Silva-Nguyen A straight forward application of the Permit System can do much to minimize the probability of accidents. Instructions given verbally may be misunderstood or wrongly conveyed, and therefore, must be put in writing is such a way that is unmistakably clear. The Safe Work Permit is an AGREEMENT between the Permit Issuer and the Permit Acceptor that documents the conditions, preparations, precautions and limitations which must clearly be understood and signed off prior to work commencing. The Safe Work Permit aids the identification and control of hazards but will not, by itself, make the job safe. Other Safe Work Processes and Procedures such as Mechanical Isolation, Job Safety Training and Hazard Analysis are also necessary components that will aid in ensuring all work is completed in a safe manner. Proper execution of safe work permitting and associated work strengthens compliance with Suncor Safety Values; Safe Work Permitting Page 1 of 23

This means by protecting the safety and health of our team members, the environment, and conducting our operations reliably and efficiently. 2.0 SCOPE This document applies to Suncor Personnel and Contractors who are working within assets owned and operated by Suncor Energy Distribution Terminals. References in this document to Suncor Personnel include directors, officers, employees, contract workers, consultants and agents of Suncor. 3 4.0 PRECAUTIONARY STATEMENTS 4.1 Specific Hazards Typical hazards that may be encountered while executing the work permitting process may include; Ergonomic Hazards Awkward Body Position Walk area not clear / level Excessive force Heavy / Awkward Lifting Working at Heights Injuries resulting from a fall from an elevation Elevated work platform hazards Overhead work being done by others Work Environment Hazards Exposure to electrical hazards Exposure to Hydrocarbons or other chemicals Slips or trips Limited access / egress Lack of Oxygen Exposure to Welding Fumes Personal Limitations Distractions in work area Confusing instructions No / lack of training Fitness for Duty Safe Work Permitting Page 3 of 23

4 4.2 Training Proper training and demonstrated competency are prerequisites to performing this Standard Operating Procedure. Training shall be provided by a competent person and records shall be maintained. 4.3 Personal Protective Equipment Personal protective equipment as outlined in Distribution Standard Operating Procedure, Cat A- 1, SOP #10 or as specified on the Safe Work Permit, dependent upon work to be completed as per job scope requirements 5.0 RESPONSIBILITIES 5.1 Distribution Terminal Management Responsible for ensuring full implementation and compliance of this standard operating procedure. Responsible to complete quarterly work permit evaluations. Responsible to ensure all Terminal Employees who are required to issue work permits as part of their normal job function are verified for competency at a frequency not to exceed three years. 5.2 Environmental Health & Safety Responsible for ensuring full implementation and compliance of this standard operating procedure. Responsible to audit terminal compliance to this Standard Operating Procedure. 5.3 Permit Issuer 1. Permit Issuers must be trained and competent in their role. 2. Ensures that where preparation of equipment or area is necessary to do the work, that such preparation is done, is verified by the job site inspection and is clearly identified on the permit. 3. Engages the Permit Acceptor in discussions concerning the scope of work, what equipment / location that the task is taking place, the hazards and potential hazards involved in the task, the required PPE requirements and how the task will be completed Safe Work Permitting Page 4 of 23

