State the number of cases you have tried to conclusion in courts of record during the past five years: Of those, in how many were you sole, associate, or chief counsel? 2. What percentage of your appellate appearances in the last five years was in: (a) Federal appellate courts? (b) Court of Appeals of Washington? (c) Washington Supreme Court (d) Other appellate courts? 3. What percentage of your appellate practice in the last five years was: (a) Civil? (b) Criminal? 4. How many appellate cases have you handled through appellate decision during each of the past five years? Of these, in how many did you do the bulk of the written and/or oral work? 5. Describe the more significant appellate matters that you have handled (no more than 10) For each, please give the name and address of opposing counsel, the names of the judges hearing the matter (if the Court of Appeals), and the citation if the case was reported.

2 6. Summarize your appellate experiences before the last five years. If during any prior period your appellate practice was greater than during the last five years, indicate the periods during which this was so, and give for each prior period the same date requested in question If you have had any other experience in the legal profession that you believe enhances your qualifications for service as an appellate judge, briefly describe it here. 8.
