Bonnie Campbell was issued a Washington Education Certificate, No J, on August 19, That certificate currently remains valid. 2. On October 31, 2001, tbe Office of Professional Practices, OSP, received a complaint letter from tbe superintendent of tbe Peninsula School District alleging a violation of the Code of Professional Conduct, Washington Administrative Code (WAC) , by Bom1ie Campbell; OPP No. D On September 7, 2001, Bomlie Campbell was put on suspension with pay pending the outcome of an investigation into allegations of misconduct concerning her handling of a special services student ndividual Education Plan (EP); OPP No. D On October 29, 2001, Superintendent James J. Coolican issued a letter of Reprimand and Suspension to Bomlie Campbell for falsifying and altering information in regards to student EPs. No student was denied meaningfctl educational benefits as a result of the altered documents; OPP No. DOl Bmmie Campbell was reassigned to teach alternative education at the high school. Ms. Campbell was so successful on this assignment the district decided not to return her to her previous assigmnent; OPP No. DOl On February 10, 2003, OPP issued an Agreed Order of Reprimand to Bonnie Campbell; OPPNo. DOl BONNE CAMPBELL FNAL ORDER OF SUSPENSON -2-

On October 13, 2005, Bonnie Campbell was cited and taken into custody by the California Highway Patrol for Driving Under the nfluence and No Driver's License in Possession, California Penal Code (CPC) 23152(a)(b) and 12951(a); California Highway Patrol Citation No AQ and Tuolumne County Superior Court No. CRM On November 4, 2005,.a Complaint was filed in the Tuolumne County Superior Court, based on the October 13, 2005 arrest, charging Bonnie Campbell with Driving Under the nfluence of Alcohol or Drugs, Driving While Having a 0.08% or Higher Blood Alcohol and Possession of License, CPC 23152(a)(b) and 1295l(a); Tuolumne County Superior Court No. CRM On November 10, 2005, Bonnie Campbell plead guilty to Reckless Driving, based on the October 13,2005 arrest, CPC 23103; Tuolumne Cotmty Superior Court No. CRM On November 15, 2006, Bonnie Campbell was arrested by the Santa Clara Police Department for Driving Under the nfluence, Hit& Run Property Damage m1d Driver's License Suspended or Revoked after officer's responded to a hit and run collision, Califomia Vehicle Code and CPC 23152(a) and 1460l.l(a); Santa Clara Police Department ncident No and Santa Clara Superior Court No. CC BONNE CAMPBELL FNAL ORDER OF SUSPENSON -3-

On December 19, 2006, a Misdemeanor Complaint was filed in the Santa Clma County Superior Court, based on the November 15, 2006 arrest, charging Bonnie Campbell with Driving Under the nfluence of Alcohol, Driving with a Blood Alcohol Level of 0.08 or More, Driving When License Suspended or Revoked, and Driving When Privilege Suspended or Revoked for Refusal of Chemical Test, CPC 23152(a)(b), 1460l.l(a), (a); Santa Clara Superior Court No. CC On January 8, 2007, the California Highway Patrol arrested Bonnie Campbell for Driving Under the nfluence, Driving Under the nfluence with BAC.08 or Higher, Suspended License, No nsurance after responding to a solo vehicle non-injury traffic collision, CPC 23152(a)(b), (a) and 16028(c); California Highway Patrol Citation No CU and Merced County Superior Court No. MM21 () On January 30, 2007, Bonnie Campbell plead guilty to Driving with a Blood Alcohol Level of 0.08 or More and Driving When License Suspended or Revoked, based on the November 15, 2006 arrest, CPC 23152(b) and (a); Santa Clara County Superior Court No. CC On May 2, 2007, an Affidavit of Court Probation Violation and Order to Appear was filed in the Tuolume County Superior Court, based on the October 13, 2005 arrest. On May 18, 2007, a Criminal Complaint was filed in Merced Cotmty Superior Court, based on the January 8, 2007 arrest, charging Bonnie Campbell with Driving Under the nfluence of Alcohol or Drugs, Driving at.08 or Above, Driving when Privilege Suspended or Revoked, aod Failure to Provide Proof of Finaocial Responsibility for Vehicle, CPC 23152(a)(b), (a), 16028(a); Merced County Superior Court No. MM On September 1; 2007, Bomrie Campbell was arrested by the Tuolumne County Sheriffs Office for Disorderly Conduct: Alcohol after being fotmd heavily intoxicated, passed out, in her vehicle in front of Chicken Raoch Bingo; CPC 647(f); Tuohunne County Sheriffs Office ncident No aod Tuolumne County Superior Court No. CRM On September 7, 2007, a Complaint was filed by the Tuolumne County Superior Court of California, based on the September 1, 2007 arrest, charging Bonnie Campbell with Public ntoxication, CPC 647(f); Tuolumne County Superior Court No. CRM On or about September 27, 2007, Bomlie Campbell was charged with Failure to Appear for willfully and unlawfully failing to appear on a charge of Public ntoxication, based on the September 1, 2007 arrest, CPC 647(f) and 1320(a); Tuolumne County Superior Court No. CRM BONNE CAMPBELL. On November 3, 2007, Bonnie Campbell was arrested by the Tuolumne County Sheriffs Office for Public ntoxication after Black Oak Casino security found B01mie Campbell collapsed ol.rtside the front door of the casino, complaining that she had been beaten, CPC 647(±) 1320(a); Tuolunme Cotmty Sheriffs Office ncident No and Tuolumne Cotmty Superior Court No. CRM On November 13, 2007, a Bench Warrant was issued by Santa Clara Superior Court for Bonnie Campbell's arrest, based on the November 15, 2006 arrest; Santa Clara Superior Court No. CC On November 14, 2007, a Complmnt was filed in Tuolumne Cotmty Superior Court, based on the November 3, 2007 arrest, charging Bonnie Campbell with Public ntoxication, CPC 647(±); TuolUimte Cotmty Superior Court CRM On or about November 28, 2007, Bonnie Campbell was charged with Failure to Appear for willfully and unlawfully failing to appear on charges of Public ntoxication and Failure to Appear, based on the September 1, 2007 arrest, CPC 647(±) and l320(a); Tuolunme County Superior CoUii No. CRM BONNECAMPBELL FNAL ORDER OF SUSPENSON -6-