5 including the tools and equipment to be used, until both issuer and approver are in clear agreement. 4. Visits the work permit site as outlined in Section 12 with the Permit Acceptor to review the permit requirements prior to the permit being issued. 5. It is the Permit Issuers responsibility to STOP the work if he/she becomes aware of changes in conditions which may affect the safety of the workers. 6. Work with the Permit Acceptor in identifying hazards, precautions or other specific requirements that may be necessary to safely complete the documented job scope. 7. Ensures that appropriate atmospheric testing is completed by a competent person and that the results, time of test are entered on the work permit. 8. Ensures that continuous atmospheric testing is undertaken and recorded where applicable. 9. Ensures that all sections of the permit are completed prior to signing the Permit as Issuer. 10. Periodically inspects the job site during work execution to ensure compliance with the Safe Work Permit requirements. 11. Ensures that work is stopped and the Safe Work Permit is withdrawn when aware of changing conditions which may impact the safety of the job. Ensures that the Safe Work Permit is not renewed and work does not resume until it is safe to do so 12. Ensures that the work is stopped and permit revoked should emergency situations arise, conditions change or the Permit Acceptor has violated any of the permit requirements. 13. Ensures that the permit is not renewed until all is safe to do so. 14. Inspects the worksite for job completion and cleanliness with the Permit Acceptor prior to signing off the Safe Work Permit. 15. Acknowledges the return of the permit and the job completion status by completing the appropriate section of the Safe Work Permit. 16. Ensures distribution of copies as outlined on the permit. 5.4 Permit Acceptor & Workers 1. Maintain compliance with all applicable legislation, regulations, rules and safety requirements of all lawful government authorities. 2. Maintain compliance with generally accepted industry safety standards except where higher standards are specifically required herein. 3. Workers must immediately notify the permit issuer of conditions or changes in conditions not identified or consistent with this permit. 4. It is the Permit Acceptors responsibility to STOP the work if he/she becomes aware of changes in conditions which may affect the safety of the workers. Safe Work Permitting Page 5 of 23

6 5. Comply with all Suncor Energy Rules and Regulations. 6. All Permit Acceptors and workers must have a valid Terminal Orientation. 7. All Permit Acceptors and Workers must adhere to all Terminal requirements, specifically in regard to Spill Reporting and Environmental Control. 8. Engages the Permit Issuer in discussions concerning the scope of work, what equipment / location that the task is taking place, the hazards and potential hazards involved in the task, the required PPE requirements and how the task will be completed including the tools and equipment to be used, until both issuer and approver are in clear agreement. 9. Permit Acceptor assists the Permit Issuer in identifying hazards, precautions or other specific requirements. 10. Visits the work permit site as outlined in Section 12 with the Permit Issuer to review the permit requirements prior to the permit being issued. 11. Permit Acceptor or any Worker must never perform any work or utilize any tool equipment that are not specifically outlined on the permit. 12. Confirms acceptance of and understanding of the permit and its terms by signing as the Permit Acceptor. 13. Permit Acceptor reviews all of the conditions and requirements of the Safe Work Permit to each member of the Work Crew. Each member of the Work Crew initials the Safe Work Permit to signify complete understanding of the requirements. 14. Ensures that if the job is delayed or if all Workers are absent from the job site for a period of time greater than 30 minutes, the Permit Issuer is to be consulted to determine if there are changes workers should be aware of. 15. Ensures all Workers complete Field Level Risk Assessment prior to initiating work. 16. Ensures the work is completed in a safe manner and to avoid injury to people and damage to equipment or the environment. 17. Ensures that all work is stopped and reports back to the Permit Issuer should the permit expire for any reason. In this case, work is not to commence until the permit has been revalidated and signed off indicating that there have been no changes in condition that would warrant issuance of a new permit. 18. Ensures that a Qualified Person accepts the Permit Acceptor s responsibilities for the job and the permit should the current Permit Acceptor leave the job site. Also ensures that the Permit Issuer is made aware of the change in personnel. 19. Inspects the worksite for job completion and cleanliness with the Permit Issuer prior to surrendering and sign off the Safe Work Permit. Safe Work Permitting Page 6 of 23

7 5.5 Gas Tester A Gas/Atmosphere Tester is the individual assigned responsibility to conduct hazardous atmosphere testing and monitoring. In some cases, the gas tester and permit issuer may be one and the same. The Gas/Atmosphere Tester responsibilities are: 1. Conducts the tests specified by the Permit Issuer. Ensures the immediate work area and the surrounding areas have been representatively tested. Should unsafe conditions be encountered, the Gas Tester must immediately advise the Permit Issuer. 2. Competent with all atmospheric testing equipment and procedures. 3. Verifies the integrity and operation of the atmospheric testing equipment prior to use. 4. Ensures the time and readings are recorded on the permit and are clearly communicated to the permit issuer. 6.0 DEFINITIONS Terminal Management; highest level of authority on site, may be manager, supervisor or a delegated responsibility Work is defined as job tasks that the individual is or would be expected to perform as part of their normal duties in his or her normal work area. Example: An employee operating equipment and/or performing routine maintenance. Permit Issuer, is the owner or competent delegate, of the facility / area where the work defined in the Safe Work Permit is to be carried out. The Permit Issuer is trained and qualified to identify all potential hazards associated with the work to be done, to fully complete and sign the Safe Work Permit and to prepare the facility in such a way that it is safe to be worked on by the Permit Acceptor. Permit Acceptor, is the individual completing the work or part of the work crew completing the work. The Permit Acceptor must understand the scope of the work to be completed and work with the Permit Issuer to document this on the permit as well as identify potential hazards, precautions or controls necessary to complete the work safely. The Permit Acceptor is responsible to review the Permit with all workers and to ensure that work stops if permit conditions or the work scope changes. Safe Work Permitting Page 7 of 23

