Name on Customer's Credit Card: Customer's Credit Card No.: Expiration: CVV: Terms and Conditions This Master Lead Purchase Agreement ("Master Agreement") is entered into between Leads2Success, LLC, a North Carolina limited liability company, ("L2S") and the above-named customer ("Customer"). L25 and Customer shall sometimes be collectively referred to as the "Parties," or singularly as a "Party." This Master Agreement provides the general terms and conditions applicable to each and every purchase of Leads by Customer from L25 and Customer, however, the specifics of each such request for the purchase of Leads by the Customer shall be found in the lead order form completed by the Customer at the time of purchase (the "Lead Order Form"). To the extent of any inconsistency or discrepancy between the terms and conditions of this Master Agreement and the Lead Order Form, the Parties specifically intend that this Master Agreement shall control. Customer's execution of this Master Agreement herein below shall constitute acceptance of each and every one of its terms and conditions set forth herein, notwithstanding anything to the contrary in the Lead Order Form or any other document, communication between the Parties, writing and/or instrument whatsoever. 1. Leads and Term I Termination: L25 agrees to sell to Customer and Customer agrees to purchase from L25 certain sales leads (the "Leads") subject to all terms, conditions and provisions set forth herein and as requested by Customer on a Lead Order Form. This Master Agreement shall be effective between the Parties as of the date this Master Agreement is executed by the Customer and shall terminate immediately upon the sending of notice of termination by L25 to the Customer at the Primary address set forth above, or, if termination given by Customer, ten (10) days following the sending of notice of termination by Customer to info2@leads2success.com (the "Term"). Notwithstanding any termination of this Master Agreement given by either Party, any provisions hereof to be performed or enforced following termination hereof by either Party shall survive any such termination of this Master Agreement.

This Master Agreement and any Lead Order Form may be amended only by.written agreement executed by all of the Parties hereto, and no provision of this Master Agreement or any Lead Order Form may be waived except by written agreement executed by the Party waiving the provision, right or obligation. This Master Agreement and/or corresponding Lead Order Form, or any part thereof, shall be enforceable as an original. This Master Agreement may be executed and enforced in counterparts, each of which shall be deemed an original, but all of which together shall constitute one and the same instrument. Upon execution of this Master Agreement by the Customer, the same must be transmitted to L2S only by facsimile to (336) Section Headings: Section headings contained in this Master Agreement are for purposes of reference only and shall not affect the meaning or interpretation of any provision of this Master Agreement. 14. Waiver of Trial by Jury: THE CUSTOMER HEREBY EXPRESSLY CONSENTS TO THE WAIVER OF THE CUSTOMER'S RIGHT TO TRIAL BY JURY TO THE MAXIMUM AMOUNT PERMISSIBLE BY LAW. By execution of this Moster Agreement, Customer acknowledges and specifically agrees to each and every one of the foregoing provisions and and evidences such agreement by this Moster Agreement below. In the event of default under this Master Agreement by Customer, the undersigned promises to pay the full amount of all obligations, damages and other sums which may be due L2S, together with all costs of collection, including attorney's fees and court costs, whether for primary, appellate or bankruptcy proceedings.
