Import Permit - An import permit must be secured by directly communicating with: Animal Health Division Bureau of Animal Industry Department of Agriculture Visayas Ave., Diliman Quezon City, Philippines Tel: (632) ; ; Fax: (632) giving pertinent information of the pet, i.e. species, sex, age, quantity, etc. The Animal Health Division shall issue the import permit and send the same to the applicant. This permit together with other documentation should be presented at the port of entry upon arrival of the animal. 2. Health Certification - Pet owners should also secure a health certificate from a duly licensed veterinarian certifying that the animal is free from, and has not been recently exposed to any dangerous or communicable disease, and that it has been given antirabies and other required inoculations. (N.B. Date of certificate should not be more than 30 days before arrival of the pet in the Philippines). This certification should be presented to the Philippine Consulate General for legalization shortly before the shipment of the animal together with a copy of the veterinarian s license. If a copy of the veterinarian s license is not available or if the request for legalization is requested by mail, pet owners should likewise secure a certification from the appropriate agency in Canada which has jurisdiction over the concerned veterinarian. Said certificate should state, among others, that the veterinarian is licensed to practice within its jurisdiction and that his signature, as appearing on the certificate, is authentic. 1

If authentication is requested by mail, please enclose a self-addressed stamped envelope. Do not send cash by mail. How do I go about having my documents authenticated by the Consulate General? When persons executing or making the declarations in the document (e.g., power of attorney, affidavits, certifications, contracts) appear in person and sign the document before a Consular Officer at the Consulate General, the document will be notarized by the Consular officer in the form of an Acknowledgment, Jurat or Certification, as the case may be. The Consular Officer in this case performs the functions of a Notary Public. Hence, individuals personally appearing before a Consular Officer are no longer required to have their documents notarized by a Notary Public before submitting the same for notarization. When persons executing the document could not appear in person before a Consular Officer at the Consulate General, the document should first be notarized by a Notary Public. The signature of the Notary Public shall be authenticated by the Office of the Lieutenant Governor of the Province, or in the case of the province of Alberta, by the Deputy Provincial Secretary s Office of the Province of Alberta located at Street, Edmonton, AB T5K 2E8 (telephone numbers ) The Consulate does not take any responsibility for the contents of the documents it authenticates. Following are the requirements that you should submit when having documents authenticated/legalized: For Authentication: Original and one (1) photocopy of the document; Photocopy of an ID of the person submitting the document to the Consulate; 2

DO NOT SEND CASH IN THE MAIL Acknowledgment (documents presented and to be signed at the Consulate such as Special Powers of Attorney, General Powers of Attorney, Deeds of Sale of Property, Deeds of Donation, etc.) Original and one (1) photocopy of the document; Two (2) photocopies of 2 different forms of identification wherein your photo and your name as used in your document is shown such as passport, citizenship card or driver s license of the person(s) who will execute the document; The fee of Can $ (Cash or Money Order payable to the Philippine Consulate General Calgary) For Jurat (documents such as Affidavits of Support and Consent, Parental Travel Permits, Complaints, Affidavits of Illegitimacy, Applications for Marriage License and Oaths of Professionals) 1. Original and one (1) photocopy of the document; 2. Two (2) photocopies of the identification wherein your photo and your name as used in your document is shown such as passport, citizenship card or driver s license of the person(s) who will execute the document; 3. The fee of Can $ (Cash or Money Order payable to the Philippine Consulate General Calgary) For Certification (document issued by the Consulate certifying that the applicant personally appeared at the Consulate for SSS/GSIS purposes) The original and two (2) photocopies each of the following: (a) Passport; (b) Citizenship card - if the applicant is a Canadian citizen; and (c) British Columbia Care Card; A fee of Can$ (Cash or Money Order payable to the Philippine Consulate General Calgary) is charged per document authenticated. The processing time for this type of service normally takes three (3) working days counted from the receipt of application, provided the requirements and procedures listed above are complied with. The Consulate is NOT RESPONSIBLE for delay or loss in mail or any other uncontrollable circumstances surrounding acknowledgment or authentication of your documents. 3

