1 TORONTO LICENSING TRIBUNAL: Summary of Decisions M. Calderwood / M. Laverty / R. Quan 1. Belal Mohammed Ahmed (Report No. 6559) Applicant for a Taxicab Driver's Licence (Application No. B645250) Belal Mohammed Ahmed's licence will be issued, effective immediately, subject to the following conditions: (1) Prior to the issuance of the licence, Mr. Ahmed must provide to Municipal Licensing and Standards, at his own expense, an original updated abstract of his criminal record (to be provided within 30 days of the July 14, 2016 Tribunal hearing, failing which the application/licence will be cancelled); (2) Prior to the issuance of the licence, Mr. Ahmed must provide to Municipal Licensing and Standards a certificate confirming the completion of his counselling (to be provided within 30 days of the Tribunal hearing, failing which the application/licence will be cancelled); (3) Immediately upon being issued, the licence will be placed on probation for a period of four (4) years to commence on the date of issuance; (4) Prior to each of the next four (4) renewals of the licence, Mr. Ahmed must provide to Municipal Licensing and Standards, at his own expense, an original updated abstract of his criminal record; (5) During the probationary period, if Mr. Ahmed incurs any new charges and/or convictions under the Toronto Municipal Code or the Criminal Code, he must notify Municipal Licensing and Standards, in writing, within three (3) business days. The notification shall include his ML&S licence number and the ticket number(s). Mr. Ahmed can notify Municipal Licensing and Standards in one of the following ways:
2 (6) During the probationary period, if Municipal Licensing and Standards has concerns with any new charges or convictions, those matters and report No. 6559, and any updating material, shall be brought back before the Tribunal for a full hearing. (7) During the probationary period, Mr. Ahmed shall not operate any Wheel Trans motor vehicle; (8) During the probationary period, Mr. Ahmed shall not have any passengers in the front seat of any taxicab. The Tribunal further ordered that Condition Nos. 7 & 8 be indicated on Mr. Ahmed's photo identification card. 2. Faiz Bhuiyan (Report No. 6594) Holder of Taxicab Driver's Licence No. D ; Drivers' List No The Tribunal adjourned this matter to a date suitable to all parties. 3. Kiflom Haile Ameha (Report No. 6586) Holder of Taxicab Driver's Licence No. D ; Drivers' List No The Tribunal deemed the employment service of Mr. Kiflom Haile Ameha as having been uninterrupted. 4. Gezae Asgedom Wureta (Report No. 6395) Holder of Taxicab Driver's Licence No. D ; Drivers' List No The Tribunal deemed the employment service of Mr. Gezae Asgedom Wureta as having been uninterrupted. 2

Adil Kanan (Report No. 6575) Applicant for a Tow Truck Driver's Licence (Application No. B646299) Adil Kanan's licence will be issued, effective immediately, subject to the following conditions: (1) Immediately upon being issued, the licence will be placed on probation for a period of four (4) years to commence on the date of issuance; (2) During the probationary period and for the first two yearly renewals of the licence, Mr. Kanan must provide the Municipal Licensing and Standards, at his own expense, an original updated abstract of his criminal record every six (6) months; and every year for the third and fourth years; (3) Prior to each of the next four (4) renewals of the licence, Mr. Kanan must provide the Municipal Licensing and Standards, at his own expense, an original updated abstract of his driving record; (4) During the probationary period, if Mr. Kanan incurs any new charges and/or convictions under the Toronto Municipal Code, the Highway Traffic Act or the Criminal Code, he must notify Municipal Licensing and Standards, in writing, within three (3) business days. The notification shall include his ML&S licence number and the ticket number(s). Mr. Kanan can notify Municipal Licensing and Standards in one of the following ways: (5) During the probationary period, if Municipal Licensing and Standards has concerns with any new charges or convictions, those matters and report No. 6575, and any updating material, shall be brought back before the Tribunal for a full hearing. 6. Yorgi E. Georgey (Report No. 6331) o/a Perms & Cuts Unisex Hair Design Applicant for Renewal of Personal Services Settings Licence No. B The Tribunal approved the following jointly proposed resolution: 3

Georgey's personal services settings licence will be renewed, effective immediately, subject to the following conditions: (1) All fees, documents and/or any outstanding requirements must be submitted to the satisfaction of Municipal Licensing and Standards within 30 days of the Tribunal hearing, failing which the application/licence will be cancelled; (2) Immediately upon being renewed, the licence will be placed on probation for a period of one (1) year to commence on the date of issuance; (3) Prior to the next renewal of the licence, Mr. Georgey must provide to Municipal Licensing and Standards, at his own expense, an original updated abstract of his criminal record; (4) During the probationary period, if Mr. Georgey incurs any new charges or convictions under the Toronto Municipal Code or the Criminal Code, he must notify Municipal Licensing and Standards, in writing, within three (3) business days. The notification shall include his ML&S licence number and the ticket number(s). Mr. Georgey can notify Municipal Licensing and Standards in one of the following ways: (5) During the probationary period, if Municipal Licensing and Standards has concerns with any new charges or convictions, those matters and report No. 6331, and any updating material, shall be brought back before the Tribunal for a full hearing. 7. Westview Paving Inc. (Report No. 6600) Antonio Massi, Director/Trades Person Roy Tabangi, Director Francesco Giordano, Managing Director Applicant for a Driveway Paving Contractor Licence (Application No. B645918) The Tribunal approved the following jointly proposed resolution: Westview Paving Inc.'s licence will be issued, effective immediately, subject to the following conditions: 4
5 (1) All fees, documents and/or any outstanding requirements must be submitted to the satisfaction of Municipal Licensing and Standards within 30 days of the Tribunal hearing, failing which the application/licence will be cancelled; (2) Antonio Massi, Roy Tabangi and/or Francesco Giordano must provide to Municipal Licensing and Standards proof of payment of any outstanding fines; (3) Immediately upon being issued, the licence will be placed on probation for a period of one (1) year to commence on the date of issuance; (4) During the probationary period, if Westview Paving Inc., Antonio Massi, Roy Tabangi and/or Francesco Giordano incur any new charges and/or convictions under the Toronto Municipal Code, they must notify Municipal Licensing and Standards, in writing, within three (3) business days. The notification shall include their ML&S licence number and the ticket number(s).