8 Resident Contractor, Individual contractor or contracting company that is employed at a Suncor owned / operated facility in a continuous full time capacity. Hot Work, is generally categorized as any work in which heat is used or generated of such intensity that it is possible to ignite flammable liquids, vapors, gases or combustibles. Confined Space, is an enclosed or partially enclosed space that is not designed or intended for continuous human occupancy with a restricted means of entry or exit which may become hazardous because: of it s design, construction, location or atmosphere of the work activities, materials, or substances in it the provision of first aid, evacuation rescue or other emergency response service is compromised; or of other hazards relating to it Asbestos-Containing Material (ACM), means material that contains 0.5 per cent or more asbestos by dry weight; 7.0 SUSTAINMENT AND CONTINUAL IMPROVEMENT This document is classified as a Critical Safe-Work Practice and as such must comply with the Critical Safe-Work Practice Standard as outlined in Distribution Procedure Program, Cat A-3, SOP #01. Distribution Leadership Team shall initiate document review process. To ensure this Standard Operating Procedure documents meets all applicable OEMS standards, this document is to be reviewed at a frequency not to exceed 3 years or when conditions change to warrant a review Safe Work Permitting Page 8 of 23

9 8.0 PROCEDURAL REQUIREMENTS: 1. No work may be carried out by Contractors without written permission in the form of a Safe Work Permit, with the following exceptions a. Safe Work permits are not required for personnel conducting walk-throughs of terminal areas. However, such personnel must contact permit issuing authority for verbal authorization and sign the area Sign-in Book. Examples of non-hazardous activities that may utilize the sign-in procedure include tours, visual inspections, engineering field checks. b. Janitorial / HVAC work in Buildings. c. Landscaping work outside of Terminal Fenceline d. Rail shunting by rail crews e. Work within the Maintenance Shops by resident contractor f. A Safe Work Permit is not required when Suncor Operating Personnel work on equipment that they operate or are in control of and the work is performed in an area that they control. g. Work performed by Suncor personnel completing Laboratory type work when the work is performed in their normal work location or when procedures are in place. 2. Permit Issuers and Acceptors MUST communicate in an open, reasonable and responsible manner while preparing the Safe Work Permit. BOTH are equally responsible for ensuring the Safe Work Permit is prepared in full and is free of misunderstanding and intent. 3. When hazardous materials are involved, Safety Data Sheets must be utilized as a reference for determining the extent of Personal Protective Equipment required and other precautions. 4. Inspection of the job site and equipment is a critical aspect of safe work and must be done together by both the Permit Issuer and Permit Acceptor prior to the issuance of permits as outlined in Section 12, Job Site Inspection. 5. There will be no difference in work permitting requirements based on who is completing the work. 6. The work permit shall be revoked by the Permit Issuer should conditions change or the Permit Acceptor has violated any of the permit requirements. Safe Work Permitting Page 9 of 23

10 7. In the event of an emergency during the execution of any work permit(s): All work in the Terminal is to be immediately shutdown and the work area rendered safe. All Contractors must immediately report to the muster area. Work must not resume until the Contractor has been notified by Suncor personnel that it is safe to do so. Original work permits must be initialled by both the Permit Issuer and Permit Acceptor provided inspection of the work site confirms that there has been no change in the conditions as set forth on the original permit. In the event of hot work or confined space entry, atmospheric testing MUST be repeated prior to work resuming. 9.0 ALLOCATION OF COPIES 1. Safe Work Permits are issued in triplicate, serially numbered. The original top copy will be displayed at the work space as they are issued to the space not the individual worker. 2. The original top ( white ) copy must be returned and signed off by the permit issuer and permit acceptor at the completion of work or at the end of the shift. 3. The second copy ( yellow ) will be posted in a prominent location in the Terminal so that all Personnel are aware of the work in progress. 4. The third copy ( pink ) is to be attached to the white copy once it has been returned by the worker. Both white and pink copies must be retained for review by Terminal Management. 11 10.0 DURATION OF SAFE WORK PERMIT 1. Duration of the permits are as specified on the Safe Work Permit 2. Permits are declared VOID and all work MUST stop when: a. The Permit Issuer shift changes b. An unsafe condition arises c. When conditions of the job site change sufficiently to create additional hazards to the worker. d. When the scope of work and or tools required to perform the work change. e. Emergency Alarm sounds. f. Workers are absent from the job site for an extended period of time. 3. If a job is to carry on beyond the time specified in Section 4 of the permit, a permit extension may be requested and granted by the completion of Section 13. A permit extension must be completed by the original permit issuer and permit acceptor, any changes in either personnel will require a new permit. 4. If a job is to carry on beyond shift change, a new permit must be issued by the incoming shift. 5. Atmospheric testing is ideally completed immediately prior to the Worker striking the initial arc, when this is not possible, atmospheric testing must be completed within 30 minutes of striking the initial arc TYPES OF WORK Distribution Terminals use a single Safe Work Permit document. Different types of work are listed on the document. The Permit Issuer and Permit Acceptor jointly must ensure the applicable type of work involved in a particular task are checked off and the appropriate levels of requirements are addressed in the body of the permit. In some cases, multiple types of work must be checked off. Safe Work Permitting Page 11 of 23

