Marg, New Delhi 02 (Academic Branch) Instruction For Admission in UG (MBBS) - AIQ / DUQ Students (Admission Process will be done in Room No.- 114) 1. Submit Photocopy of NEET (UG)-2014 Admit Card, Rank Letter and Provisional Seat Allotment Letter with proforma Proforma For Admission to 1 st Year MBBS Course duly filled by the student downloaded from the college website ( and submit to the branch. 2. Take the Original Copy of proforma duly signed by Dean. 3. Then go to Delhi University with original documents and proforma for completion of admission formalities and come back to college. Common Instruction For AIQ/DUQ Candidates 1. Submit Allotment letter issued by the Faculty of Medical Sciences, University of Delhi 2. After verifying name of the candidate a file will be issued to the candidate 3. Deposit Rs. 4445/- (Rupees Four Thousand Four Hundred Forty Five only) through Challan in Dean s A/C at Syndicate Bank (situated in MAMC Campus opposite Central Library). 4. Candidate has to fill all the details in Block Capital Letters in the file and handed over to the branch with following documents:- a. NEET (UG)-2014 Examination Admit Card b. NEET (UG)-2014 Rank Letter c. Matriculation or equivalent mark-sheet and certificate showing date of birth. d. Certificate of having passed 12 th Class Examination along with a statement of marks. e. Proof of deposit of fee i.e., 4445/- f. The Character Certificate should cover the period of not more than 6 months from the date of admission. The Character Certificate issued by School or gazetted officer. g. Schedule Caste / Schedule Tribe certificate, if applicable (Proforma Attached- APPENDIX-I) h. Father s SC/ST Certificate, if applicable. i. OBC certificate, if applicable. (Proforma Attached- APPENDIX-II)

Physically Handicapped Certificate issued by any of the 4 (Four) Disability Assessment Board constituted by Min. H&FW, Govt. Of India. Affidavit on a stamp paper of Rs. 10/- in case there is a gap of six months or more between school leaving and joining of MBBC course, if applicable. n. Three (03) passport size color photographs o. 5 BOND FOR STUDENTS SEEKING ADMISSION TO MEDIACAL COLLEGE KNOW ALL MEN BY THESE PRESENT WE Son / Daughter of Sh. here in after called the students (In case he/she has completed 18 years of age) and Father of Natural guardian of the students (In case he/she is a miner ) which expression shall unless excluded by or repugnant to the context include his/her heirs administrators and assigns and son/daughter of Sh. hereinafter called the surety (which expression shall unless excluded by repugnant to the context included his/her heirs administrators and assigns bind ourselves jointly and severally to pay the President of India/Governor of (hereinafter called the Government ) on demand and without demur a sum of Rs. 5,000 (Rupees Five Thousand only) or if payment is to be made in a country other than that India, the equivalent of the said sum in the currency of that country converted at the official rate fo exchange between that country and India. Dated this Day of One thousand nine hundred and WHEREAS the Govt. has decided to prescribe a condition for admission to medical college in the State/Union Territories of for the MBBS degree course (hereinafter to the said Course that every student so admitted shall complete the said course form the college to which he/she is admitted form any other Medical college in and part course form the college to which he/she is admitted or from any other medical collage in and part fo the country and that on such completion he/she shall if so required by Government to serve in and part of the country in rural areas for a minimum period of two years on such remuneration as may he prescribed therefore and shall furnish a suitable bond for the due performance of the said conditions. AND WHEREAS the students who had applied for admission for the said Course in Medical College (hereinafter referred to as said College ) has been admitted for said course. NOW THE CONDITON OF THE ABOVE WRITTEN obligation is that is the student shall:- 1) Diligently prosecute and complete the said course at the said college or and other medical college in the country and passed the prescribed university examination for the said course and; 2) Within a period of 30 days from the date of his/her completing the Period of internship or rotating houseman ship give to the Govt. by registered post notice in writing intimation the completion of his/her period of internship of houseman ship provided however, the if the student is desirous of continuing postgraduate studies, such notice expressly specify such student is desirous of continuing postgraduate studies, such notice expressly specify such desire and request for permission of the Government to continue such post graduate studies and it shall be open to the Government in its absolute discretion to refuse such permission 1

6 or to grant it subject to the condition that student shall within one month from the completion for post graduate course give a like notice to the government and; 3) When called upon by the Government at any time with in a period of six months form the student as aforesaid join the central Health Service/State Medical Service and serve in rural area anywhere in State for a minimum period of two year such remunerations may be prescribed before and in the event fo the student being not so called upon by the Government in the rural areas or the C.G.H.S or S.M.S for the aggregate period of two years and during the period during which the student is required to serve under the provision of his/her bond faithfully discharge the duties assigned to him/her by the Govt. or his/her other superior with utmost diligence and efficiency and shall observe the rules for time being in force regulating the conduct of persons so serving. THEN THE ABOVE WRITTENBOND shall become void otherwise the same shall remain in full force and virtu: AND in the event of student committing a breach of any of the above terms and conditions the whole of the amount of Rs.5000/-(Rupees Five Thousand only) or such lesser sum as the Govt. May in its absolute discretion decide, shall become payable jointly or severally the student or the Father/Natural Guardian of the student in case of a minor students and the above Surety (Full Name of the surety) Government may without prejudice to any other rights and remedies of the Govt. recover the same form the student or form the Father/Natural Guardian in case of minor students and above surety (Full Name of the surety) AND upon making of such payment the written obligation shall be void and fo no effect, otherwise it shall be and remain in full force and virtue: PROVIDED always and it is hereby agreed and declared that the decision of the Government is to whether the said student has not performed and conceived any of the obligations and conditions hereinbefore recited and the amount compensation payable in this behalf shall be final and binding on the parties hereto. Provided further that the liability of the surety hereunder shall be impaired or discharged by reasons of time being granted or any forbearance, act or commission of the Govt. (with or without the knowledge or consent of the surety) in respect of or in relation to the several obligations and conditions to be performed of discharged by the student not shall it be necessary for the Govt. to see the student or father/natural of a minor students before Suring the surety for the amount due hereunder and provide further that his bond shall all in respect be governed by the laws of India. 2

7 The expanses of stamp duty of the bond, if any shall be borne by the Govt. In witness thereof the said student or father/natural guardian of the student in case he is a minor and the said surety have put their respective hands the day and the year hereinabove written.
