1 Background Check

2 Effective January 1, 2016, all prospective employees, interns, volunteers and contractors of Department of Developmental Services (DDS) licensed or funded programs must complete a fingerprint-based background check of the state and national criminal history databases. All offers of employment are contingent upon passing all background check requirements for your position and program. In addition to other Seven Hills background check requirements, including but not limited to CORI, RMV, pre-employment physical and drug test, applicants for employment must register for an appointment via the IndentoGO by Morpho Trust USA registration system.

4 The following is an overview of the fingerprinting process: Upon notification by Seven Hills Foundation, an applicant must register for a fingerprinting appointment via either the Morpho Trust USA (IndentoGo) registration website or the Customer Service Telephone Center. 5 Applicants must schedule an appointment before they are fingerprinted. To register for the appointment the applicant will need a DDS Person Identification Number that will be generated by DDS and sent to Seven Hills, once they have completed the CORI check. This DDS Person Identification Number will be provided to the applicant by a Seven Hills representative (hiring manager or administrative personnel). Upon receiving this Person Identification Number the applicant must call or go online within 48 hours to schedule your appointment.

6 Once the appointment is scheduled, if the applicant needs to reschedule their appointment they must contact Morpho Trust directly at The applicant can pay by credit card, personal check or money order. The cost for fingerprinting is $ Once hired and all of the background checks are completed and cleared, Seven Hills will reimburse the applicant for the cost of the fingerprinting ($45.00).

7 The applicant must bring their DDS Person Identification Number with to the fingerprinting appointment. The applicant must also bring an acceptable form of identification. Fingerprinting will not be taken without an acceptable form of identification. Acceptable forms of identification include: Driver s License from any U.S. state or territory Valid State Identification Card from any U.S. state or territory U.S. Passport or U.S. Passport Card Permanent Resident Card or Alien Registration Receipt Card Foreign Passport and Form I-94 or Form I-92A

8 All IDs must include an identifiable photo, the applicant s full name, and date of birth. All documents must be verifiable and unexpired. A complete list of acceptable forms of identification are listed on the Morpho Trust website. Upon completion of the fingerprinting, the national background check will be processed. The applicant will be contacted by Seven Hills when their national background check is complete.

11 Ways that you can expedite the process: 1. Explain the process to your prospective employees in addition to providing them with the letter that HR developed. 2. Help your applicants schedule their appointments after you receive their DDS Personal Identification Number. 3. Make certain you have a personal for your applicants. 4. Follow up and check on their status in the Virtual Gateway.
