1 NORWAY VISA APPLICATION CHECKLIST The Norwegian Embassy has provided guidance to help you submit your application. This guidance is available on the following website. orwww.selfservice.udi.no This form will be used by the staff member who accepts your visa application to record the documents you have submitted with your application. PLEASE NOTE: 1. Children below 18 years of age should attach a full birth certificate and consent letter if traveling alone or with one parent. 2. Please ensure that passport is signed. 3. Applicants should submit in person. 4. Applicants should come with their original documents for submission. 5. Any other additional documents may be asked by VFS staff based on Embassy requirements. MANDATORY REQUIREMENTS Completed online Application form, the online cover letter, online receipt of 60 Euros and an equivalent of 30 Euros payable in UGX at VFS upon submission. A valid passport in respect to each applicant with at least 2 clear pages and should be valid for at least a period of 3 months from date of return Travel Health insurance (must cover from date of departure to date of return) Insurance companies accepted by the Norwegian Embassy: UAP, AIG, National insurance Corporation, BUPA, Jubilee insurance, East African underwriters,britam Flight Reservation Children s Birth certificate (if applicable) Photocopy of Bio- data page of your current passport and previous Schengen, UK,USA,Canada and Australian visas(if any) One passport size photograph(45*35mm)not more than 6 months old, coloured with a white or clear background For foreign nationals, work-study or residence permit valid for at least 6 months and above endorsed in passport Signed letter of invitation Marriage certificate (if applicable) Letter of introduction from employer, organization or school. Applies to only thosebelow 18 years: Consent letter Copy of parents ID Death certificate if deceased

2 From applicants 1 Pay slips for at least 4 months 2 Bank statements for the last 4 months FAMILY AND FRIENDS VISITS 3 In case of family visit documentation of relationship 4 Invitation forms For Norway:UDI guarantee form For Sweden: Duly filled and signed Referees appendix from host(vu1) For Denmark:VU2 Invitation form For Lithuania: Invitation letter For Switzerland: Invitation Letter For Iceland: Invitation letter From Host 1 Invitation letter, letter stating type of relationship, purpose of visit and period 2 Copy of hosts passport 3 Documentation of relationship 4 In case of boyfriend/girlfriend documentation of relationship e.g. pictures, entry /exit stamps or visas. 5 For Iceland: Invitation letter For Switzerland: invitation letter (if the host is working for an International Organization, or Diplomatic Mission in Geneva or Bern, a clear copy of host s valid carte de legitimation and confirmation of employment signed by the International Organization or Diplomatic Mission must be provided. 5 OFFICIAL VISITS 1 Official invitation letter from inviting organization signed and addressed to the Embassy stating purpose of visit, period of visit, and the party covering flight ticket and accommodation expenses. 2 Original Note Verbal from the Ministry of Foreign Affairs (Diplomatic Note) 3 Letter of introduction from employer stating work relationship with the applicant. If the employer is sponsoring the travel, it should be indicated. 4 Valid resident or work permit for non-ugandans. Signature: Initials:

6 BUSINESS VISITS 1 Official invitation letter from business partner/organization/institution. If the inviting party is covering travel costs, it should be indicated. 2 Letter of introduction from employer/organization/institution. Indicating purpose of visit and covering of expenses for flight ticket and accommodation 3 Registration of company documents 4 Copy of ID and bank statements of sponsor for the last 4 months (applicable for company in Uganda) 5 Pay slips and bank statements of applicant for the last 4 months 6 Hotel reservation 7 Family Appendix D Invitation form for business for Denmark(VU1) Duly filled and signed Appendix A for Sweden Signature: Initials:
