 a.m. to 12:00 midnight on 1

2 Friday and Saturday, and 12:00 noon to 11:00 p.m. on Sunday). CLASS E (Restaurants) License to sell and service mixed drinks for consumption on the premises. With seating capacity of 50 or less ($1,050.00), with seating capacity of 51 to 100 ($1,800.00), with seating capacity of 101 to 150 ($2,700.00). (Between the hours of 12:00 noon to 12:00 midnight on Monday through Thursday; between 12:00 noon to 1:00 a.m. on Friday; between 10:00 a.m. and 1:00 a.m. on Saturday; and between 10:00 a.m. and 10:00 p.m. on Sunday). On January 1, the closing hours shall be 2:00 a.m. CLASS P Brew Pub License, which shall authorize the on-site manufacture and storage of beer at a designated premises and the making of sales to importing distributors and distributors, as well as the direct retail sale to nonlicensee consumers for consumption off the designated premises in sealed containers or on the designated premises through a full service restaurant. The number of gallons of beer sold by a brewpub each year for off premises consumption shall not exceed the number of gallons authorized for sale each year by a brewpub in the liquor control act of Class P licensees must have at least one BASSET certified site manager on premises whenever offering alcohol for sale. Every class P licensee must have a valid brewpub license from the state of Illinois. A brewpub may also offer for sale at retail, for use and consumption on the licensed premises, any beer or wine purchased from a licensed distributor or importing distributor. With seating capacity of 50 or less ($385.00), with seating capacity of 51 to 100 ($660.00), with seating capacity of 101 or more ($950.00). Between the hours of 11:00 a.m. and 11:00 p.m. on weekdays and 12:00 midnight on Saturday and 12:00 noon to 11:00 p.m. on Sunday). CLASS Q Microbrewery microbrewery license, which shall authorize the on-site manufacture and storage of craft beer at a designated premises and the making of sales to importing distributors and distributors, as well as the direct retail sale to non-licensee consumers for consumption off-site in sealed containers or on the designated premises in an accessory tap room. The number of gallons of beer sold by a microbrewery each year for off-premises consumption shall not exceed the number of gallons authorized for sale each year by a brewpub as defined in the liquor control act of Class Q licensees must have at least one BASSET certified site manager on premises whenever offering alcohol for sale. Every class Q licensee must have a valid brewpub license from the state of Illinois. With seating capacity of 50 or less ($385.00), with seating capacity of 51 to 100 ($660.00), with seating capacity of 101 or more ($950.00). Between the hours of 11:00 a.m. and 11:00 p.m. on weekdays and 12:00 midnight on Saturday and 12:00 noon to 11:00 p.m. on Sunday). CLASS W-1 (Wine Bar) Wine by the glass for consumption with and without food on the premises or sale of wine by the bottle for consumption on or off the premises. Wine sales shall be limited to those hours when food service is available. Only employees of the licensed establishment shall provide service of such alcoholic liquors. With seating capacity of 50 or less ($385.00), with seating capacity of 51 to 100 ($660.00), with seating capacity of 101 or more ($950.00). Between the hours of 11:00 a.m. and 11:00 p.m. on weekdays and 12:00 midnight on Saturday and 12:00 noon to 11:00 p.m. on Sunday). CLASS X Craft Distillery $385 License shall authorize the on-site production and 2

3 storage of alcoholic liquor beverages, including specifically spirits and wine, in quantities not to exceed the number of gallons authorized for production each year by a craft distillery and limited wine manufacturer in the liquor control act of , for sale by the bottle for off premises consumption or by the glass for consumption in an accessory tasting room or retail outlet. A tasting room or retail outlet allows patrons to taste samples of products manufactured on site, and to purchase products by the glass or bottle for either on site or off premises consumption, and to purchase related sales items. Sales of alcohol manufactured outside the site are prohibited. Class X licensees must have at least one BASSET certified site manager on premises whenever offering tasting samples of alcohol. Every class X licensee must have a valid craft distiller license, and, if applicable, a limited wine manufacturer s license, from the state of Illinois. Sales of food and non-alcoholic beverages are permitted, but the sale or consumption of alcoholic liquor manufactured outside of the facility is prohibited. (Between the hours of 11:00 a.m. to 11:00 p.m. on Monday through Thursday, and 11:00 a.m. to 12:00 midnight on Friday and Saturday, and 12:00 noon to 11:00 p.m. on Sunday). 3
