2 Applicatin fr a temprary resident visa checklist Cmplete and place this checklist n tp f yur applicatin. Yu must include a certified translatin with any dcument that is nt in English r French. Failure t submit all required dcumentatin may result in the refusal f yur applicatin r delay prcessing. False statements r submissin f fraudulent dcuments will result in immediate refusal. Yu must submit the fllwing items: Tw phtgraphs, meeting the phtgraph specificatins, fr each applicant (scanned phts are nt acceptable). Signed, dated and taken in the past 6 mnths. Applicants wh need t give their bimetrics d nt need t submit paper phtgraphs. Cnsent t verificatin f dcuments frm. Family Infrmatin Frm IMM 5707, fully cmpleted, dated and signed. Cmpleted and signed Schedule 1 frm (Applicatin fr Temprary Resident Visa IMM 5257B). Valid passprt frm each applicant. Yur passprt shuld be valid fr at least six mnths frm the date f travel t Canada. The validity f a visa cannt g beynd the validity f the passprt. If there are nt at least 2 blank pages, yu will have t btain a new passprt (n additinal sheets). Include cpies f all cancelled r expired passprts as prf f previus trips. If yu d nt reside in yur cuntry f citizenship, please prvide a cpy f yur wrk permit r temprary stay permit. Explain the purpse f yur trip - details f yur itinerary in Canada, prvisinal travel arrangements (airline r htel bking), letter f invitatin frm the persn r business yu will visit, registratin at a cnference, letter frm yur emplyer (as applicable). Prf f sufficient funds t cver expenses fr the duratin f yur visit. CAN+ If yu have travelled t Canada in the last ten (10) years r currently hld a valid United States nnimmigrant visa and can prvide prf (entry stamps in passprt, prf f the valid visa), n prf f financial supprt is needed. 1

If yu are travelling n business, the letter shuld als utline the purpse f yur trip, and include the names and addresses f the cmpanies yu will visit. If yu are self-emplyed, please prvide evidence f yur business (business registratin, license and financial dcuments etc.). If yu are planning t visit a friend r relative, please prvide a letter f invitatin. Fr Parents and Grandparents Super Visa, please prvide: A letter f invitatin written by yur child r grandchild which utlines the arrangements fr yur care and supprt. The letter must als indicate the number f individuals residing in the inviter s husehld. Prf f relatinship (birth certificate, baptismal certificate r ther fficial dcument naming yu as a parent) Prf that yur child r grand-child meets the Lw Incme Cut-Off (LICO) minimum Prf that yu have private medical insurance valid fr a minimum f ne year frm a Canadian insurance cmpany and that: Cvers health care, hspitalizatin and repatriatin; Prvides a minimum cverage f $100,000; and is valid fr each entry t Canada and available fr review by a prt f entry fficer. Prf f enrlment in schl - a current letter and transcript frm the educatinal institutin yu are attending. Cpies f marriage Certificate and Birth Certificates fr each family member applying fr a visa.