12 11.01 COLD WORK: i. A cold work permit is issued for work that does not or will not introduce sources of ignition of such intensity that it is possible to ignite flammable liquids, vapors, gases or combustibles at the work site. ii. Cold work is generally categorized as work with hand tools, visual inspections, general site preparation. iii. In some cases, atmospheric testing may be required in conjunction with a cold work permit to prove an area is free of hydrocarbon vapour or gas. Examples of Cold Work include but not limited to the following: Opening a flange Repairing a Pump Seal Blanking and De Blanking Work site preparation Removing a pump or valve Cold cutting a line Opening of Vessels HOT WORK: 1. Hot work is generally categorized as any work in which heat is used or generated of such intensity that it is possible to ignite flammable liquids, vapors, gases or combustibles. 2. In all cases of Hot Work, atmospheric testing is a requirement of the permit. 3. Where Hot Work is to be completed on piping or equipment, the piping or equipment must be isolated and freed of all flammable or combustible materials or vapour. 4. All hot work job sites require at minimum one (1) 20 LB ABC fire extinguisher. Examples of Hot Work include but not limited to the following: Radiography Use of portable electric tools Jackhammering Sandblasting Gas or electric welding Use of a combustion engine Grinding Driving vehicles into Tank Compounds Soldering Torching Use of non-intrinsically safe equipment such as cameras, cell phones, wearable technology. 13 When issuing hot work permits, additional consideration should be given to; Atmospheric Testing When verifying atmospheres to conduct hot work, LEL reading must be <1.0% LEL for hot work to proceed. The gas test is to be performed as close to the work as possible For non confined spaces, atmospheric testing is required; o Within 30 minutes prior to actual start of hot work, ideally completed immediately prior to the introduction of an ignition source. o When conditions change sufficient to warrant another test o At the request of the Performing Authority o When Workers are absent from the job site for a period of time greater than 30 minutes. o After a work stoppage due to an emergency Continuous atmospheric testing is not a requirement for routine hot work outside of a confined space but may be requested by the Performing Authority In the case of a confined space, continuous atmospheric testing is required. Containing and Quenching Sparks When cutting or welding is being performed above grade or over floor openings and these areas cannot be closed, precautions must be taken to ensure complete spark containment to avoid initiating fires. Once hot work is completed, work area should be monitored for at least 30 minutes for any smouldering. Hot Work Barricades If elevated hot work is in progress, the area on the ground must be barricaded to prevent inadvertent personnel access to the area. Barricade tape may be used but must be removed when work stops for the day provided that no other hazard exists (for example, temporarily supported material). Covering Drains and Sewers for Hot Work All drains and sewers within 35 feet of the cutting or welding activity must be covered with a rubber mat. If conditions result in a large amount of vapors, to the point that vapors may escape from under the mat, the hot work permit should not be issued until the vapor condition can be adequately managed. 14 Welding on Storage Tank Floors Before attempting to weld on the bottom of storage tanks, determine whether oil has collected under the bottom. If oil is found, the bottom must be cold cut to remove oil soaked sand. Special means of inerting can be employed in some cases. Compressed Air Lines Welding on air lines can release light hydrocarbons from lubricating oil which can coat the pipe therefore welding should never be attempted on compressed air lines without verifying the lines are gas and oil free. Other Hot Work Precautions Where hot work is in progress, the unit operator must determine if other hazardous work can be permitted in the same area. Examples include: Installing blinds, pulling blinds, un-heading vessels, opening heat exchangers, bleeding lines, and draining pumps VEHICLE ENTRY 1. A vehicle entry permit is required whenever any vehicle or other motorized piece of equipment must enter an area that is normally closed to vehicle traffic such as a tank compounds, process units or controlled roadways. 2. Vehicle Entry Permits are issued to the vehicle and operator, the permit is issued specifically for the purpose of vehicle entry only. Any additional work must be covered under a separate work permit. However, in cases where the vehicle or equipment is required to assist a crew to do a job, a separate permit for the operator is not needed and Vehicle Entry can be specified on the Safe Work Permit issued to the crew. 3. Prior to a vehicle entering a restricted area, a gas test for flammable or explosive atmospheres MUST be performed by a qualified person and results must be entered on the Safe Work Permit. Gas testing is to be done along the path of entry and exit. 4. Vehicle Entry Permits are issued to the vehicle or equipment operator. to deliver chemicals or material into a tank compound to remove or return equipment from a tank compound use of a crane to remove / install mixers / manways Safe Work Permitting Page 14 of 23

15 11.04 EXCAVATION 1. An Excavation Permit is required when disturbance of the ground over 30 cm is necessary. This may include the breaking of concrete, hole boring, stake driving, foundation installation. 2. All Excavation work must be completed in accordance with the Distribution Standard Operating Procedure for Ground Disturbance, Cat A-1, SOP # CONFINED SPACE ENTRY 1. A Confined Space Entry Permit is required whenever anyone must enter a confined space. A Confined Space is an enclosed or partially enclosed space that is not designed or intended for continuous human occupancy with a restricted means of entry or exit which may become hazardous because: of its design, construction, location or atmosphere of the work activities, materials, or substances in or nearby the confined space. the provision of first aid, evacuation rescue or other emergency response service is compromised; or of other hazards relating to it 2. All Confined Space Entry work must be completed in accordance with the Distribution Standard Operating Procedure for Confined Space Entry, Cat A-1, SOP #01. Some examples of confined spaces include but are not limited to the following: Above Ground Cone Roof Storage Tanks Excavations / Open Trenches / Ditches Manholes Underneath Tank Floating Roof Sewers Pipeline Tunnels Process equipment, drums, towers, Internals of railcars / tank trucks Above Tank Floating Roof Oil Water Separators Safe Work Permitting Page 15 of 23

