1 Dear Applicant: Thank you for your interest in the Carbon Lehigh Intermediate Unit. The position for which you are applying for has a one step process. Below you will find the instructions for completing the application packet. Please read the instructions carefully and return the required items to CLIU Human Resources Department, Attn: Recruitment Secretary. Applicants will only be considered for an interview when the below documents are received. NOTE: Clearance instructions, driver s license check instructions and clearance applications are enclosed. You must complete and send in the following documents. 1. Application 2. Driver History Information Form ( Required for Vehicle Drivers, Job Coaches and Mechanics) 3. Letter of Interest 4. Resume (optional, unless otherwise requested) 5. Two Letters of Recommendation: Please note: Letters of Recommendation cannot be more than 1-year old and must contain the date and signature of the author. 6. Official Sealed Transcripts (not copies if applicable) 7. Completed Arrest/Conviction Report and Certification Form (under Act 24 of 2011 and Act 82 of 2012). 8. As part of the application process a Commonwealth of Pennsylvania Sexual Misconduct/Abuse Disclosure Release Form (Pursuant to ACT 168 of 2014) must be submitted. 10 2 of 3 INSTRUCTIONS Pursuant to 24 P.S (c.4) and (j), the Pennsylvania Department of Education developed this standardized form (PDE-6004) to be used by current and prospective employees of public and private schools, intermediate units, and area vocational-technical schools. As required by subsection (c.4) and (j)(2) of 24 P.S , this form shall be completed and submitted by all current and prospective employees of said institutions to provide written reporting of any arrest or conviction for an offense enumerated under 24 P.S (e) and (f.1) and to provide notification of having been named as a perpetrator of a founded report of child abuse within the past five (5) years as defined by the Child Protective Services Law. As required by subsection (j)(4) of 24 P.S , this form also shall be utilized by current and prospective employees to provide written notice within seventy-two (72) hours after a subsequent arrest or conviction for an offense enumerated under 24 P.S (e) or (f.1). In accordance with 24 P.S , employees completing this form are required to submit the form to the administrator or other person responsible for employment decisions in a school entity. (3) An offense SIMILAR IN NATURE to those crimes listed above in clauses (1) and (2) under the laws or former laws of: the United States; or one of its territories or possessions; or another state; or the District of Columbia; or the Commonwealth of Puerto Rico; or a foreign nation; or under a former law of this Commonwealth. A reportable offense enumerated under 24 P.S (f.1) consists of any of the following: (1) An offense graded as a felony offense of the first, second or third degree, other than one of the offenses enumerated under 24 P.S (e), if less than (10) ten years has elapsed from the date of expiration of the sentence for the offense. (2) An offense graded as a misdemeanor of the first degree, other than one of the offenses enumerated under 24 P.S (e), if less than (5) five years has elapsed from the date of expiration of the sentence for the offense. (3) An offense under 75 Pa.C.S. 3802(a), (b), (c) or (d)(relating to driving under influence of alcohol or controlled substance) graded as a misdemeanor of the first degree under 75 Pa.C.S (relating to grading), if the person has been previously convicted of such an offense and less than (3) three years has elapsed from the date of expiration of the sentence for the most recent offense. 12 Instructions for Completing Clearance Applications Under Act 151(PA Child Abuse Clearance), Act 114 (Federal Criminal Clearance) and Act 34 (PA Criminal History Clearance) are required by law to be obtained by everyone who is employed by a School Entity. The Carbon Lehigh Intermediate Unit requires clearances for New Hires to be current within one (1) year of board approval for hire. Directions for obtaining background clearance records are as follows: 1. Child Abuse Clearance a) The Child Abuse History Clearance is to be completed online by going to Please create an account and submit your application online as you should be able to obtain immediate access to the results/status of your clearance. There is a $13.00 fee payable by credit card for this clearance. b) The CLIU does not offer employment to any applicant unless a current Child Abuse Clearance is on file. 2. PA Criminal History Check a) The Criminal History Clearance can be completed online at epatch.state.pa.us/home.jsp (do not place a www in front of the internet address). By submitting your application online you should have immediate access to the results/status of your clearance. There is an $22.00 fee payable by credit card for this clearance. b) The CLIU does not offer employment to any applicant unless a current PA Criminal Check is on file. 3. Federal Criminal History Record a) Fingerprinting is done digitally at a registered fingerprinting site. Registration for this clearance must be done online at prior to going to the fingerprinting site. b) Click on Schedule or Manage Appointment, and then you will have to create a profile and answer questions to register and schedule your appointment. c) There is a $22.60 fee for this clearance. d) Federal Criminal History Record can take 2 weeks or longer for processing, please plan accordingly. Applicants are to submit a copy of the Registration ID. e) The CLIU does not offer employment to any applicant unless a current Federal Criminal History Record is on file. 4. Full Driver License History a) A Full Driver s License History check must be done for Bus Driver, Mechanic and Job Coach Positions. b) Any candidate that does not have a Commercial Driver s License (CDL) and is applying for a Bus Driver, Mechanic, or Job Coach position must complete the Full Driver s License History check online at the second interview, with a CLIU representative. The candidate is responsible for the ten dollar ($10.00) credit card fee that will be incurred.
