1 UNICEF/2017/Patrick Bangladesh Humanitarian Situation report No.5 (Rohingya influx) Highlights 519,000 newly arrived Rohingya refugees have reportedly entered Bangladesh since 25 August. UNICEF is providing safe water to 86,000 new arrivals per day and improved sanitation to 92,000 people. Since 25 August, a total of 25,978 children under-five have been screened. Among them 492 SAM children received treatment. 3,412 Pregnant and Lactating Women (PLW) received IYCF counselling; as of 4 October, approximately 6,200 have received maternal, neonatal and child health services and treatment for diarrhoea with UNICEF support. To prevent a cholera outbreak, an Oral Cholera Vaccine (OCV) campaign has been planned by the Ministry of Health from 10 October UNICEF has procured 900,000 doses of OCV. 3,882 at-risk adolescents have been identified and received psychosocial support and life skills education. During the first week of October, an additional 1,928 Rohingya children were enrolled for early learning and non-formal basic education in 208 learning centres in five makeshift/spontaneous settlements. During that week, 23 transitional learning spaces were established. UNICEF s revised six months response plan and the inter-agency Humanitarian Response Plan (HRP) have been finalized. Out of the total US$434 million inter-agency appeal, UNICEF is requesting US$76 million to provide immediate life-saving humanitarian assistance to affected children and women. 2 Situation Overview & Humanitarian Needs As of 8 October, an estimated 519,000 1 Rohingya refugees have entered Bangladesh because of the recent escalation of violence against them in Rakhine state in Myanmar. The total number of Rohingya refugees in Bangladesh, including those who were in the country before the crisis, has now reached a total of 800,000, all residing in makeshift and spontaneous settlements in Ukhiya and Teknaf sub-districts in Cox s Bazar district. Among the new arrivals, 80 per cent are estimated to be children and women, including new-born babies and pregnant and lactating mothers. 2 According to reports from IOM 3, 58 per cent of the current influx are children and 10 per cent are pregnant and lactating mothers. The inter-agency Humanitarian Response Plan (HRP ) identified the areas of WASH, health, nutrition and food security and shelter for immediate scale-up to save lives in both settlements and host communities. Humanitarian agencies are still not operating to scale or with full coverage. In view of risks of intra- and inter-group conflicts among the newly arrived refugees and the host communities, a conflict-sensitive comprehensive approach is being advocated by humanitarian actors responding to this emergency. As per the HRP, the Rohingya population in Cox s Bazar is highly vulnerable, many having experienced severe trauma, and are now living in extremely difficult conditions. The limited WASH facilities in the refugee established settlements, put in place by the WASH sector partners including UNICEF prior to the current influx, are over-stretched, with an average of 100 people per latrine. New arrivals also have limited access to bathing facilities, especially for women, and urgently require WASH supplies including soap and buckets. Prior to the new influx in August, only 25 per cent of Rohingya had access to safe water, and the situation is even worse for the host communities, with only 10 per cent with access to safe water for drinking and personal hygiene. The entire population of the 1.2 million affected people, including children and women, need access to sufficient water of appropriate quality UNICEF/2017/Brown and quantity for drinking and personal hygiene. Given the rainy weather conditions in Cox s Bazar, there is a need to provide more than 15 million litres of safe water every day as per minimum standard. In the present context, on a life-saving basis UNICEF is distributing only 2.5 litters /person /day for drinking purpose. UNICEF is planning to scale up this quantity towards the international standards in the coming weeks based on availability of resources and partners. Given the current population density and poor sanitation and hygiene conditions, any outbreak of cholera or acute watery diarrhoea, which are endemic in Bangladesh, could kill thousands of people residing in temporary settlements. Urgent nutrition needs have been prioritized for under-five children (including infants), pregnant and lactating women and adolescent girls. These include close to 17,000 children under five suffering from SAM to be supported over the next six months. Nutrition sector partners are planning to cover 70 per cent of the identified needs in the makeshift and new settlements, host communities and official camps. Children, adolescents and women of both the Rohingya and host communities are exposed to high levels of violence, abuse and exploitation including sexual harassment, child labour and child marriage and are at greater risks of being trafficked. There are many traumatized and 1,675 separated and unaccompanied children needing psychosocial support, family tracing and reunification, and a protective environment. 1 Situation Update: Rohingya Crisis, Inter Sector Coordination Group (ISCG), 8 October UNICEF Bangladesh Revised Response Plan, October Needs and Population Monitoring Report, IOM, September