16 11.06 ENERGY ISOLATION In addition to work permits, when work scope includes electrical or mechanical isolation, further documentation is necessary: Electrical Isolation i. An Electrical Isolation permit is required when any electrical device is required to be isolated from the grid. The permit is issued specifically for the purpose of electrical isolation and not for any additional work. ii. iii. iv. In the case of low voltage isolation, ( less than 750V ), a completed electrical isolation form must be included with the Safe Work Permit, copies left at the point of isolation and Terminal Offices. In the case of high voltage isolation ( greater than 750V ), a completed High Voltage Electrical Approval form must be completed by the Contractor and Terminal Management. All Electrical isolation work must be completed in accordance with the Distribution Standard Operating Procedure for; a. Electrical Isolation, Cat A2, SOP #09 b. Working on or near Energized Equipment, Cat A-1, SOP #04 Mechanical Isolation i. A Zero Energy Plan is required in addition to a completed work permit when job scope includes the necessity to isolate all forms of hazardous energy to ensure the safety and health of Suncor Personnel and Contractors where the unexpected start up or release of stored energy could cause injury. ii. All mechanical isolation work must be completed in accordance with the Distribution Standard Operating Procedure, Cat A-1 SOP #03 Mechanical Isolation. Safe Work Permitting Page 16 of 23

17 11.07 REPETITIVE PERMITS Repetitive permits may be utilized for tasks which are repetitive by nature and often of low risk. The intent of a Repetitive Safe Work Permit is to allow work to continue for periods of time that falls under a single work permit. In the case of a repetitive permit, an initial work permit must be issued that details in full the job requirements. This initial permit is used as a basis for repetitive permits to capture daily sign off and atmospheric testing requirements. Repetitive Permit Requirements: The use of a repetitive permit must be approved by the applicable Manager Terminal Operations using the Distribution MOC process. One repetitive permit per Contractor A Field Level Risk Assessment must be completed every time the repetitive permit is utilized. Each permit issuer and acceptor must review and initial original permit. Each permit issuer and acceptor must initial repetitive permit when required to do work. Inspection of the work site is required to confirm that there has been no change in the conditions as set forth on the original permit. New Workers to the job must review and initial the original permit and henceforth, initial the repetitive. Repetitive permits must cease and new permits issued when: Work conditions on the original permit change Change in Work Scope Safe Work Permitting Page 17 of 23

18 12.0 PERMIT COMPLETION STEPS Permit Section 1 ISSUED TO, Name of contractor company to be legibly written. In this section, type of work permit must be indicated, this may be more than one selection. Permits shall be issued to a single Contractor or Contractor company, multiple contracting companies on the same permit is not allowed. In the case of companies sub-contracting the work, the name of the contractor on the permit shall be that of the sub-contractor. Include time the permit was issued, for example, issued to ABC Contracting at XX:XX hours. The COLD box must be checked when there is no possibility of an ignition source, flame or spark producing activity directly associated with the work. Checking the HOT box indicates provisions above and beyond those for cold work, and that work may be possible for a tool or piece of equipment to create enough heat to ignite flammable or combustible materials or vapors. The CONFINED SPACE box must be checked whenever anyone must enter a confined space. A Confined space is an enclosed or partially enclosed space that is not designed or intended for continuous human occupancy with a restricted means of entry or exit which may become hazardous because: 1. Be enclosed or partially enclosed 2. Not be designed nor intended for continuous human occupancy 3. Have limited or restricted means for access / egress that may complicate the provision of first aid, evacuation, rescue or other emergency response. 4. Be large enough and so configured that a worker could enter to perform assigned work. 5. Where the potential for a hazardous atmosphere may exist. The EXCAVATION box must be checked whenever ground disturbance / excavation work in excess of 30 cm is to be performed. Safe Work Permitting Page 18 of 23

19 The VEHICLE ENTRY box must checked whenever any vehicle or other motorized equipment MUST enter an area which is normally closed to vehicle traffic The ENERGY ISOLATION box must be checked when work to be performed involves: o Mechanical Isolation o Electrical Isolation o Working on or near Energized Electrical Equipment Permit Section 2- LOCATION OF WORK, Shall include name of terminal, area within the terminal ( if applicable ) and identification of equipment if available, location and equipment shall be entered as specific as possible. Permit Section 3 DESCRIPTION OF WORK, Insert a DETAILED description of the work that clearly documents the work scope, permit requirements and limits area of where work is permitted as agreed to by both the permit issuer and acceptor. Permit Section 4 DATE ISSUED, Insert date of permit and specify expiry time using 24 hour clock. Fill in expiry time, never beyond normal work day, never beyond Permit Issuers end of shift. Permit Section 5 SPECIFIC HAZARDS, Check off each hazard that exists and precautions taken. List any hazard that is not covered under other. Permit Section 6 WORK PREPARATIONS, All areas of this section must be evaluated to ensure all equipment necessary to minimize potential risk is in place. - box checked indicates this is a requirement and has been carried out - box not checked indicates this is not a requirement Safe Work Permitting Page 19 of 23