On or about November 28, 2007, Bonnie Campbell was charged with Failure to Appear for willf1.jlly and urtlawfully failing to appear on a charge of Public ntoxication, based on the November 3, 2007 arrest, CPC 647(f) and 1320(a); Tuolumne County Superior Court No. CRM On or about July 2, 2008, Bonnie Campbell was charged with Failure to Appear for willfully and unlawfully failing to appear on charges of Public ntoxication and 2 colmts of Failure to Appear, based on the September, 2007 arrest, CPC 647(f) and 1320(a); Tuohunne County Superior Court No. CRM On or about July 2, 2008, Bonnie Campbell was charged with Failure to Appear for willfully and unlawfully failing to appear on charges of Public ntoxication and Failure to Appear, based on the November 3, 2007 arrest, CPC 647(f) and 1320(a); Tuolumne Com1ty Superior Court No. CRM On or about July 2, 2008, Bonnie Campbell was charged with Failure to Appear for willfully and lmlawfully failing to appear on a charge of Public ntoxication, based on the October 13, 2005 arrest, CPC 647(f) and 1320(a); Tuolumne County Superior Court No. CRM On November 30, 2009, Bom1ie Campbell pled Nolo-Contendere to Driving with a Blood Alcohol of.08 or Above, based on the January 8, 2007 anest, CPC 23152(b); Merced County Superior Court No. MM BONNE CAMPBELL FNAL ORDER OF SUSPENSON -7-

On or about December 10, 2009, Bonnie Campbell was charged with Failure to Appear for willfully and unlawfully failing to appear on charges of Public ntoxication and 3 counts of Failure to Appear, based on the September 1, 2007 arrest, CPC 647(f) and 1320(a); Tuolumne Cmmty Superior Court No. CRM On or,about December 10, 2009, Bonnie Campbell was charged with Failure to Appear for willfully and tmlawfully failing to appear on charges of Public ntoxication and 2 cmmts of Failure to Appear, based on the November 3, 2007 arrest, CPC 647(f) and 1320(a); Tuolumne County Superior Court No. CRM On December 28, 2009, CCTC issued Bonnie Campbell a Notice of Committee Recommendation stating the Committee of Credentials found probable cause to recommend the suspension of her teaching credential(s) and all other certification documents for a period of sixty days as a result of her misdemeanor convictions in 2007 and On or about March 3, 2010, Bonnie Campbell was charged with Failure to Appear for willfully and unlawfully failing to appear on charges of Public ntoxication and 3 counts of Failure to Appear, based on the November 3, 2007 arrest, CPC 647(±) and 1320(a); Tuolunme County Superior Court No. CRM BONNE CAMPBELL FNAL ORDER OF SUSPENSON -8-

