THE MEDICAL SUPERINTENDENT, ESIC MODEL HOSPITAL, RAMDARBAR-CHANDIGARH, invites limited E-Tenders under Two-Bid system through e-procurement solution from the interested and eligible agencies/firms/consultants located in Tricity (Chandigarh, Panchkula and Mohali), Greater Mohali and NCR Delhi/ Faridabad/ Gurugram/ Noida for Re-validation of ISO 9001:2015 Certificate of ESIC Model Hospital, Ramdarbar, Phase II, Chandigarh for Three years. ESIC Model Hospital, Ramdarbar, Phase II, Chandigarh is a 70 bedded hospital under the Ministry of Labour and employment. The hospital presently has ISO 9001:2015 certificate which is valid up to ESIC Model Hospital, Ramdarbar, Phase II, Chandigarh invites limited E-Tenders under Two-Bid system through e-procurement solution from the interested and eligible agencies/firms/consultants located in Tricity (Chandigarh, Panchkula and Mohali), Greater Mohali and NCR Delhi/ Faridabad/ Gurugram/ Noida for Re-validation of ISO 9001:2015 Certificate of ESIC Model Hospital, Ramdarbar, Phase II, Chandigarh for Three years w.e.f SCOPE OF WORK 1. Re-validation of ISO 9001:2015 Certificate of ESIC Model Hospital, Ramdarbar, Phase II, Chandigarh for three years duly following all standard documentation and audits as per latest ISO standards. Eligibility Criteria for choosing ISO service provider- (1) Should be registered with QCI (Quality Council of India). (2) Should have completed at least two (2) ISO 9000 projects in either Government sector or PSU sector of which one project should be in Healthcare organization of more than 50 beds. (3) Should not be blacklisted by Central Government/ State Government/ E.S.I.C. or any other autonomous institution and should not have been convicted for any criminal offence (Affidavit as per Annexure II). (4) ITR of last 03 (Three) years filed by Agency/firm/consultants i.e. financial year , and [Assessment year , and ]. (5) PAN Number of Agency/firm/consultants as on ITR. (6) Registration Details, registration with Company s act/shops and Establishment act or any other proof of business. (7) GST Registration, if applicable. (8) Labour Licence and ESI/EPF registration, if applicable.

5 5 II. Terms and Conditions Relating to Submission of Bids: 1. The interested agencies/firms/companies are required to upload duly signed and stamped filled technical bid as per Annexure I and BLANK financial bids in chronological order with scanned copies of all relevant certificates, documents etc. in correct resolution on the E-Tender portal 4. Tender will be evaluated on the basis of documents submitted online. 5. In addition to online submission, bidders are also required to submit hard copy of the documents as described below: a) Sealed Envelope containing EMD of Rs 3,000/-(Rupees Three thousand only) in the form of Demand Draft in favor of ESIC Fund Account No. 1, payable at Chandigarh. OR Documents in support of the exemption of EMD. Validity of this document should not expire during the validity of Tender and during the entire duration of contract if contract awarded after Technical and Financial bid evaluation. b) Undertaking as per Annexure II (On non-judicial stamp paper of appropriate value). c) Advance pre-receipt affixing revenue stamp along with the full particulars of Bank details in order to refund the EMD amount of unsuccessful bidders through ECS. 6 6 8. All entries in the tender form should be legible and filled clearly. If the space for furnishing information is insufficient, a separate sheet duly signed by the authorized signatory may be attached. Authorized Signatory must countersign all cuttings in the Technical bid application. 9. Technical bids will be scrutinized by an evaluation committee as constituted by the Medical Superintendent to check all requisite and relevant documents and their authenticity. The bidders, whose Technical bids are found eligible in all respect will be informed about the date and time of opening of Financial bid. 10. The bidder shall pay bid security (EMD) of Rs 3,000/-(Rupees Three thousand only) by Demand Draft drawn on any Nationalized Bank / Scheduled Bank in favour of ESI Fund Account No. 1, payable at Chandigarh. Bids received without Earnest Money deposit (EMD) shall stand summarily rejected. OR Documents in support of the exemption of EMD. Validity of this document should not expire during the validity of Tender and during the entire duration of contract if contract awarded after Technical and Financial bid evaluation. 11. The bid security (EMD) of successful bidders will be adjusted in performance security of successful bidder. 12. The bid security (EMD) of unsuccessful bidders shall be returned back without accrual of interest. 13. Advance pre-receipt affixing revenue stamp along with the full particulars of Bank details in order to refund the EMD amount of unsuccessful bidders through ECS should be submitted by the bidders. 14. The EMD shall be forfeited if: a) Bidder withdraws his participation after opening of Price bid. b) If at any stage of tender processing, any information given by the bidder is found to be false. c) Successful bidder fails to undertake the work after award of contract. 15. Only those bidders who have successfully qualified the Technical bid will be informed about the date and time of opening of Price bid. 16. As a guarantee towards due performance and compliance of the contract work, the successful bidder has to deposit an amount of Rs 8,000/-(Rupees Eight thousand only) as Security Deposit by way of Demand Draft drawn on any Nationalized Bank / Scheduled Bank in favour of ESI Fund Account No. 1 payable at Chandigarh with acceptance of tender otherwise the contract may be cancelled and EMD will be forfeited. This Security Deposit should be valid for 60 days beyond the date of completion of all contractual obligations of the agency including statutory obligations. The Security Deposit shall be forfeited if: a) Successful bidder fails to comply with any of the terms & conditions of the contract. b) Successful bidder is found to be implicated of indulging in any fraudulent practices or is blacklisted any time during the contract.