20 Permit Section 7 PERMIT JOB SITE INSPECTION For higher risk work, inspection of the permit job site and equipment is a critical aspect of safe work and must be completed jointly by both the permit issuer and permit accepting parties prior to the issuance of a Work Permit. Examples of higher risk work shall include but not limited to the following: Confined Space Entry All Hot Work that may generate open flame or sparks. All Mechanical / Electrical Isolations Excavations over 30 cm in depth. Working at Heights Work involving potential for dropped objects. Permit Section 8 WORKERS MINIMUM SAFETY REQUIREMENTS, This section to be completed in conjunction with Permit Acceptor. Identify all PPE, fire fighting and respiratory requirements. Permit Section 9 OTHER HAZARDS / REQUIREMENTS / DOCUMENTS Free text section where input can be entered not covered in sections 5, 6, or 7. In addition, this section is for the inclusion of other documents, such as confined space entry checklists, rescue plans etc. In this section as well, consideration must be given for the possibility of asbestos containing material, ACM. In Distribution, exposure to ACM rarely occurs, limited mainly to terminals with steam or hot oil boilers and associated insulated piping, however asbestos may be found in gasket material, therefore due caution must be exercised. Permit Section 10 ATMOSPHERIC TESTING, All results of the atmospheric testing are to be entered into Section 10. In all cases, Atmospheric Testing shall be completed by Suncor Employees or Designated Safety Contractors, under no circumstances shall the Permit Acceptor or Workers complete the required Atmospheric Testing unless specified. 21 In the case when atmospheric testing by the Permit Issuer is not practical due to multiple work locations, responsibility for gas testing is to be identified on the permit as being the Permit Acceptor s / Worker's responsibility. Workers must carry a portable gas tester capable of testing for the presence of flammable or explosive atmospheres at all times. All Workers must be trained in the use of the gas tester. Atmospheric testing by Permit Acceptor / Worker may be applied in the following situations only: Snow removal using ploughs, snow-blowers etc. Landscaping activities such as lawn cutting, mechanical weed removal. The use of battery operated tools or instruments which are not approved as intrinsically safe. Taking of pictures using a flash attachment. Permit Section 11 WORKER RESPONSIBILITY Outlines Permit Acceptor and Worker Responsibilities. Permit Section 12 AGREEMENT & SIGNATURE Both Permit Issuer and Acceptor acknowledge agreement of the permit. Permit Acceptor MUST review the details of the work permit with ALL members of the work crew. All members of the Work Crew accept understanding of the agreement by initialling the Permit Acceptors copy of the permit. Permit Section 13 PERMIT SURRENDER, At the end of the day or completion of work, all copies of the permit are surrendered and signed off by both Issuer and Acceptor in the areas provided. Permit Section 14 PERMIT EXTENSION, If required, permit may be extended beyond the expiry time specified in Section #4. Both the original permit acceptor and issuer must sign off on the extension. Permit may not be extended beyond the date of issue. Safe Work Permitting Page 21 of 23

22 13.0 WORK PERMIT EVALUATIONS 1. Overall compliance to the Safe Work Permit System shall be initiated by completing a Safe Work Permit evaluation. The evaluation will determine whether improvement opportunities are required. 2. Permit evaluations shall be completed a quarterly basis. 3. All confined space entry permits shall undergo an evaluation and shall be completed in addition to the quantity of permits identified in (2) WORK PERMIT RETENTION All work permits shall be retained for a minimum period of two years PROCEDURE DEVIATION Deviations from this standard operating procedure must be authorized using the Management of Change procedure.