On March 11, 2010, CCTC adopted the recommendation of the Committee of Credentials and suspended B01mie Campbell's certification documents for a period of sixty days, effective April10, 2010 through June 8, On June 24,2010, the Office of Professional Practices, within OSP, received information through the National Association of State Directors of Teacher Education and Certification, that. the state of California had taken action against B01mie Campbell's teaching credentials. After reviewing information pertaining to the CTCC action, OPP determined not to open an investigation at that time; OPP No. Hl On October 15, 2010, Bonnie Campbell was booked into the Stanislaus County Sheriffs Department on 3 cotmts of Under nfluence Alcohol/Dmgs, 9 counts of Failure to Appear and Recldess Driving after being found passed out drunk on the grass near a park bench, CPC 647(f), 1320(a) and 23103(a); Stanislaus County Sheriffs Department No and Stanislaus County Superior Court No. MPD On or about November 15, 2010, Bonnie Campbell was charged with Failure to Appear for willfully and tmlawfully failing to appear on charges of Public ntoxication and 4 counts of Failure to Appear, based on the November 3, 2007 arrest, CPC 647(f) and 1320(a); Tuolumne County Superior Court No. CRM BONNE CAMPBELL FNAL ORDER OF SUSPENSON -9-

On or a about November 15,2010, Bonnie Campbell was charged with Failure to Appear for willfully and tmlawfully failing to appear on charges of Public ntoxication and 5 counts of Failure to Appear, based on the September 1, 2007 arrest, CPC 647(f) a:pd 1320(a); Tuolumne Com1ty Superior Court No. CRM On November 16, 2010, a warrant for Bonnie Campbell's arrest for failure to appear was issued by the Tuolumne Cotmty Superior Court, based on the October 13, 2005 arrest, CPC 1320(a); Tuolurrme Com1ty Superior Court CRM On December 8, 2010, a Criminal Complaint was filed in the Sta11islaus Com1ty Superior Court, based 011 the October 15, 2010 arrest, charging Bonnie Campbell with Disorderly Conduct, CPC 647(f); Stanislaus County Superior Court No. MPD On Jm1e 29, 2012, CCTC issued Bmmie Campbell a Notice of Committee Recommendation stating the Committee of Credentials found probable cause to recommend the. revocation of her teaching credential(s) and all other certification documents and denial of any application which may be submitted to the CCTC. 41. On October 5, 2012, CCTC issued Bonnie Campbell a notice revoking all credentials, certificates, permits or other documents authorizing employment in the public schools of California. On February 15, 2013, the Office of Professional Practices, within OSP, received information through the National Association of State Directors of Teacher Education and Certification, that the state of California had talcen action against Bonnie Campbell's teaching credentials. 43. On July 9, 2013, OPP requested documentation from Santa Clara County Superior Court, Stanislaus County Superior Court. and Tuolumne County. Superior Court concerning Boru1ie Campbell; court documentation was received from each of the agencies. 44. Good moral character and personal fitness is a continuing requirement to maintain a Washington Education Certificate. BONNE CAMPBELL FNAL ORDER OF SUSPENSON --

12 CONCLUSONS OF LAW 1. OSP has jurisdiction over Bonnie Campbell and over the subject matter of this action. 2. OSP has shown by clear and convincing evidence that the Washington Education Certificate No J, of Bom1ie Campbell, should be suspended for tmprofessional conduct and/or lack of good moral character or personal fitness. 13 ORDER THEREFORE, it is hereby ordered that the Washington Education Certificate, No J, of Bonnie Campbell be suspended for twenty-four (24) months. RENSTATEMENT will require: (1) successful completion of, or continued compliance in an alcohol treatment program which is mutually agreed upon between Bonnie Campbell and OPP; (2) that Bonnie Campbell provide written consent for OPP to release documents to the provider for purposes of completing the evaluation; (3) that Bonnie Campbell will provide OPP with evidence of his/her successful completion of or continued compliance in his/her treatment program and; ( 4) if requested, Bonnie Campbell will sign consent forms authorizing OPP to have access to all records pertaining to her treatment and to discuss any and all treatment nndertalcen with the providers administering treatment. The cost of conformance to all reinstatement requirements will be the responsibility of Bonnie Campbell AND Reinstatement shall (also) require submission of a new application, including Character and Fitness Supplement, provided by OPP and having Bonnie Campbell fingerprints be checked by both the Federal Bureau of nvestigation (FB) and the Washington State Patrol (WSP). Reinstatement shall also be contingent upon Bom1ie Campbell's fingerprint backgrdlmd check returning with no criminal convictions that are listed in WAC , RCW 28A , any felony convictions.