7 7 17. The work shall be started by the successful bidder within seven (7) days of acceptance of tender otherwise the contract may be cancelled and EMD will be forfeited. 18. The bid shall be valid and open for acceptance of the Competent Authority for a period of 90 days from the date of publishing of NIT. 19. After evaluation, the work shall be awarded to the bidder fulfilling all the conditions and who has quoted the lowest rate after complying with all the provisions stated / referred to for adherence in the limited E-tender. In case two or more bidders are found to have quoted the same rates, M.S, ESIC MH Chandigarh, shall decide about the bidder to which the offer shall be granted based on the report on the past performance of the bidder, and length of experience etc. Such decision by the Competent Authority shall be final. 20. Medical Superintendent, ESIC MH Chandigarh, reserves the discretionary right to award on the same rate parallel contract to the next eligible bidder, in case, the bidder with lowest rate at any stage after award of contract fails to perform successfully / satisfactorily. 21. Medical Superintendent, ESIC MH Chandigarh reserves the right to accept or reject any or all bids without assigning any reasons. M.S, ESIC MH Chandigarh also reserves the right to reject any bid which is non- responsive or violating any of the conditions/specifications without any liability to any loss whatsoever it may cause to the bidder in the process. 22. Canvassing in any form is strictly prohibited and the bidders who are found canvassing are liable to have their tenders rejected out-rightly. 23. An agreement between Successful Bidder & Medical Superintendent will be entered into on Rs. 100/- Non Judicial Stamp Paper. Stamp value will be paid by the bidder. 24. All the disputes and differences arising out of and in any way touching the concerns of this contract shall be referred to the Sole Arbitrator to be appointed by the ESIC Model Hospital Chandigarh. The Arbitrator so appointed shall be a Government Servant who has dealt with matters not related to this contract and in the course of his duties not expressed on all or any of the matters in dispute. The award of the arbitrator so appointed shall be final and binding on both the parties to this contract subject to the provision of the Arbitration and conciliation Act, 1996 as amended time to time. 25. Any legal dispute will be subject to Chandigarh jurisdiction only. 26. Interpretation of terms and conditions of tender document by the Medical Superintendent ESIC MH Chandigarh will be final and binding to bidder. 27. The agencies/firms/consultants at all times should indemnify ESIC Model Hospital, Chandigarh against all claims, damages or compensation.

8 28. Payment Procedure:- Payment will be released in following phased manner- (1) 70% of total fees after completion of ISO Certificate Re-validation. (2) 15% of total fees after completion of First Surveillance with favourable outcome at the end of second year. (3) 15% of total fees after completion of Second Surveillance with favourable outcome at the end of third year. 8
