1 Dear Parent/Guardian, USA Educational & Cultural Program Travel is a form of learning as it exposes an individual to cultures other than his or her own. Travelling gives an opportunity to experience foreign cultures and an ability to adapt to a globalized world. It teaches real life lessons, effective time management, planning and organizing, ability to handle uncertainty and many more things. The Sanskaar Valley School is delighted to provide you information about the Tour Abroad Programme for students of Sopaan and Shikhar during the Summer Holidays of 2017 and the proposed destination is United States of America. The tour is an interesting mix of Sightseeing, and the NASA Space Programme. This 12 nights / 13 days program is designed to inspire, excite & motivate students and an opportunity for students to work together, gain and share knowledge. During the program at the NASA facility, students will learn about space activities, interact with scientists and astronauts, perform simulated space shuttle missions, get an exclusive tour of Kennedy Space Center and witness the everyday challenges faced by the astronauts. The tour will include travel to: Orlando Washington Niagara Falls New York City Moreover, it is a unique experience that will be lasting and memories will be carried on through their life. A visit to Universal Studios, Statue of Liberty, Times Square, Hershey s Park and amazing views of Niagara Falls will take them to an unimaginable limit of enchantment, which will make the tour a truly mesmerizing experience. Program Details: 1. The program is open to school students willing to explore new ideas & horizons. The ideal age group for this program is from Grade 6 th - 12 th and is scheduled for April-May, Teacher Coordinators and Inizio organizers will accompany the students. 3. The total program fee is INR 2,50,000/inclusive of all taxes, medical & travel insurance. The cost also includes flight tickets from Bhopal to Delhi/Mumbai International Airport. 4. The group members will be charged extra for travel & 2 day stay arrangements during USA Visa Interview in Mumbai/Delhi and also for travel from City to Mumbai/Delhi International Airport and back. (Approximately Rs ). Note: - Copy of Passport (Validity till January 31, 2018) is required to be submitted with the Consent Form. Payment Policies: 1. The first installment of INR 75,000 should be deposited by cheque/draft/cash in favor of The Sanskaar Valley School latest by January 13, This is the final deadline for processing the application. 2. The second installment of INR 50,000 should be deposited by 20 th February, 2017 by cheque/draft/cash in favor of The Sanskaar Valley School. 3. Balance amount to be paid after visa approval or by March 10, Payment for Visa Interview (amount and date) will be informed once the interview date is scheduled.

6 UNDERTAKING: INIZIO EDUTECH PVT. LTD. I s/o, Father s Name d/o Grandfathers name and resident of do hereby undertake that I want to enroll my child into the Inizio Space School Program to USA with Inizio Edutech Pvt. Ltd. I will abide by the terms and conditions and their consequence as stated below: 1. Any consumption or possession of alcoholic beverages or drugs is strictly prohibited and may result in expulsion of student from the tour. The families concerned will pay for the cost of early flights back to INDIA and the student/parent as the case may be shall solely be responsible for any civil/criminal & other infringement of law and Inizio Edutech shall not be responsible for any such infringement directly or indirectly. 2. Vandalism or stealing will result in expulsion from the program unless there are extenuating circumstances. 3. Students are required to be in groups of two or more (buddy system format) when in shopping areas, swimming pools, sightseeing areas and during social functions. 4. Unlawful acts by student will result in the immediate expulsion from the tour and appropriate punishment by civil Authorities concerned. 5. Picture Policy: Inizio Edutech can publish any or all pictures, thesaurus, research, findings and videos of student's work taken during the course of this program in any publication, public relations, and/or advertising purposes, without limitation, reservation or any additional compensation. 6. Field Trips: Students will be required to participate in field trips during the course of the Space Center program. They will be put under the direct supervision and care of the Inizio staff. Participants will not hold Inizio staff, any volunteers, or Space Center responsible for any injuries or loss of property, which may be sustained by student as a direct or indirect result of participating in the Inizio Space School program. 7. Transportation Liability: Students will be transported at times during the Inizio Space School program for field trips and other special circumstances. While en route, the child will be under the direct supervision of the Inizio or their appointed/associated staff/driver/volunteers and will be subject to all regulations set for the safety of the child. Participants will not hold the Inizio/driver, Inizio staff, volunteers, Space Center or any other person responsible for any injuries or loss of property, which may be sustained as a direct or indirect result of this service. 8. Passport will remain in the custody of Inizio Edutech from day of issue of visa till the end of the tour. The trip price includes: round trip transportation, shared hotel accommodations, motor coach or buses, three meals per day while on tour or sightseeing, NASA fee, visa fee, theme park tickets, taxes and customer services. 9. Hotel accommodations: Four Students will be accommodated in a room and with students of same gender. 10. Flights: We do not guarantee non-stop flights and any special airport or airline. While we attempt to get groups their preferred airport, but can never guarantee so. Seating assignments will be made by the airlines, and upgrades, stopovers, changes and use of frequent flyer mileage are not possible. Inizio Edutech is not responsible for schedule changes, mechanical or weather delays that may occur. 11. Sponsorship: Unless otherwise noted, this tour is not sponsored by any school, school district or public entity, and is organized by a private company. Inizio Edutech reserves the right to refuse or cancel a registration/registrations at its sole discretion for any reason what so ever. 12. Behaviour: While travelling, participating students will be expected to abide by local laws, including those concerning drugs or alcohol or use/possession of any harmful article(s). They will also be responsible for any

7 personal calls or incidental personal expenses, or any damage done to the buses or hotels while on tour. Breaking these laws and rules may be the grounds for being sent home early from the trip at their expense. 13. Cancellation Policy: In the event of participant cancelling the tour after an enrolment for any reason, the standard cancellation charges remain effective as per the circular/notice issued or terms and conditions agreed by the school. In other cases but not limited to the cases mentioned below, following are the terms and conditions - In case US Embassy doesn t approve an applicant s visa then, an amount of INR 35,000 will be deducted and the remaining deposited installment will be refunded to the applicant. If case a student withdraws from the program after the issuance of USA visa, an amount of INR 75,000 will be forfeited from the submitted fees. If case a student withdraws from the program within 6 weeks of departure date, an amount of INR 1,00,000 will be forfeited from the submitted fees. In all other scenarios, an appropriate refund will be made after due consultation with school authorities. All refunds will be processed within four weeks after due cancellation announced by the company/school. 14. Additional Provisions: Inizio Edutech cannot be held responsible for events beyond its control, such as acts of God, war, terrorist activities, and incidents of political violence, civil unrest, illness or quarantine, strikes or government restrictions. We reserve the right to cancel any tour for any reason, in which case, and no other case you will receive an appropriate refund. Inizio cannot be held responsible for loss of personal property including money, and is not responsible for personal injury, property damage, or loss of earnings from any event whatsoever caused by hotels, railways, airlines and all sub-contracted services.. All parties agree that in the event of legal action concerning this agreement, all litigation shall be brought only in Patiala Court in India and that such litigation shall be decided pursuant to the laws of INDIA. 15. There is limited space available in the program. A space is not held or secured until a completed registration form and written consent is received with full payments for the program. 16. Waiver / Itinerary Confirmation: Inizio Edutech, in due consultation and consultation with the school authorities, reserves the right to make changes in fees, departure dates, departure cities, itinerary, airlines, hotels, field trips and activities. Such changes are not the grounds for withdrawal from the tour and demanding refund. Inizio may cancel a program due to insufficient enrolment, unforeseen operational difficulties, lack of projected availability or any other reasons. If Inizio Edutech cancels a trip, a full refund will be made to you within ten weeks of the cancellation date. 17. Travel & Medical Insurance Coverage: All travelers will be insured before final departure of the group. 18. In case any student, unlawfully, attempts to or flees away from the group, then his case would be reported to the nearest law authorities and a formal police report will be lodged against him. Also in the above mentioned scenario any member from the school or Inizio Edutech will not be responsible for the illicit action of the student. Also in such situation no Indian or foreign court of law can frame and file charges against Inizio Edutech or any school member on behalf of Parents. Also I agree not to present Inizio or its officers in bad light in any public forums. 19. This document can be used as a legal instrument in the court of law of India and country visited by participant under any program initiated by Inizio. 8 Document Checklist Following documents must be submitted by each student along with the Inizio Registration Card. If any of the below mentioned document(s) are missing, the application will not be accepted. o Copy of the original passport: First page, Last page and any Observation page (one photocopy to be pasted on the registration card in the space provided, other photocopy to be submitted separately). o Notarized affidavit: Please prepare the attached affidavit formats on a stamp paper of value INR 100 and it has to be notarized by a legally appointed public notary. Also both affidavits should be affixed with a special stamp/marking, making them a legal instrument in foreign land too. It says "VALID FOR FOREIGN COUNTRY EMBASSY AND HIGH COMMISSION" or "VALID FOR TRAVEL ABROAD". Both affidavits should be prepared separately. o Identity proof of both the parents: This can be a photocopy of any one of these - Passport, PAN Card, Driver s License, Ration card etc. Two photocopies for each parent is required. o Original bank statement of one of the parent s savings/current/joint account for duration of three months. The account should have a closing balance of around INR 2,00,000 and it should reflect in the statement. o Four 5*5 cm photographs, clicked on white/light background on matte paper. o Authority Letter for Counter Passport Collection Only parent/applicant name and signatures are required. The rest sections should be left blank. o Original Date of Birth Certificate for students who are below 14 yrs of age.