3 US$76 million 4 is required by UNICEF for the next six months to provide immediate life-saving humanitarian assistance to affected children and women. UNICEF is responding to this emergency in partnership with the Government of Bangladesh, UN agencies, and NGOs and with support from donor agencies. UNICEF s appeal is part of the inter-agency humanitarian response plan (HRP) with a total appeal amount of US$434 million. The required assistance includes emergency WASH, nutrition, child protection, health including cholera prevention and response as well as education services to vulnerable populations, mainly women and children. This response plan will be reviewed and revised after a period of six months. Estimated Population in Need of Humanitarian Assistance: Total Male Female Total Population in need 1,200, , ,000 Children (Under 18) 696, , ,880 Children Under Five 348, , ,440 Pregnant and lactating women 120, ,000 Adolescents 204,000 95, ,120 Source: Calculated based on the Needs and Population Monitoring, IOM, September 2017 Humanitarian Leadership and Coordination The overall humanitarian response for the Rohingya refugee crisis is facilitated by a sector-based coordination mechanism, the Inter- Sectoral Coordination Group (ISCG), established for refugee response in Cox s Bazar. This coordination mechanism is so far unique in the humanitarian refugee context. On the government side, a National Task Force (NTF) established by the Ministry of Foreign Affairs (MoFA) leads the coordination of the overall Rohingya crisis. However, after the recent influx since August 2017, the Ministry of Disaster Management and Relief (MoDMR) has been assigned to coordinate the Rohingya response with support from the Bangladesh Army and Border Guard Bangladesh (BGB). In this structure, the role of the Refugee, Relief and Repatriation (RRRC) Commissioner and the Deputy Commissioner (DC) of Cox's Bazar district are critical for daily coordination and information sharing. UNICEF and all other humanitarian organizations operating from Cox s Bazar are required to provide daily updates to keep district authorities informed. At sub-national level, UNICEF continues to lead sectoral coordination in the nutrition sector and child protection sub-sector and colead education with Save the Children. For WASH, UNICEF co-leads the sector along with Action against Hunger (ACF). It is important to note that the cluster system has not been officially activated. Humanitarian Strategy UNICEF s comparative advantage is its ability to work simultaneously with the Government, local and international NGOs and other civil society organizations, coordinate and mobilize their support as appropriate. UNICEF is working in close coordination with all the humanitarian actors at national and sub-national level including government line ministries and departments, such as the Department of Public Health Engineering (DPHE), to effectively scale up WASH interventions, and with the Civil Surgeon s Office ( Ministry of Health) to organize a mass immunization campaign (Measles, Rubella, Polio and Oral Cholera Vaccine). Summary Analysis of Programme Response Nutrition The pre-influx nutrition situation in Cox s Bazar district was very poor with critical level of acute malnutrition observed in the refugee settlements (GAM rate-21.2% and SAM rate of 3.6%, SMART survey, May 2017). Recent nutrition screening data and field observations by partners on the ground are indicative of a worsening of the nutrition situation with high incidence of acute malnutrition reported. There is potential for aggravation of the nutrition situation as a result of poor water and sanitation, limited health care and food insecurity conditions in the Rohingya refugee settlements. 4 UNICEF Bangladesh revised Humanitarian Action for Children (HAC) appeal will reflect the requirements and targets 3

4 UNICEF has triggered life-saving nutrition interventions for the new arrivals in the new six spontaneous settlements and in two makeshift settlements. UNICEF in collaboration with partners operationalized four outpatients therapeutic programmes (OTP) centres for management of SAM children in 4 settlements and five more OPT centres are under construction. Since 25 August, a total of 25,978 children under five have been screened. Among them 492 SAM children received treatment out of 578 identified at four OTP centres and inpatient facilities. About 69,000 children of 6-59 months received Vitamin A supplementation during supplementary immunization campaign for the Rohingya children. To date, 3,412 Pregnant and Lactating Women (PLW) received IYCF counselling. Five breastfeeding spaces out of 65 are being operationalized in Unchiprang settlements. UNICEF/2017/Brown Health UNICEF is supporting the Ministry of Health to roll out a vaccination campaign targeting all Rohingya children under 15 years against polio, measles and rubella. Measles Rubella (MR) Supplementary Immunization kicked off on 16 September 2017, targeting 250,000 children aged 0-15 years for the MR and bivalent oral polio vaccine (bopv) and Vitamin A supplementation. As of 3 October, some 207,853 children aged 0-15 years received MR and bopv (135,519 children received MR vaccines, 72,334 received polio vaccines) and 72,064 children received Vitamin A supplementation. To prevent a cholera outbreak, an Oral Cholera Vaccine (OCV) campaign has been planned by the Ministry of Health from 10 October UNICEF is providing support through the procurement of 900,000 doses of OCV along with capacity building of the district and Upazila health team and social mobilization. Twelve medical teams under the district health department have been supported by UNICEF to provide basic emergency health services to the new arrivals in the sub-districts of Teknaf and Ukhia. As of 4 October, over 2,000 patients received health services from three health care centres established with UNICEF support. In addition, over 1,000 under-five children received treatment for childhood illness including diarrhoea and pneumonia, and 741 pregnant and lactating women received antenatal and UNICEF/2017/Dian post-natal care services. 