4 Applicants who applied in person but who want their documents mailed back to them should also submit a self-addressed Canada Post registered mail or ExpressPost envelope along with the regular requirements. What documents are required for the shipment of human remains and urns for repatriation to the Philippines? Aside from the accomplishment of the Report of Death of deceased Filipino citizen, a Mortuary Certificate is also required for the shipment of human remains or urns for repatriation to the Philippines. The following documents for the Shipment of Human Remains should be submitted (original and two (2) photocopies): Death Certificate issued by the Vital Statistics Office; Embalmer s Certificate stating that the body was duly embalmed and is in sanitary condition for shipment abroad; Mortuary Certificate stating that the casket contains only the body of the deceased; Certification from the attending physician stating that the cause of death was not a contagious disease*; Burial or transit permit issued by the local state office; Photocopy of the identification page of the deceased s passport; Flight itinerary of the remains, including the name address and telephone number of the receiving funeral home or consignee in the Philippines; address and phone number of the next of kin in Canada and in the Philippines; and Processing fee of Can$ Duly accomplished Report of Death (ROD) for Filipino Citizens Note that bodies of persons who died as a consequence of any communicable disease should be properly embalmed and placed in a hermetically sealed metallic casket, which should be enclosed in a wooden coffin or case and securely screwed or nailed and carefully disinfected. In requesting the shipment of urns, the following documents should be submitted (original and two (2) photocopies: Death Certificate by the Vital Statistics Office; Medical Certification of Death issued by the physician; Cremation Certificate; Certification from the crematorium stating that the urn contains only the ashes of the deceased; Photocopy of the identification page of the deceased s passport; 4

5 Name and flight schedule of the person accompanying the cremated remains to the Philippines; and Fee of Can$ For a deceased Filipino citizen, a duly accomplished Report of Death (ROD) - please refer to the Report of Death form for the separate set of requirements and applicable fee. In order to ensure timely and expeditious processing of the accompanying documents, the requesting party should submit all the original documents required and the processing fee as soon as possible. Payment in cash, company check and postal money order is accepted. Can I Take My Oath of Profession at the Philippine Consulate General? Professional Regulation Commission (PRC) has enacted Resolution No Series of 2006 for new Filipino professionals (those who have passed or recently passed board examinations) who left for abroad after taking their board exams in the Philippines and before the exam result is released and wants to register with the PRC. Briefly, PRC Resolution No Series of 2006 is to allow Filipino professionals based outside the Philippines to apply for initial registration with the PRC. Prior to this resolution, all professionals had to personally appear at the PRC to fill-out all entries and affix their signature in the registry book during initial registration. Filipinos who have passed or recently passed board examinations in the Philippines and have left for abroad without registering their names at the PRC Registry of Professionals can file for initial registration through their duly authorized representatives subject to the following requirements and conditions: 1. Personal appearance by registrant at the Philippine Embassy or Consulate in the country of residence 2. Requirements: - Certificate from the PRC indicating that the registrant had passed the Board/Professional Examination (present original and submit photocopy) - Duly-accomplished Oath form or Panunumpa ng Propesyonal form required by the PRC (download it from the PRC website). 5

6 The registrant should take the oath in the presence of the Philippine Embassy or Consulate and affix a passport photograph (colored with white background) to the oath form. Present original and submit a photocopy. 3. The registrant shall designate his/her representative in the Philippines and execute a Special Power of Attorney (SPA) preferably before the same officer of the Philippine Embassy or Consulate who administered his/her oath. The SPA should expressly declare that his/her representative is fully authorized to accomplish, sign and execute documents, and perform all acts necessary for his/her registration. 4. The registrant is required to go to the PRC to personally sign the Registry Book of Professionals as soon as he/she arrives from overseas. PRC Resolution No Series of 2006 Do I have to schedule an appointment for my Oath Taking of Profession with the Philippine Consulate General? Oath Taking of Professionals are scheduled every Tuesday at 2:00 pm. Only those new professionals who have satisfactorily submitted all the required documents will be called in for this oath taking. 6
