Temporary Employment Pass This in-principle approval allows the Employment Pass applicant to commence employment for a period of 1 month from the date of this letter provided he/she has a valid Short Term Visit Pass endorsed in his/her travel document. However, if the Employment Pass is approved for a duration of 3 months or less, he/she must collect it before he/she can commence employment. Short Term Visit Pass upon submission of document at the Work Pass Division The In-Principle Approval letter does not validate the stay of the above applicant in Singapore. Prior to the collection of the Employment Pass, please ensure that the above applicant has a valid Short Term Visit Pass duly endorsed in the travel document. In the case of renewal, please ensure that the existing Employment Pass is still valid. Extension of the existing pass may be requested for from the Work Pass Counters at Ministry of Manpower, 18 Havelock Road, Singapore It is an offence to stay in Singapore without a valid pass. Employment Pass Conditions The Employment Pass holder, SORKIN VIACHESLAV, is to work as RESEARCH ENGINEER with INSTITUTE OF HIGH PERFORMANCE COMPUTING only. He/she is not to engage in any employment other than that specified in the pass as endorsed in the travel document. Should there be a change in his/ her duties or designation, the employer is required to inform the Work Pass Division. If he/she changes employment, he/she must apply for a new pass. 2 The employer has to report in writing, and advise the Employment Pass holder to cancel the Employment Pass in person at the Work Pass Division within 7 days upon termination of employment. Failure to do so may result in overstaying charges. The employer should explain the contents of the in-principle approval letter to SORKIN VIACHESLAV. Any violation of the above conditions is a contravention of the Employment of Foreign Manpower (Work Passes) Regulations and the offender is liable to a fine and/or imprisonment upon conviction. In addition, failure to do any of the above may affect the employer's future applications for Employment and other passes issued by the Work Pass Division. Fees payable for the collection of the pass(es) Payment of fees has to be made upon collection of the pass. Acceptable payment modes are by NETS, Cashcard, cash or cheque only. The amount of fees payable are as follows: S$30 per year or part thereof for a/an Employment/Dependant's Pass S$60 per issue of the Long Term Visit Pass S$30 per issue of a Multiple Journey Visa (if applicable) S$30 per issue of a Single Journey Visa (if applicable) Cancellation To cancel an Employment Pass, the following documents must be produced at the Work Pass Division within seven days from the date of cessation or termination of the employment of the Employment Pass holder : A letter from the employer requesting cancellation of the Employment Pass; The valid travel document and the Disembarkation/Embarkation Card (Green card) of the pass holder; and The IMM 27 (Immigration White Card) duly completed. If the Employment Pass holder has left employment without informing the employer and the travel document cannot be produced for cancellation of the Employment Pass, the employer must also produce a police report. Any Dependant's Pass and/or Long Term Visit Pass tied to an Employment Pass must be produced for cancellation along with the Employment Pass. Failure to do so will result in the Dependant's Pass and/or Long Term Visit Pass holder becoming an overstayer. The employer may come personally or give written authorization to a representative to submit the documents needed to process the Pass, within 2 weeks of the applicant's arrival in Singapore. To collect the pass, the employer or authorized representative will need to get a queue number from the Queue Ticket Counter machine, Level 1, Ministry of Manpower (MOM), 18 Havelock Road, Singapore Queue tickets are issued from 8am to 4pm on weekdays and from 8am to 12pm on Saturdays. The busy periods are from 11am to 3pm on weekdays and 10am to 12pm on Saturdays. The pass(es) can be collected about three hours after the submission of the documents over the counter. They can be collected on the same day where documents are submitted before 2.30pm (Mondays to Fridays) and 10am (Saturday). (2 of 6)12/5/2008 9:43:32 AM

3 The following documents must be produced at the point of collection. For non-english certificates/documents submitted, copy(ies) of the official English translation must also be submitted. MOM reserves the right to request the submission of any other documents including original copy(ies) of documents that have already been submitted, after the collection of the pass: This In-Principle Approval letter (original); The applicant's original passport/travel document: For collection of S Pass, the S Pass applicant's original passport/travel document must be valid for at least 7 months; (Where the validity is less than 7 months, please note the expiry of this S Pass will be issued up to 1 month before the expiry date of the applicant's passport/travel document). For collection of other passes except S Pass, the applicant's original passport/travel document must have at least 2 blank visa pages. The original and completed EP online Declaration form duly signed by the applicant and local Employer/Sponsor; Applicant who is currently holding a/an Employment Pass, S Pass, Training Employment Pass or related pass (Dependant's or Long Term Visit Pass), and wish to change employer or pass type, is required to bring along the following documents to cancel his/her current pass: Disembarkation/Embarkation Card, or S Pass Card; Letter from the current employer/sponsor requesting for the cancellation of the existing work pass and/or related pass(es) (if any); Applicant who is currently holding a work permit has to ensure that the current employer has submitted a Cancellation request via WP Online to cancel the work permit before he/she can collect the Employment/S Pass. The employer will print a copy of the WP Online Cancellation result page. They are required to bring the following documents at the point of collection: A printed copy of the WP Online Cancellation result page with the Cancellation application status indicating "Pending Verification" (This page will appear after the employer has successfully submitted the applicant's Cancellation request); and Work Permit Card; Please note that Dependant's Pass and Long Term Visit Pass can only be collected after the Employment/S Pass has been issued.