2,441 cases of diarrhoea were treated in the past week (29 September 5 October) by the medical teams and health centres supported by UNICEF. UNICEF has also provided support to Cox s Bazar District Hospital to treat sick new-born babies through a special care unit (SCANU). UNICEF provided financial support to expand the SCANU, will provide additional equipment and support capacity building of staff managing this Unit. From 29 September to 5 October, a total of 88 sick new-borns received treatment in the SCANU. Implementation capacity of the Civil Surgeon s office has been strengthened with support from one national technical expert and one HMIS consultant to keep the district control room functioning for 24/7. 4

5 WASH Nearly 1.2million people in Cox s Bazar are in need of WASH services. This is further complicated by the appalling situation in the makeshift camps and increasing number of spontaneous settlements. The WASH sector/cluster aims to reach 887,000 people for water and 950,000 for sanitation. UNICEF plans to reach 50 per cent of the sector s target population on water and sanitation and 40 per cent on hygiene for six months (September 2017 to February 2018). UNICEF s WASH response focuses on the immediate provision of safe water supply, basic sanitation and community engagement around hygiene practices. Overall, the number of refugees gaining access to safe water, sanitation and hygiene is increasing; over one third of the target population has been reached with safe water through construction of tube wells and water trucking. However, sanitation remains a challenge due to poor geophysical conditions and sludge management issues, with around 15 per cent of the target being met so far. Additionally, 11,540 new arrivals have benefited from hygiene kit distribution for enhanced hygiene practices. UNICEF remains active in WASH sector coordination at the sub-national level as well as in the camps and settlements. The WASH UNICEF/2017/Brown programme is constantly engaging with the sector to identify solutions to challenging issues such as the preparedness and response plan for a possible AWD outbreak, latrine sludge management and long-term solutions to camps with limited water resources. A new treatment plant is being constructed in one of the camps, and discussions are ongoing with two NGOs with expertise on sludge management to find sustainable solutions. A strategy for reinforcing hygiene promotion with C4D has been developed and validated by partners, aimed at delivering harmonized messages to the affected population. Major gaps still exist as more spontaneous settlements are emerging, while geographical constraints hamper timely implementation of interventions in other settlements. A cholera task force has been formed and a draft preparedness and response plan will be shared with the sector partners next week. The water quality of shallow boreholes is a serious concern as there is potential for major contamination. In new sites, physical access is a major concern in scaling up the WASH emergency response. In addition, congestion in the receiving sites is also a major concern, resulting in overburdening existing facilities, complicating access for emptying latrines and increasing the public health risk in the sites. Faecal sludge management therefore remains a high priority for the WASH sector. Child Protection Approximately 348,000 children (6-18 years) from the affected community require a safe and protective environment. Many children have experienced violence and traumatic events and need immediate psychosocial and recreational services to build their resilience. Proper identification of accompanied and separated children is necessary for family tracing, reunification and referral services. Children are exposed to many risks including negative coping mechanism, drug addition, child marriage and child labour, and require life skills, GBV information and services. The community engagement and outreach mechanism needs to be strengthened to address the needs of the most vulnerable children and adolescents. 5

6 An estimated 180,000 children have been targeted for psychosocial and recreational support and services, and 26,866 children have already been reached to date. Implementing partners have established 12 static and 64 mobile Child-Friendly Spaces in makeshift settlements, host communities and spontaneous locations in Ukhiya and Teknaf. Approximately 35,000 at-risk adolescents have been targeted; 3,882 have already been identified and received psychosocial support and life skills education. UNICEF and partners are working with sectors, other UN agencies and Government officials to identify and address the priority needs of Rohingya children. Education A total of 453,000 affected Rohingya and Bangladeshi host community children aged 4 18 years are in need of education. UNICEF has started to respond to this need by targeting 205,265 children aged 4 14 years. During the first week of October, an additional 1,928 Rohingya children of (4-14 years boys and girls) were enrolled for early learning and nonformal basic education in 208 learning centres in five makeshift/spontaneous settlements. As of 4 October, UNICEF and partners reached a total of 17,026 children (8,937 girls; 8,089 boys). During the week, 23 transitional learning spaces were established and preparatory work for the establishment of transitional learning spaces in different new spontaneous settlements has started. Over 350 School-in-a-Box, 65 ECD Kits and 31,500 school bags have been distributed to implementing partners targeting around 31,500 children. UNICEF has started to support children s education in new areas such as Unchiprang and Kutupalong extension sites. However, space is a UNICEF/2017/Brown major challenge. The operational environment is harsh, the newly arrived populations are still on the move within settlements, and qualified human resources are scarce. Interventions for immediate response in education still require standardization to ensure quality across different service providers in terms of classroom construction, teacher training, psychosocial support, etc. The education sector identified sector targets and funding requirement for the 6-month humanitarian response plan. Education remains one of the least funded areas in the on-going response. Communications for Development (C4D), Community Engagement & Accountability Through the UNICEF Information and Feedback Centre (IFC), 502 people in Unchiprang and Balukhali were reached with information about service points and delivery such as health clinics, food distributions, water and sanitation supplies, and at the same time provided feedback and concerns. While most of the complaints from communities are being addressed locally, efforts are in place to harmonize the feedback mechanism with sectors to respond to communities concerns and needs on the ground. More than 1,000 families were provided with life-saving messages on health behaviour and practice through youth and women outreach volunteers in the two camps. UNICEF is working to scale up the cholera vaccination campaign and promote appropriate hygiene behaviours. The C4D and WASH teams shared the hygiene promotion strategy with local partners to call for immediate action on awareness raising. Life-saving messages on WASH, health and protection were endorsed by the CWC working group for dissemination and shared with Radio Betar for PSA broadcasts. 6

7 Supply and Logistics Two warehouses (Rubhalls 24 x 10 m) have been erected in Cox s Bazar and are fully functional. The first two air charters of supplies have arrived. A total of 100MT and 420 cubic metres of supplies have been distributed. The supply plan to cover the first six months of emergency has been finalized and is costed at US$15 million. A first full cargo plane arrived on 28 September with supplies including family and dignity kits for 2,500 people and was distributed. The second full cargo arrived on 29 September followed by an additional air freight on 2 October with water, sanitation and hygiene supplies to reach 17,200 people with 3,440 hygiene kits and 74,450 people with 1,489 squatting plates and plastic tarpaulins. UNICEF has finalized arrangements for transportation and distribution of an additional 10,000 hygiene kits to benefit 50,000 affected people; 2,000 squatting plates for latrine construction for 100,000 people; and 6 million Water Purification Tablets (WPT) to provide safe water to 100,000 new arrivals for one month. Media and External Communication International media attention was quite high on the Rohingya settlements due to the ongoing influx. UNICEF received high coverage through interviews and reports on its interventions on the ground. More than 10 press releases have been issued in local and international media on the crisis since 25 August highlighting humanitarian interventions, supply delivery and the appeal. Two fundraising appeals for Rohingya have been launched both at local and HQ level. Currently a communication team is on the ground to collect video messages, photos and stories for HQ publication. The visit by the UNICEF Executive Director was well covered in local and international media. Security The overall security situation has deteriorated since August Children, adolescents and women of both the Rohingya and host communities are exposed to high levels of violence, abuse and exploitation. The security of children, women and adolescent girls became critical and the risk of sexual violence and abuse and trafficking increased. Inter-agency focus groups in the community were set up as preventive mitigation measures. The Bangladesh Army, Border Guard Bangladesh and Bangladesh Police are keeping a close eye on the overall security situation. The Army has requested that all humanitarian staff wear ID cards. Strangers without ID cards will be arrested as per Bangladesh section 144 which is a Code of Criminal Procedure. No one will be allowed in the camps after dark. The district administration and security agencies have asked for the list of all staff of UN agencies, INGOs, NGOs, private and voluntary agencies. Funding As part of UNICEF s original 2017 Humanitarian Action for Children (HAC) regional appeal for South Asia launched in January 2017, US$9.45 million was required to respond to the humanitarian needs in Bangladesh. Following the recent influx of Rohingya and to deliver immediate life-saving humanitarian assistance, the estimated UNICEF s revised response plan requires US$76.1 million to provide life-saving and other services to over half a million children, which includes both the existing, new influx and the vulnerable host community children. UNICEF wishes to express its sincere gratitude to CERF, Japan, Canada, Sweden, Denmark, the United States, the UK and various National Committees who have contributed generously to the humanitarian response in Continued and timely donor support is critical to scaling up the response in Bangladesh.
